Medicines Management 2012, the 38th SHPA National Conference
The exciting theme for Medicines Management 2012 is ACTion & Influence. We extend this invitation to all pharmacists, pharmacy technicians and associates practising anywhere within the Australian health system, challenging you to reflect on your prACTice, share your experiences and showcase the range of ACTivities that have a positive Influence on our patients.
Our population is aging and the burden of chronic illness is growing. Update your clinical knowledge in treating these diseases, and learn how pharmacy prACTice is changing as the management of chronic disease changes. Get a better understanding of the patient’s perspective as they journey through the health care system.
Healthcare is adapting and evolving due to the impACT of chronic disease. With this year’s Conference held in Canberra, we will explore healthcare reform issues and decisions made in our Nation’s Capital that affect medicines management. As pharmacists and pharmacy technicians we are important stakeholders and have the ability to Influence change and ACTively shape our future!
As always, we have a diverse mix of national and local plenary and invited speakers ready to challenge you with fresh ideas and relevant insights through quality research and experience. ACCOMMODATION Special accommodation rates have been negotiated for Conference delegates at a number of hotels within close proximity of the Conference venue. To receive the rates below, bookings must be made via the Conference Secretariat and not with the hotels directly. Accommodation is only available for the Conference dates, but can be organised outside these dates on a request basis.

ACCOMMODATION
HOW TO BOOK YOUR ACCOMMODATION
Please indicate on the registration form your accommodation requirements, and please list hotels in preferential order 1, 2, 3 etc. As there is limited space available at each hotel, it is important that you book early. Accommodation will be allocated strictly in order of receipt of registration form and payment.
EARLY ARRIVAL OR LATE DEPARTURE
Please note that check in time for the Conference hotels is from 2.00pm. Please indicate or notify the Conference Secretariat in writing if you will be arriving early, as it may require you to book the night prior to guarantee immediate access to your hotel room. Please also indicate or notify the Conference Secretariat in writing if you will arrive at your hotel after 6.00pm, as failure to do so may mean your room will be released.
CHANGE OF BOOKING
Any change to a reservation must be notified to the Conference Secretariat in writing and not directly to the hotel.
CANCELLATION
Cancellations must be advised in writing to the Conference Secretariat. Cancellations received within 30 days of arrival may attract a penalty of one night’s accommodation. Cancellations within 48 hours or failing to arrive on the specified check in date may incur a cancellation or ‘no show’ fee, which can be up to the equivalent of the full value of the booked accommodation.
PAYMENT OF ACCOMMODATION
No booking can be confirmed without a credit card guarantee or a pre-payment. You need to pay the balance of your accommodation account and any incidental expenses incurred during your stay (for example, mini bar, telephone or dry cleaning) when checking out of the hotel.
CREDIT CARD GURANTEE
Your credit card details will be forwarded to the hotel to secure your reservation. No amount is deducted from the credit card until you check out of the hotel.
ACCOMMODATION PRE-PAYMENT
If you prefer, you may pre-pay the total room cost. Please indicate how much you wish to pre-pay on the registration form. A minimum deposit of one room night must be covered by pre-payment.

ABOUT CANBERRA
Canberra, the Aboriginal word for ‘meeting place’, is the capital city of Australia. Designed around three lakes, Canberra is filled with parks and gardens and is surrounded by mountain ranges. Its distinct seasons make it a picturesque and ever-changing city. Canberra has more restaurants per capita than any Australian city and boasts some excellent local wineries.
The city is home to many of Australia’s national cultural institutions and iconic buildings, all of which are located within a short distance from the Conference hotels.

GENERAL INFORMATION ACKNOWLEDGEMENT/COMMUNICATION
All communication with registered delegates will be by email. All registrants will be sent a confirmation letter and tax invoice within 3 working days of submitting a registration form. If you do not receive the email, please check your spam folder prior to contacting the Conference Secretariat as it might have been viewed as spam mail by your spam filter.
On receipt of your registration, a confirmation letter will be emailed to you. It is advisable to check all items listed. Any changes or alterations can be made in writing to the Conference Secretariat.
If your email address changes after completing the registration form, be sure to advise your new email address to the Conference Secretariat.
When you register you will be provided with an online access key. This will enable you to edit your details online via the Conference website www.mm2012shpa.com until Friday 28 September 2012.
CANCELLATION AND REFUND POLICY
Any changes or cancellation to both registration and accommodation must be notified in writing to the Conference Secretariat [email protected] or fax to 02 6285 1336.
Registration cancellations received in writing by 21 September 2012 will be accepted and registration fees will be refunded less an administrative fee of A$100. Cancellations received after 21 September 2012 will not be refunded. However it is acceptable to transfer your registration to another person. The full name and details of the person that will replace you must be advised in writing to the Conference Secretariat prior to the Conference.
No refunds will be made for non-attendance to the Conference.
CHILD MINDING
Please note that no official arrangements have been made for child care during the Conference.

DRESS
The dress for all scientific sessions and social functions is smart casual. Theme Dress for the Gala Dinner is ACTion heroes and Influential People.
INSURANCE
Registration fees do not include personal travel insurance of any kind. It is strongly recommended that a travel insurance policy be taken out to cover loss, cancellation, medical cover etc for any reason when registering for the Conference and booking travel. The Conference Committee and Conference Cecretariat do not take responsibility for any delegate failing to insure.
NAME BADGES
Your official Conference name badge must be worn at all times, as it is your entry to all sessions, the exhibition hall and social functions. Security staff will refuse entry to anyone not wearing their name badge.
PRIVACY
Information necessary for your attendance at the Conference will be gathered, stored and disseminated in accordance with the National Privacy Legislation. A participant list with name, organisation and state will be supplied to all delegates and exhibitors at the Conference. You can indicate on the registration form if you do not wish to have your details included on the participant list. Another list will be provided to major sponsors which will include your email address. Please indicate on the registration form if you do not wish your email address to be provided to sponsors at the Conference.
