Eligibility Requirements (Pg. 2)-2015 Wellness Program Enrollment Form (Due November 2014/Last chance is at the 2015 Launch Presentation)
-New Hire (Hired before May 1st 2015 must complete “All” requirements, however those hired after May 1st 2015 will be expected to do as much as they can if signed up for the program).
-Health Assessment (Must be completed April 1st – May 1st)
-Health Assessments will be accepted NO LATER than May 1st
-On-line or Paper form
-Blood Work (Deadline June 1st)-Done on site May 1st – June 1st
-(Any blood work completed and submitted to One Stop Wellness January 1st - June 1st, 2015 will be accepted for the Wellness Program).

Launching of 1st 8 Week On-going Health Management Program “Celebrity Weight Loss”
January 26th – March 20th
“Celebrity Weight Loss” – Keep your identity and pick the celebrity of your choice and compete for the greatest % of weight lost. For a second twist and to benefit those who are of a healthy weight, compete to maintain over the course of the program.
2nd Quarter (Pg. 3 & 4)Launching 2nd 5 Week On-going Health Management Program “Targeted… Triglycerides & HDL”
March 30th – April 30th
“Targeted… Triglycerides & HDL” – Take advantage of the opportunity to “Target” two of the numbers on your blood work for cold hard cash. If your Triglycerides and HDL is already in range, simply maintain this number. If it is out of range, simply improve it by the designated percentage. You will be rewarded for each or both of these numbers if you meet the criteria of the program.
Health Assessments (April 1st – May 1st 2015)Blood Work (May 1st – June 1st 2015).
3rd Quarter (Pg. 3 & 4) Launching 3rd 8 Week On-going Health Management
Program “Body Mechanics” July 6th – August 28th
“Body Mechanics” – Look at the management of your body to improve/maintain optimal function. You don’t know how much you love it until you lose it. This program will give an opportunity to evaluate the ergonomics of joints and common painful problem areas. Learn new ways to manage. In addition, you’ll learn how to minimize pain and strengthen areas that bother you.
Delivery of Results/Health Coaching (August 2015)
4th Quarter (Pg. 3 & 4) October – All about the Ladies… We’ll celebrate the
ladies and focus on women’s health issues
November – All about the Fellas… We’ll celebrate the men and focus on men’s health issues.
Deadline for Completing 2015 Qualifying Events (November 1st, 2015)
Reenrollment into the 2016 Wellness Program (November 2015)
You must have completed your 2015 Eligibility Requirements to receive a benefits discount in 2016
