    objectives, dates, venue

    TheworkshopMeaningfulInvolvementtookplaceon2224November2009attheBestWesternCityHotel,Soa,Bulgaria.Itaimedtoenhancetheinvolvementof

    organisationsledbypeoplelivingwithHIV(PLHIV)andpeoplewhousedrugsin

    projectssupportedbytheGlobalFundtoghtAIDS,TuberculosisandMalaria(Global

    Fund).

    Specic objectives o the meeting were:

    toshareinformationaboutRound10ofGlobalFundgrants,aswellas

    informationaboutproceduresandissuesrelatedtothedevelopmentofgrant

    proposalsandtheinvolvementofnon-governmentalorganisations(NGOs)asPrincipalRecipientsorSub-Recipientsofthegrants;

    todiscussbestpracticesforinvolvingpeoplewhousedrugsandPLHIVinthe

    activitiesofCountryCoordinationMechanisms;

    todiscussthetechnicalsupportneedsoforganisationsledbypeoplewhouse

    drugsandPLHIV,aswellasofNGOsprovidingsupporttothem;

    toplanregionalcooperationforstrengthenedinputfromandparticipationof

    peoplewhousedrugsandPLHIVinGlobalFundprojects,andtoexpandaccess

    toup-to-date,high-qualitytechnicalsupport.Thisincludesplanningactivities

    fortheXVIIIInternationalAIDSConference(Vienna,1823July2010);andtogivecivilsocietyrepresentativesanopportunitytointeractdirectlywiththe

    participantsinthe2009GlobalFundEasternEuropeandCentralAsiaRegionalMeeting,heldinSoaon2426November2009.

    2. Participants, organizers

    and experts, and undersForty-oneparticipantsfrom17countriesand15resourcepeopleattendedthemeeting.Thirty-fourparticipantsweresupportedwithscholarships,afterbeing

    selectedfromaround60applicantswhorespondedtoanopencallaboutthe

    workshopandscholarshipsavailablethatwasdisseminatedthroughharmreduction,HIV,PLHIVandGlobalFundcontacts.Oftheparticipants,22werefromdruguserand

    PLHIVgroupsandorganisations.

    TheworkshopwasinitiatedbytheCivilSocietyActionTeam(CSAT),aglobal

    programmeforimprovingcivilsocietyparticipationintheGlobalFundsprocesses.

    ItwasorganizedbytheEurasianHarmReductionNetwork(EHRN)andAIDS

    FoundationEast-West(AFEW).Fundingforthemeetingwasgenerouslyprovidedby

    GrantManagementSolutions(GMS),aswellasthroughanon-restrictivegrantfrom

    Merck&Co.LocallogisticalpartnerwastheBulgarianFamilyPlanningAssociation.

    Topreparethecontentoftheworkshopandselectscholarshiprecipients,theOrganizingCommitteewasestablished,comprisingexpertsfromtheorganizers,

    theGlobalFund,GMSandtheEastEurope&CentralAsianUnionofPLHIV(ECUO).

    TheOrganizingCommitteedenedtheobjectives,developedtheagenda,actively

    participatedinsessionsasexpertsandpresenters,andmadetheworkshopsnal

    evaluation.

    3. Methods and materialsThemeetingintegrated15presentationswithfacilitateddiscussions,twogroup

    sessionsandquestionandanswersegments.Mostsessionshaddidacticelementstobuildacommonunderstandingonprinciples,processesandguidelinesamong

    experiencedparticipantsandpeoplethathadbeenlessinvolvedinGlobalFund

    processessofar.Theagendawasusedexibly,andsomesessionswereextendedto

    allowparticipantstoexpresstheiropinions,clarifyconceptsandaskquestions,which

    resultedinextendingsomesessionsandshorteningothers.

    TheworkshopwasconductedinEnglishandRussianwithsimultaneoustranslation

    andbilingualfacilitators;presentationsweregiveninbothlanguagessimultaneously.

    Asetofmaterialsgroupedbysessionswasprovidedtoparticipantsinelectronic

    format,alongwithhandoutsofprintedpresentations.Themeetingdiscussionswere

    recordedindetailednotes.

    4. Sessions and discussionsThemeetingssessionsstartedwithlearningabouttheGlobalFundsstructures,processesandrequirements.Theywerefollowedbydescribinganeedsassessment

    andwhatitisusedfor,andhowtosearchfortechnicalassistancetomeettheneeds

    identied.Themeetingwasconcludedwithdiscussionsaboutregionalcooperation

    anddevelopmentofthemeetingsresolution.

    4.1. Overview o the Global Fund

    TheGlobalFundwascreatedtodramaticallyincreaseresourcestoghtthreeoftheworldsmostdevastatingdiseases,andtodirectthoseresourcestoareasofgreatest

    need.Sinceitscreationin2001,ithassupported572projectsin140countrieswith

    $15.6billionoffunding.

