We provide a full range of medicines management and optimisation services, backed by many years’ experience in commissioning and providing clinical medicines-related services across a range of NHS and social care sectors. We lead three large Area Prescribing Committees serving over 16 CCGs and surrounding trusts. Our health care systems knowledge means we can solve medicines issues between organisations and deliver at scale across ICS/STPs. Our clinical medicines services include medicines optimisation clinics, polypharmacy, de-prescribing and care homes medicines review. Transforming and improving medicines optimisation (MO) Our MO services deliver better patient outcomes, consistently high levels of customer satisfaction, and best value, evidenced by: · £9.2 million saved across five embedded MO teams · RoI of £1.65 per £1 spent on polypharmacy review for Birmingham South Central CCG · £0.2 million saved in first six months of a medicines waste reduction project for two Lancashire CCGs · reduction of 4.3% in wasted prescription items across GP practices, delivering reduced costs and GP workloads and improved patient safety. We use leading-edge MO tools to support our customers, such as our Medicines Safety Assurance Tool™, which captures safety alerts; and our unique high cost drugs Validate™ tool, which identifies inappropriate prescribing invoice requests and uses NICE guidance.

The broader capabilities of MLCSU mean we can introduce the right skills from outside our MO team to set up bespoke services designed around our customers.

Polypharmacy clinics: with 3250 patient medicines reviews; with over 80% face-to-face/domiciliary with patients

3000 interventions, of which 1,364 related to patients with high-risk medicines (>£320k cost benefit). Plus hospital admission avoidance confirmed by adapted RIO measures

Implemented self-care policy for two Lancashire CCGs

45% (£714,000 p.a.) decrease in total prescribing costs for self-care items

Identified West Cheshire CCG as an outlier in prescribed inhaled corticosteroids on RightCare data

Directed clinical pharmacists to review inhaler prescribing (£40,000 savings)

RoIs of up to 28 times; savings of £256,000 p.a. at Trafford CCG, and £612,000 p.a. at Morecambe Bay CCG, with legacy identified for scaling up & roll-out across wider GP practices

Repeat prescribing ordering support - with bespoke projects in five CCGs. In addition, now scoped at-scale prescription call centre hub solution via our CSU Referral Management Call Centre in Leyland for CCGs.

Local pilot example: Warrington CCG - delivered 12-practice pilot, 2.7% items reduction; £108k reduced prescribing

We estimate we can save each CCG at least £250k p.a, and achieve around 5% waste reduction (items).
