Therapy for Parkinson’schanges West

Knoxville man’s lifeJust a few years ago, Joseph Lessard couldn’t walk. A Parkinson’s

disease diagnosis two decades earlier had restricted him to awheelchair. His New England accent had become a whisper.

Lessard had pursued physical therapy to manage hisParkinson’s, but no significant improvements were made. He wastaking medications and going to the doctor regularly when heheard about the BIG and LOUD program at the Parkwest TherapyCenter.

BIG and LOUD are offshoots of the Lee Silverman Voice Training(LSVT) technique that was established in 1987. The four-week pro-grams are meant to combat the limitations Parkinson’s causes interms of movement and speech. Four times a week, patients spendan hour exaggerating common motions to increase mobility andusing speech software to complete vocal exercises.

“By the third week, I was up and walking. It was a huge improve-ment,” Lessard said. “I started with standing up without falling,

then moved to walking and running.” “It changed his whole outlook when he saw that it was working,”

Michele Waak, Lessard’s daughter, said. “He had something to lookforward to and made him excited to go to his appointments.”

Much of the success that can come from the BIG and LOUD pro-gram stems from a commitment to doing your homework. “Youcan’t miss any practices at home,” Lessard said. “You have to do itevery day or it won’t work.”

Beyond his mobility, Lessard cites a renewed sense of balance asthe real treasure of the BIG and LOUD program. “It made me moreconfident and able to move more steadily, but I also developed alifestyle that allowed me to enjoy things again,” he said.

Lessard no longer comes to the Therapy Center for appoint-ments, but diligently does his exercises each day. He only uses awheelchair at night. “You can’t be wishy washy,” he said. “You haveto dedicate your life to it.”

Dr. Perkins indicated that over the last decade the use ofintraocular injections has become a major advance in the treat-ment of retinal disease and is a highly successful, relatively pain-less in-office procedure. Patients typically have a series of month-ly treatments. However, the exact number of treatments dependson the particular disease and the individual response to treat-ment. Since the early 2000s, the use of intraocular injections hasaccelerated, initially fueled by clinical trials in which SERA par-ticipated. Today, intraocular injections have become the first-lineof therapy for treating and stabilizing most cases of neovascularage-related macular degeneration as well as many cases of diabet-ic retinopathy and retinal vascular occlusion.

Symptoms that may indicate a retinal problem exists includefloaters, blurred vision, waviness, distortion or dark gray spots inthe central vision. Patients are usually initially evaluated by theirprimary eye-care provider and, if appropriate, referred to SERA.SERA physicians do not replace, but instead, work alongside the

patient’s primary eye care provider to deliver specialty care. If younotice any of these potentially concerning symptoms, see your pri-mary eye care provider to help determine if you might have a reti-nal problem.

SERA’s physicians have met the stringent requirements of med-ical school, internship, ophthalmology residency, vitreo-retinal fel-lowship and state licensure. vision problems in their infancy stages. Many eye experts advisethat the average person should have an eye exam every one tothree years, depending on his or her age. The AmericanOptometric Association says children should have their first eyeexam at six months of age. Subsequent exams can take place atage 3, and then when the child is beginning school. If an eye doc-tor determines a person has a particular condition that requiresmonitoring, more frequent eye exams will be advised. Childrenrequire less frequent eye exams, but as a person gets older, he orshe should schedule eye exams every year. Consult with an eyedoctor if you have questions on when to schedule an eye exami-nation or if you are experiencing vision problems.

People who sit in front of a computer for long periods of timeoften encounter a variety of uncomfortable symptoms.Headaches, neck strain, backaches and wrist pain are common,but sadly, the most prevalent symptoms of prolonged computeruse – eye strain, blurred vision and dry eyes - are often over-looked.

Computer Vision Syndrome, also referred to as Digital EyeStrain, describes a group of eye and vision-related problemsthat result from extended computer, tablet, e-reader and cellphone use. At greatest risk for developing CVS or Digital EyeStrain are those persons who spend two or more continuoushours at a computer or using a digital screen device every day.Accordingly, the level of discomfort appears to increase with theamount of digital screen use.

Many of the visual symptoms experienced by users are onlytemporary and will decline after stopping computer work or useof the digital device. However, some individuals may experiencecontinued reduced visual abilities, such as blurred distancevision, even after stopping work at a computer. If nothing is done

to address the cause of the problem, the symptoms will continueto recur and perhaps worsen with future digital screen use.

In most cases, symptoms of CVS or Digital Eye Strain occurbecause the visual demands of the task exceed the visual abili-ties of the individual to comfortably perform them.

Even if you don't need glasses for driving, reading or otherthings you do, you still may have a minor vision problem that canbe aggravated by computer use. You may need a mild prescrip-tion to wear only for use of electronic devices to reduce visionstress. A thorough eye exam every year for computer users is agood idea.

If you wear glasses for distance vision, reading, or both, theymay not give you the most efficient vision for viewing your com-puter screen, which is usually about 20 to 30 inches from youreyes. Tell your doctor at The Eye Group about your job tasks andmeasure your on-the-job seeing distances.