    Itsprinciplesdenethatitnancesbutdoesnotimplementactivities;itsgrant-making

    processisquick,withanindependentreviewprocess,transparent,andaccountable.TheGlobalFundpursuesanintegratedandbalancedapproachtopreventionand

    treatment,acrosstheregionsandthreediseasesAIDS,tuberculosisandmalaria.

    Itsupportsprogrammesthatreectnationalownership,anditmakesavailableand

    leveragesadditionalnancialresources,withoutdeterminingceilingsoffunding.

    Thegrant-seekingproposalsaregathered,evaluatedandfundedthroughfundingcyclesthatarecalledrounds.Eachyearthereisatleastoneround,andbytheendof

    2009therehavebeenninerounds.Theproposalsarefundedintwophases:afterthe

    rstthreeyears(phase1)thereisareview,whichisusuallyfollowedbyafurther,two-

    yearphase2.Disbursementsoffundingaredonebasedonperformance.Ifaproject

    performsbadly,theGlobalFundcansuspendgrants.

    Sixty-onepercentoftheGlobalFundsfundshavebeenallocatedtoHIVandAIDS.Most

    ofthecostsareincurredinpurchasingmedicinesandcommodities(45%),followedby

    humanresources(23%).Importantly,theGlobalFundnancesbuildingadministrative

    capacities,recommendingthat510%ofgrantsareusedformonitoringandevaluation.

    TheGlobalFundisaninnovativepartnershipbetweenimplementinganddonor

    governments,private-sectorandprivatedonors,NGOs,andinternationalagencies.

    Therefore,itsgovernance(Board)includesallthosesectors,anditrequiresthecountries

    whichrequestfundingtoalsodevelopamultisectoraldecision-makingbodythat

    includesgovernment,theprivatesector,civilsocietyandinternationalagencies.Itis

    calledaCountryCoordinatingMechanism(CCM).

    TheGlobalFundoperatesasaSwissfoundationwithheadquartersinGeneva,whereits

    operationalbody(Secretariat)isbased.TheSecretariatdoesnothavenationaloces,andatcountrylevelitisassistedbyacontractedLocalFundAgent(LFA).Anumberof

    workshopparticipantshadexperienceofcommunicationwiththeSecretariat,mainly

    throughitscivilsocietyocersandsomealsothroughportfoliomanagers.

    4.2. The Board and its latest decisions

    DecisionsregardingtheGlobalFundsfunding,strategiesandpolicies,budgets,

    advocacyandmobilizationofresourcesaremadebyitsBoard,whichiscomposedof20delegations.

    The three most relevant delegations or the meeting participants are:

    theDelegationofCommunities(regionalrepresentatives:TerryWhite,Timur

    Abdulaev;andTerryAnderson,whowaspresentatthemeetingaspartofthedelegationtoo);

    theDelegationofDevelopingCountryNGOs(AnastasiaSolovyeva,Elena

    Grigorieva,AlbertPancic,KarloBorasandtheMacedonianrepresentative);and

    theDelegationofEasternEuropeanandCentralAsianStates(VladimirZhovtyak

    fromAll-UkrainianNetworkofPLHIVisdelegatedbytheUkrainiangovernmentandCCM).

    InvolvementintheBoardanditsDelegationsrequiresasubstantialtimecommitment,goodEnglishskills(oratranslator)andInternetaccess.Workshop

    participantsrevealedthattheyhadratherlimitedawarenessabouttheDelegations,

    theirmembershipandwork.TherelevantDelegationsneedtoimprovetheir

    communications,andtheircommunicationpoints(apersonwhoisresponsiblefor

    sharinginformationabouttheDelegation)shouldbebetterutilizedasanentrypoint

    totheDelegations.

    Aroundfourtosixmonthsofpreparationworkneedstobedonebeforeproposals

    areactuallydiscussedbytheBoard.Draftversionsofdecisionsareusuallydeveloped

    intheBoardscommittees.Alldecisionsneedsupportfromvarioussectors.For

    example,withregardstoRussiasRound3grant,dierentDelegationswithEasternEuropeanmembershipwereinvolvedandcontactedotherDelegations.Alotof

    informalnegotiatingisdonetoensuresupportfordecisions.Coordinationofand

    acrossDelegationsisnotaneasytask.

    At the Boards last meeting our important decisions were made:

    newarchitecture:onePrincipalRecipientperdiseaseatcountrylevel(so-called

    SingleStreamfunding);

    approvalofRound9proposalsforfundingof$1.9billion(butwith10%ofprojectbudgetscutforeciency);

    two-yearfundingforprevention,extendingHIVRound3grantintheRussian

    Federation;and

    Round10tobelaunchedinMay2010withdecisionsongrantsscheduledattheendof2010orearly2011.

