CHIROPRACTOR

A licensed individual concerned with the mechanical disorders of the musculoskeletal system. Treatments include manipulation (not under anesthesia), acupuncture, physical therapy, heat therapy, massage therapy, etc. Does not include stem cell treatment or pain management injections/medication.

CHIROPRACTOR CLINIC

An entity dedicated to the performance of chiropractic services (manipulation not under anesthesia, acupuncture, physical therapy, heat therapy, massage therapy, etc.). Services are rendered by licensed chiropractors, chiropractic assistants, acupuncturists and massage therapists. Does not include manipulation under anesthesia, stem cell treatments, ketamine administration or pain management injections.

DENTAL CLINIC

An entity dedicated to general dental care treatment, including annual exams and cleanings for teeth and gums, root canals, fillings, crowns, bonding, bridges, etc. Does not include oral surgery or procedures rendered under anything greater than local anesthesia, surgical TMJ treatment, bone-impacted extractions or orthodontia.

EYE CARE CLINIC

An entity dedicated to general eye care treatment, including annual exams, glaucoma and macular degeneration screenings, eyeglasses, and contact lens

prescription evaluations, etc. Services are rendered by optometry assistants, optometrists and ophthalmologists. Does not include ocular surgeries.

DIALYSIS/HEMODIALYSIS CLINIC

An entity dedicated to the performance of dialysis treatment, also known as renal replacement therapy, for patients with kidney dysfunction or failure. Services are rendered by licensed nephrology RNs or LPNs.

PHYSICAL/OCCUPATIONAL THERAPY CLINIC

An entity dedicated to the treatment of disease, deformity or injury through targeted movements and exercise, massage therapy, muscle stimulation, etc. Services are rendered by licensed physical therapists, physical therapy assistants, occupational therapists, occupational therapy assistants and massage therapists. Does not include stem cell therapy or pain management injections or entities solely dedicated to massage therapy or spa services.

PRIMARY CARE CLINIC

An entity dedicated to the first point of care, including disease and chronic illness prevention and management, annual exams and physicals, minor (nonsurgical) injuries, etc. Services are typically rendered by certified medical assistants, RNs, LPNs, LVNs, nurse practitioners, physician assistants, and general practitioners or family medicine physicians. Does not include 24-hour urgent care services, surgical treatment, inpatient facilities, free-standing emergency services or similar.

WEIGHT LOSS CLINIC

An entity dedicated to monitoring weight loss and nutrition management. Services are typically rendered by licensed RNs, LPNs, LVNs, dietitians and nutritionists. Does not include bariatric surgeries.

VETERINARIAN CLINIC

An entity dedicated to the prevention and treatment of disease, illness and injuries of domestic household animals, such as dogs, cats, birds, rabbits, small snakes and turtles. Services are typically rendered by certified medical assistants, licensed veterinarian technicians and veterinarians. Does not include the treatment or breeding of large equine, cattle, professional racing animals and exotic animals.

IMAGING SERVICE

An entity dedicated solely to the performance of scanning MRIs, CAT scans, X-rays, mammograms, etc. Services are typically rendered by licensed technicians. Does not include interpretation or diagnosis of images.

MEDICAL STUDENT (INDIVIDUAL)

An individual working toward completing their United States Medical Licensing Exam (USMLE). Coverage includes training in a clinical setting for direct patient care under the supervision of a licensed physician. Coverage not intended for currently or previously licensed physicians.

WELLNESS EXAMS/PHYSICALS

An entity dedicated to conducting general checks on vitals, BMI, family health history, discussion of current lifestyle and health choices, administration of shots and immunizations, screening for STDs and STIs, cholesterol and blood pressure checks, etc. Typically rendered by a registered dietitian or nutritionist, clinical nurse specialist, RN, nurse practitioner, physician assistant or general practitioner.

HEALTHCARE TRAINING/EDUCATION/ CERTIFICATION PROGRAMS

An entity dedicated to allied healthcare education, training and certification such as a vocational or trade school program for certified medical assistants, nursing and paramedic students, first aid/CPR training, etc. Faculty are typically licensed medical providers. Does not include four-year osteopathic or doctor of medicine programs.

OUTPATIENT COUNSELING SERVICES

Organization that offers counseling, cognitive behavioral therapy, dialectical behavioral therapy, group talk therapy, family counseling or substance abuse counseling on an outpatient basis. Services are typically rendered by licensed counselors, psychologists and social workers. Does not include any inpatient services or entities dispensing medications.
