MASTER SERVICE LIST FOR INTERVENORS

Use of this list: The entities on this list have provided the following information to allow parties in workers’ compensation cases to easily contact the company and its representatives regarding an intervention interest under Minn. Stat. § 176.361 in a workers’ compensation case. The left column “intervenor” lists the address to use to serve a Notice of Right to Intervene in a workers’ compensation proceeding along with the date the information was updated. The middle column lists contact information for settlement or case resolution purposes. The right column provides, where available, an alternate contact person if the primary contact person is unavailable. If a Motion to Intervene in an individual case lists a different contact person or persons, please contact the persons listed in the motion. This list is updated per the request of the entities on this list. We cannot guarantee that the information remains accurate.