    Theparticipantscommentedthatcuttingfundsisbadlyaectingnancingofcapacity-buildingandotherimportantaspectsofgrants,particularlyforcivilsociety,

    whichoftendoesnothavealternativefundingsources.Itisunfortunatethatsuch

    anissuereceivesratherlittleattentionfromcommunitiesandcivilsocietygroups.

    TheBoardwascongratulatedonitsdecisionregardingRussiasRound3grant,

    andsomeconcernswereexpressedabouttheinvolvementofcommunitiesintheimplementationanddecision-makingofthisextendedgrant,aswellassome

    disappointmentthatitdidnotaddresstreatmentproblemsinthecountry.

    ItisextremelyimportanttounderstandtheGlobalFundsdecisions,astheyoften

    aectgrants,theirdecision-makingandmanagementontheground.

    4.3. Proposal development: opportunities

    or civil society organisations

    Theessentialperiodwhenmostofthedecisionsaremadeisduringthedevelopmentoftheprojectproposal.Proposaldevelopmentisdoneinstages:

    1.FormationoftheCCM

    2.Initiatingproposal(deningaimofthefutureproject)

    3.Establishingaworkinggrouptodeveloptheproposal

    4.Deningobjectivesoftheproposal

    5.Collectionofdatafordevelopingtheproposal

    Therearewaysforcivilsocietytoinuencedecision-makingatallthesestages.If

    the governmentdoesnotmakeatimelydecisiontoformaCCM,anNGOconsortiumcan

    serveasCCM.GettingmorecommunitypeopleinvolveddirectlyintheCCMandworking

    groupscanhelptogetcommunityissuesintoproposalsmoreeasily.Similarly,involving

    knowledgeableexpertsandcoordinatingwithotherpartnersisessential.Theimportantphaseinproposaldevelopmentiscollectionofdata.Atthisstage,community-basedgroups

    need to have data on the needs of communities and must manage to turn those needs

    andproblemsintoobjectivesfortheproposal.Developingbudgetsisamajorweakness

    andaproblematicareaforcommunityinvolvement,butcommonlythereisapossibility

    ofobtainingexternalsupportformakingthebudgetsanditshouldbeused.Budgetingrequiresinformationonsalaryrates,costsfordeliveringservices,andassessmentofneeds

    fortechnicalassistanceandorganisationaldevelopment.Thecostsrequirejusticationin

    terms of rates and proof that the services or activities need to be funded.

    Duringtheproposaldevelopmentstage,gettinglegitimatecommunityrepresentativesintotheCCMandproposaldevelopmentCCMshouldgotogetherwithensuringtheircapacitytoactivelyparticipate.Communitygroupsneedtoknowtherequirementsand

    recommendationsoftheGlobalFundandproposaldevelopmentexpertsandneedtobe

    intouchwithvariousparties.Therepresentativesthenshouldhavewaytocommunicate

    withlocalcommunitygroupsinorder,forexample,tocollectadditionaldataandreviewthe

    proposalsobjectives,plansandbudgets.Allthisrequiresadequatetimeandcommitment

    from the community representatives.

    4.4. CCMs and opportunitiesfor community groups

    ThereareguidelinesandrecommendationsfortheCCMsthatareworthstudyingmore

    closelytounderstandhowtoimprovetheworkofcommunityrepresentativeswithand

    insideCCMs.CCMsshouldcarryoutvefunctions,includingproposaldevelopmentand

    oversightofgrantsandPrincipalRecipients.Theyarecountrypartnershipsthatshouldmeet

    thefollowingsixcriteria:

    transparentselectionprocessforCCMmembership;membershipofpersonsaectedbyHIV/AIDS,TBormalaria;

    transparentanddocumentedprocesstoinviteandreviewproposals;

    transparentanddocumentedprocessfornominatingthePrincipalRecipientand

    overseeingprogrammeimplementation;

    ensuringtheinputofabroadrangeofstakeholders;and

    havingawrittenconict-of-interestplan.

    WhileCCMsandcommunityinvolvementhavemadeamajorpositivestepforbringinggovernmentsandcommunitiestogether,thereareanumberofproblemspersistingontheground.Theoversightfunctionisoftenonlypartlyimplemented.Communitygroupsorevenbroadercivilsocietygroupsareoftenmarginalizedandmakeupasmallfractionofoppositionwhendecisionsaremade(sotheirproposalsareexcludedtransparently).

    Somegovernment-aliatedNGOsareincludedinCCMstomeettheformalcriteriaforCCMs.SomecommunitiesthatarevulnerablearenotrepresentedinCCMssuchastheRomacommunity,particularlyintheBalkancountries.EachCCMdenesforitselfwhataectedbyHIVmeans.

