This directory provides a list of Medi-Pak Advantage (HMO)’s network providers. This directory is for Benton, Carroll, Madison and Washington Counties. This directory is current as of August 2014. Some network providers may have been added or removed from our network after this directory was printed. We do not guarantee that each provider is still accepting new members. To get the most up-to-date information about Medi-Pak Advantage (HMO)’s network providers in your area, you can visit www.arkansasbluecross.com/medicare or call our Member Service Department at 1-877-349-9335, Monday – Friday, 8 a.m. to 8 p.m. TTY users should call 711.

Health Advantage is a Medicare-approved Part D sponsor and is a Medicare Advantage organization with a Medicare contract. Enrollment in Health Advantage Medi-Pak Advantage (HMO) depends on contract renewal. This information is available for free in other languages. Section 1 – IntroductionThis directory provides a list of Medi-Pak Advantage (HMO)’s network providers. To get detailed information about your health care coverage, please see your Evidence of Coverage.

You will have to choose one of our network providers who are listed in this directory to be your Primary Care Provider (PCP). The term “PCP” will be used throughout this directory. Generally, you must get your health care coverage from your PCP. A PCP is your primary resource for your medical care. Your PCP will make sure you get the care you need. Your PCP can also give you advice about staying healthy and help educate you on how to manage your chronic conditions. He/she will help diagnose your condition and recommend treatment options. He/she will help you choose a specialist, if one is needed. We consider your PCP your “medical home.”

The “network providers” listed in this directory have agreed to provide you with your health care coverage. You may go to any of our network providers listed in this directory. If you have been going to one network provider, you are not required to continue going to that same provider. In some cases, you may get covered services from non-network providers. Other physicians/providers are available in our network. Medi-Pak Advantage (HMO) does not require members or their providers to obtain a referral or authorization from our plan as a condition for covering medically necessary services that are covered by our plan. If you have any questions about whether we will pay for any medical service or care that you are considering, you have the right to ask us whether we will cover it before you get the service or care.

Providers that accept Medi-Pak Advantage (HMO)’s Terms and Conditions have agreed to bill Medi-Pak Advantage (HMO) for the services. In cases where non-contracting providers submit a bill directly to the enrollee, the enrollee should not pay the bill. You should contact Medi-Pak Advantage (HMO) and we will work to resolve the issue.

Emergency care can always be obtained in or out of the service area from the nearest available provider. You must use plan providers for urgent care. When out of the service area urgent care may be obtained from the first available provider. You must use plan providers except in emergency or urgent care situations or for out-of-area renal dialysis or other services. If you obtain routine care from out-of-network providers neither Medicare nor Medi-Pak Advantage (HMO) will be responsible for the costs.