    AgoodwaytomanagemembershipoftheCCMistoonlyallowcommunitygroupstojointheCCMthatarealreadyorganizedinsomenetwork,thuscandemonstratewithdocumentsandothermeansthattheycancarryoutaneedsassessment,havecommunicationlinksandcanhavearepresentativewhowouldnotrepresentonlyhim-orherself,notevenonesingleorganisationbutthewholerelevantconstituency.

    Thisnetworkcouldbeformalorinformalbutwithdocumentedinterestrepresentationprocesses.Itcouldusepartnershipswithinternationalorganisationsand/orsympatheticgovernmentocialstosupportthiscommunitysrequesttojointheCCM.Anyrequestformembershipshouldfollowthisprocedure.

    TransparencythatismentionedinanumberofCCMcriteriameansthattheprocessesshouldbedocumentedanditsdecisionsshouldrepresenttheopinionofthemajority.Iftheproceduresarenotfollowed,formalcomplaintsshouldbesubmitted.First,suchinquiresandcomplaintsshouldbeaddressedatcountrylevel,and,iftheCCMisnotresponsive,thentheycouldbedirectedtotheGlobalFundsSecretariat.Allcomplaints,

    inquiresetc.shouldtakeplaceinwriting.ManagingtheseprocessesrequiresagoodunderstandingofGlobalFundproceduresandusingtools,suchastheCCMsSelf-AssessmentTool(availableinEnglishandRussianontheGlobalFundswebsite).TheGlobalFundsSecretariatcanprovidetechnicalassistancetoCCMs,butthatisdoneuponrequestfromtheCCM.Otherpartnerscanprovidesuchsupporttoo.DocumentingthataCCMisnotoperatingproperlycouldbedonethroughdocumentedinterviewswithCCMmembers,byreviewingallminutesofCCMs(iftheyarenotavailable,theCCMstransparencycriteriaarenotbeingmet),alackofaconict-of-interestpolicyornooversightcommitteewhentheCCMsconstitutionforeseesitetc.ACCMshouldalwaysknowwherethemoneyis,whethermedicinesarereachingpeopleinneed,whetherresourcesandtechnicalassistancearereachingSub-Recipients,andwhetherimplementationresultsmeettheperformanceindicators.

    AnimportantaspectofCCMfunctioningisthatcountryproposalswithCCMsthatdonotmeettheabove-mentionedrequirementsarerejected,andthisisindicatedintheTechnicalReviewPanelsrecommendationsthatareforwardedtotheCCM.ThisisthecasewiththelatestKyrgyzstanproposalforRound9.Withinthemeeting,Kyrgyz

    participantsagreedwiththeGlobalFundsSecretariattofollowuponthisinmore

    detail.

    FundingforCCMscancomefromtheGlobalFund.Itcouldbeeitheratalowerlevel

    withminimumrequirementsorwithhigherfundingwhichcouldbeallocatedonly

    withtheCCMsworkplanandsomeotheradditionalrequirements.

    Ingeneral,mostproblemswithCCMsandgettingcommunityvoicesheardareatnationallevel.BeingtheminorityinCCMs,civilsocietyorganisations,particularly

    communitygroups,arestrugglingtondsolutionsatnationallevel.Thereisa

    needforexternalsupport,sincenotallproblemscanbesolvedatnationallevel.

    Internationalpressureisrequiredonsomeissues,particularlyinsuchcountriesas

    Uzbekistanwherecommunitypeopleareputbehindthebars(asindicatedbyoneparticipant).Regionalcollaborationamongcommunitygroupsisimportant,

    especiallywhentherearenosupportersontheground.

    WhileforsometheprogressofimprovingcommunityparticipationintheGlobal

    Fundprocessesistooslow,thereareexamplesofsomemajorachievementswithPLHIVandharmreductioncommunitygroupsgettinginvolvedasimplementersandgettingmoresignicantrolesinCCMs(forexample,becomingVice-ChairofCCMs,asinUkraineandMoldova).This,however,isnottrueforcommunitiesofpeoplewhousedrugs.OnelessonfromthePLHIVmovementistobelievethatthecommunitiescanmakeachangeandtoworkhardonthat,usingexternalassistanceandpartnerships,includingbeingintouchwiththeGlobalFundsportfoliomanagers.4.5.SuccessfulproposalstotheGlobalFundandintegratingcommunityneedsThereisasetofissuesthattheTechnicalReviewPanelpointstowhenmakinggeneralobservationsonweaknessesofproposalstheyhavereceived.Thesecommentsare

    importanttoconsiderinfurtherapplicationstotheGlobalFund,alongwithstrongcommunityrecommendationsofactivitiesinthenewapplications.IntheTechnicalReviewPanelscommentsofthelastRound9review,itisnotedthatpolicyissuesarenotalwayswellexplainedandhowtheyaecttheproposalandhowtheywouldbedealtwith.Co-infectionofHIVandTBisstillnotintegratedinallsubmittedproposals.Proposedactivitiesoftenlackdetails,andbudgetingofproposalsissometimesweak.Regionalproposalsoftenlackclarityabouttheiradditionalvalueatcross-countrylevel,howtheywouldinteractandnotduplicateexistingcountryprojectsthatarealreadyfundedbytheGlobalFund.InspiteofGlobalFundpolicies,submittedproposalslackintegrationofaspectsofcommunitysystemsstrengthening,andthereisaneedformoreclarityamongCCMsandproposaldevelopers.Forthersttime,theGlobalFunddecidedtofundnationalprogrammeapplications,andtheprocessselectedanumberofapplications.Amajorweaknessnotedwasalackofcostedactionplansandshowingtherelationshipwithhealthsystemstrengthening.Anumberofdisease-specicproposalsfrommiddle-incomecountrieslackanalysisofvulnerabilitiesandwhicharethemostaectedgroups;furthermore,theyarenotfocusingonvulnerablegroupsinspiteofclearindicationsfromtheGlobalFundthatitfundsfocusedactivities

    forvulnerablegroupsinmiddle-incomecountries.TheseandothercommentsaredocumentedintheBoardsminutesandareavailableontheGlobalFundswebsite.

    Representingcommunityinterestsinacountrysproposalisnotalwayseasy.Oftenitisbecausetheneedsandactivitiescalledforarenotwellunderstoodbyothers,theyare

    notwelljustiedorgroupsdonotdemonstratethelegitimacyoftheirwork.Therefore,itisimportanttodocumentvariousdemandsofcommunities.Documentationcouldbedonethroughreferencestoexistingresearchreportsandsurveys,particularlythosepublishedinrespectablejournals.Ifsuchdatadoesnotexist,rst,thisfactshouldbestatedandtherelevantstudy/surveyshouldbeincludedinthenewproposalthatisbeingdeveloped.Inaddition,theopinionsofexperts,asmallsurveyandindirectdatacouldbeprovided.AnumberoforganisationsshouldconductvariousfocusgroupsordotheirownmonitoringandevaluationwhichshouldbeusedforjusticationwhentheircountrysproposaltotheGlobalFundisbeingprepared.

    4.6. Community mobilization

    and community systems strengthening

    Understandingcommunitysystemsstrengthening(CSS)requiresunderstandingcommunities.Acommunitycomprisespeoplewhoareconnectedtoeachotherinadistinctwayforexample,theymayliveinadistinctgeographicalareaortheyhaveexperiences,livingsituations,culture,religion,identity,valuesorchallengesincommon.OneoftheexamplesisRoma.Community-basedorganisationsarethose

    organisationsorgroupsthatarerootedinthecommunitiesthattheyaimtoserve.Civilsocietyisawidertermthatincludesorganisationsthataredistinctfromthestate,thefamilyorthecommercialmarketbuthavesomeroleinpublicpoliciesandprocesses.Thesystemsneededbyasmallgroupofneighbours,forexample,areverydierentinscaleandtypefromtheneedsofalargeNGOoranationalnetworkoforganisationscarryingoutawiderangeofactivities.Furthermore,communitysystemsintheeldsofHIV,tuberculosisandmalariamayormaynotbelinkedwithhealthdeliverysystems,dependingontheirfocusandactivities(andcommunityneeds).Somecommunitysystemsareaimedatllinggapsinpublichealthserviceprovision,butmanyhaveamuchlessdirectrelationshipwithimprovinghealthoutcomes.Thusifwespeakabout

    streetdrugusers,theirneedsaremultipleand,therefore,wouldincludesuchaspectsasaddressinghomelessness.

    socialandhealthneeds.Thisisamajoropportunityforcommunitygroupsthatcouldbeusedcreatively.Communitiescouldorganizebothaccordingtotheirvulnerabilitybutalsoasacommunityorganizedaroundoneidea,suchasreducingharmsfromdrugsorraisingorphanchildrenbygrandparents.AwarenessaboutthepossibilityofreceivingsupportforCSSislowincommunity-basedgroups.Furthermore,itis

    noteasyatcountryleveltoconvincegovernment-dominatedCCMstointegrateCSSelementsincountryproposalsortosupportasubmissionofaseparateCSSproposal.CSScouldbeintegratednotonlyincountrybutalsoinregionalproposals.Regionalproposalscouldbesubmittedeitherfromregionalcoordinationmechanisms,fulllingtheabove-mentionedcriteriawhichistechnicallyrathercomplicatedorfromaregionalorganisation,suchasECUO,maybeITPCandEHRN,dependingonmeetingeligibilitycriteria.Veryfewregionalproposalsarereceivedandsupported(seealsoabovein4.5.SuccessfulproposalstotheGlobalFundandintegratingcommunityneeds).

    4.7. Monitoring and evaluation and the role o

    communities

    Themonitoringandevaluation(M&E)ofGlobalFundgrantsiswelldened,

    withregularreportsthatthePrincipalRecipienthastosubmitonprogressof

    implementationofgrants,useoffundingetc.totheCCMandLFA.TheM&EisconductedinlinewiththeM&Eplanthathastobedevelopedatthesigningofthe

    grantagreementandreviewedbytheCCMandLFAbeforethat.Thereportisdone

    onagreedindicatorsagainstsettargets.Ifnoreportsareprovided,nofundingisprovidedtherefore,GlobalFundnancingiscalledperformance-based.The

    M&Ehastoinformgrantmanagement(conductedbySub-Recipients,Principal

    Recipients),grantoversight(byCCM),reporting(byLFA)andnationalresponseformedbyministriesofhealthandrelevantnationalprogrammes.

    Communitiescanparticipateinmonitoringnotonlythroughthissystembutalso

    bydevelopingitsownindependentmonitoring.ExamplesofthesearetheSimona+

    ProjectundertakeninRussiatomonitoractualaccessofpeopleinneedoftreatment

    andwhethertheyreceiverelevantservicesinselectedregions.InKyrgyzstan,

    threenetworksarestartingtoconductmonitoringofpurchasesofmedicinesandsuppliestointroducemoretransparencyinpricingandqualityofproductsbought.

    Communitygroupscanhelptointroduceorchangeindicatorsforexample,to

    advocateforindicatorsonqualityofservices.

    M&Eisbothvaluableandapotentiallypowerfultool.M&Eshouldbeusednotonlytomonitorgovernmentbutalsotomonitoryourownwork.M&Esystemsshouldnot

    besetupfordonorsbutshouldbeusedtoimprovework.Similarlywithtechnical

    assistance:includingtechnicalassistancevisitsshouldnotbeseenasapunishment,

    itshouldbeturnedintoanopportunitytoworkbetter.

    4.8. Afected populations needs assessment

    Theneedsofcommunitiesareoftenidentiedbyintuitionanumberofpeople

    fromcommunity-basedgroupsthinkthattheycandescribetheneedsoftheircommunitieswell.However,theparticipantsagreedthatneedsassessmentsare

    neededtodenethosedemands.Themethodsthatarealreadyusedbyparticipants

    orweresuggestedvaryconsiderably.Thesecouldbesurveys,interviews,previous

    reportsofservicesorreportsoffocusgroups,statisticaldata(forexample,on

    burdenofdiseaseorinsuranceorhomelessness)orocial(governmental)records.

    Somedatamightbebasedonevidenceandsomecouldbeanecdotal.Importantly,previousreportsofprojects,analysisofM&Esystems,evenreportsofauditors

    identifyingcertaingaps,forexample,inthecapacityofaserviceprovidercouldandneedtobeusedtodescribetheneedsofpeopleweserveororganisationswework

    for.

    Usingvarioussourcesofinformationhelpstoconrmidentiedneedsandmaketheassessmentprocessmorereliableandobjective.Thedatashouldbeconvincing,

    andyoumightwanttoaskyourselfwhetherthesedatawouldbeconvincingtoa

    donortoproviderelevantfundingtomeetthoseneeds.IntheGlobalFundandother

    proposalsitisimportanttousethebestdataavailable.Ifsomedataismissingandis

    neededtoimproveapproaches,proposedactivitiesshouldincluderelevantresearch

    orassessmenttocollectthistypeofdata.Suchresearch(andfundingforit)wouldbejustied.

    Weneedtodescribebetterhowwegathertheneedsandwithwhomweconsult,

    andwhythisgroupisrepresentative.Theneedsidentiedofpeoplewhousedrugs

    mightnotbeasmuchaboutneedlesasaboutaccesstodentalservices.Thedrug

    scenemightchange,and,therefore,theneedsofpeopleusingdrugsmightchange.Therecouldbevariousneedsofpoly-drugusers,whiletheneedsofpeoplewho

    areusingamphetaminescouldbedierentfromthoseofheroinusers;needsmight

    bedierentamongpeoplewhomweserve(whowearealreadyreachingthrough

    services)andthosewhoarenotinservices.

    4.9. Technical support and assistance

    Toreceivetechnicalsupportandassistance(TSA),weneedtohaveanunderstandingoftheproblems(andprioritizationofproblems)tobeaddressed.Problemsmightbe

    relatedtoorganisationaldevelopment(suchasgovernance,leadership,oversight),

    providerandrecipient,shouldhaveaclearagreementontheproductsoftechnical

    assistance.ThemeetingpresentedfourdierentTSAprovidersthatworkwithGlobal

    Fundactorsatnationallevel.

    GrantManagementSolutions(GMS)isfundedthroughtheUSPresidentsEmergency

    PlanforAIDSRelief(PEPFAR)andprovidesTSAfreeofcharge,followingarequest

    fromCCMsorPrincipalRecipients.Itstechnicalareascover:governance,oversight,

    CCMissues;grantsmanagement(nancialmanagement,partnercoordination,e.g.PrincipalRecipientsandSub-Recipients);procurementandsupplymanagement;

    M&EandreportingforPrincipalRecipientsandCCMs.Theyinvolveinternational

    andnationalexpertsforalimitednumberofdaysandcanassistwithdiagnosticsof

    problemsorshort-terminterventions.TheirTSAcomeswithahalf-yearmonitoring

    plan.Thesupportcouldbeprovidedinthenationallanguage.

    DeutscheGesellschaftfrTechnischeZusammenarbeit(GTZ)sBackupInitiative

    supportscapacity-buildingofcivilsocietyandgovernmentstomoreeectivelyuse

    globalnancestorespondtoAIDS,TBandmalaria.ThedierentmodesofBackup

    technicalsupportare:FastAccessModeforsmall,one-oactivities(upto20,000),

    ConsultancyModeforfundingofconsultancywork(upto40,000)andProjectMode

    forimplementationofsetsofactivities(upto200,000).ProjectModerequiresaformalrequestfromaPrincipalRecipientorCCM,whichispreferredfortheother

    modestoo.TheBackupInitiativehasforms,guidelinesandfurtherinformationin

    AFEWandInternationalPlannedParenthoodFederationEuropeanNetwork(IPPF

    EN),withsupportfromUNAIDS,isstartingtohostaTechnicalSupportFacility(TSF)

    tocoordinateandprovideTSAintheeldofHIVinEasternEuropeandCentralAsia.

    TheinitiativehasasmallamountoffundingforactualTSAbutadditionallywillassist

    inndingconsultants(primarilyregionaloneswhocandeliverTSAinRussianand

    English),andmoreinformationaboutTSAwillbeprovidedonaspecialwebportal.Thefocusisonthestrategicplanning,implementationandM&Eofeortsinsupport

    ofnationalHIV/AIDSprogrammes.Theinitiativewillberegional,withocesin

    Almaty,Bucharest,MoscowandTbilisi.FurtherdevelopmentoftheTSFwilltakeplace

    in the coming months.

    EHRNishostingtheHarmReductionKnowledgeHub,whichisprovidingservicesforafee.Itsworkcoverstraining(includingadvancedseminarswithsomescholarships

    available),trainingtoolsonharmreductionwithanewlydevelopedtoolongender-

    sensitiveharmreductionservices,directtechnicalassistance,organizingstudyvisits,

    makingneedsassessmentsetc.Ithasassistedindevelopingasimilarknowledge

    hubfocusedontheCentralAsiasub-region.Therearetwootherregionalknowledge

    hubs,oneontreatmentandcareandanotheronsurveillanceandM&E.More

    informationisavailableat:www.harm-reduction.org.

    SomeTSAisalsoneededtorequestTSA;theTSFsoces,forexample,willbeableto

    assistwithdeningtermsofreferencesforconsultants.MostTSAprovidersrequire

    legalregistration(butthatisnotthecasewithGMSorEHRN).Moreover,anumberof

    peopleareassistingeachother,andthissharingofknowledgeandcollaborationis

    an important part of the support and assistance received.

    4.10. Regional cooperation

    Meansofcommunicationatregionallevelmustbalancebetweenaccessibility,

    costandexpectedresults.Peopleoftenuseemailsandlistservsand,lessoften,

    meetings.Regionalcollaborationcouldhelponanumberofcommoninterests,

    suchaseligibilitycriteria,bettercommunityinvolvementandbetteraddressingcommunityneeds,addressingsomeGlobalFundproceduresandprocesses,possible

    regionalproposalstoaddresspolicygaps,unaddressedissuesofchildrenlivingwith

    AIDS,low-thresholdaccesstotheGlobalFundgrantsforsmallorganisations,and

    addressingtheRomapopulation.

    Atthegloballevel,thereareanumberofissuesthatrequiregoodinvolvementofregionaladvocatestoensurethatsuchinitiativesasnationalstrategyfunding,

    healthsystemsstrengthening,theInitiativeonInjectingDrugUseandmanyothers

    includevoicesfromcivilsocietyandtheregionsperspective.IfEasternEurope/

    CentralAsiaandcivilsocietydonotgetinvolvedthemselves,decisionswillbemade

    withoutthem.Themajorglobalissuesthatwillhaveanimpactoncivilsocietyare

    summarizedintheconclusionsofanAmsterdammeetingthatareavailableinEnglishandRussian(availableattheGlobalFund).

    RegionalcooperationcouldhelptoutilizeourregionalrepresentativesontheGlobal

    FundsBoardandsomemajormeetingssuchastheInternationalAIDSConference(opportunitiesforwhicharepresentedattheworkshop).Moreover,togetherwecan

    helpeachothertounderstandGlobalFundprocessesanddecisions(andstronglyadvocateformakingGlobalFunddocumentsinmoreaccessiblelanguage).Priority

    ideasandprinciplesareidentiedintheworkshopsresolution,whichwillbe

    presentedattheGlobalFundEasternEuropeanandCentralAsianRegionalMeeting.

    Theresolutionwasdevelopedbyaworkinggroupfromtheworkshopsparticipants

    andagreedattheplenary.

    ollow-up

    The major immediate results o the meeting are:

    participantswereveryactiveintheGlobalFundRegionalMeetingsdiscussions;

    participantsactivelyrespondedtoaconsultationontheGlobalFundsInitiative

    onInjectingDrugUse(atleasttworeactionsreceivedinwrittenformanda

    numberofpeopleparticipatedinasessionduringtheGlobalFundRegional

    Meeting);

    technicalsupportresourceswereidentiedandatleastoneparticipant

    followedup;

    apreliminaryagreementwassetforimprovingtheCCMinKyrgyzstan;andtworegionalnetworks,EHRNandECUO,agreedtofollowupontheresultsof

    theworkshop(usingtheresolution),withparticularfocusonworkaroundthe

    GlobalFundeligibilitycriteria.

    The organizers are suggested to ollow up

    on a number o issues within a short time period:

    andrequestinformationabouttheirwork,includinginvolvementinrelevantcommittees,possibilitiesofinvolvement,andwhatsupportisneeded(with

    6. Evaluation results,

    recommendationsand lessons learnt

    Themeetingwasasuccess,eventhoughonlysome4060%ofobjectiveswere

    achievedandtherewasamixedlevelofpreviousinvolvementwiththeGlobal

    Fundintheroom.TheOrganizingCommitteeandparticipantswereunanimousinsuggestingrepeatingsimilareventsoncommunitiesandtheGlobalFundin

    the future.

    Theagendawasambitiousandratherdense,tryingtomeettheneedsofmoreandlessexperiencedparticipants.Itwasusedexibly.However,someimportant

    issueswithamajorimpactoncivilsocietygroupswerenotcovered.More

    spaceintheagendashouldbeleftforemergingissues.Moregroupworkand,if

    needed,splittingtheparticipantsintogroupsaccordingtotheirinterestscould

    be used in the future.

    Connectingthecommunitypre-meetingwiththeGlobalFundRegional

    Meetingworked.Inthefuture,thetransitionbetweentwomeetingscouldbe

    addressedbetter,sothatparticipantsarenotlost(thusnotduplicatingmeetingdates),andhavingmoreclarityandtransparencyabouttheselectioncriteriafor

    bothmeetingsandbetterlogisticalcoordination.

    TheselectionofcountryfortheGlobalFundmeetingsandcommunitymeetings

    shouldincludeanassessmentofavailabilityofmethadoneandbuprenorphine

    toensurethatanyhealthneedsofparticipantsaremet.

    Thelogisticalsideofthemeetingwasdonewell,inspiteofaratherlatestartof

    theprocess.Themeetingarrangementswerecomfortable,whilesomedetails

    couldbeimproved,suchashavingamorespaciousroom.

    TheuseofEnglishandRussianlanguageswaswellbalanced,andshowing

    PowerPointpresentationsintwolanguagessimultaneouslywasuseful;

    experiencedintheeldshouldbeconsidered.

    Wethougharedeeplyconcernedregardingexistingbarriersfortheparticipationofpeopleliving

    withHIVandgroupsatriskfromHIVandTBaswellasnon-governmentalorganizationsinthe

    implementationofprogrammesfundedbytheGlobalFund.

    WebelievethattostrengthentheresponsestoHIVandTBinthecountriesofourregionitisnecessaryto:

    1.ReviewtheGlobalFundseconomicandepidemiologicaleligibilitycriteriain

    ordertoensureuniversalaccesstoprevention,treatment,careandsupporttovulnerableandstigmatizedpopulationsinmiddle-incomecountries.

    2.Ensureaccesstoadequatetechnicalandnancialsupportforcommunity

    systems,includinglow-thresholdaccesstotheGlobalFundsresources.Thiswillenablecommunitiestocontributemeaningfullytotheprocessesof

    developmentofnationalprogrammesandpreparationofcountryproposalstotheGlobalFund,aswellastheirimplementation,monitoringandevaluation.

    3.Ensuretheinvolvementofregionalnetworksofcommunitiesintheprovision

    oftechnicalassistancetonationalcommunitysystemsstrengthening.

    WereconrmourcommitmenttocollaboratewiththeGlobalFundanditsdonors,andwith

    governments,toachievetheMillenniumDevelopmentGoals,universalaccesstoHIVandTBprevention, treatment, care and support.

    WearecondentthatthenaldocumentoftheregionalGlobalFundmeetingwillreectthese
