A number of parameters were withdrawn accreditation at the Labs request. The Lab requested a number of parameter additions although the Lab had not completed some supporting documentation, such as corrective actions for failed PT samples and acceptable PT sample results for all requested parameters where PT samples are available. The Lab decided to resend the addition request after they are able to submit the supporting documentation for the additions. Part of the Labs addition request included updating to EPA 608.3, EPA 624.1, and EPA 625.1, the Lab must be updated to these new methods prior to September 26, 2019. As a reminder, continued participation in the Ecology Lab Accreditation Program requires the lab to: • Submit a renewal application and fees annually. • Report significant changes in facility, personnel, analytical methods, equipment, the lab’s

quality assurance (QA) manual or QA procedures as they occur. • Participate in proficiency testing studies semi-annually, with the following exception: For

each parameter where all required PT samples were analyzed and results were satisfactory, you are required to submit only one PT result over this next year, and in subsequent years, as long as the results are satisfactory.

• Submit copies of current third-party Scopes of Accreditation when they are available. YOUR RIGHT TO APPEAL You have a right to appeal Ecology’s decision to the Pollution Control Hearing Board (PCHB) within 30 days of the date of receipt of this decision letter. The appeal process is governed by Chapter 43.21B RCW and Chapter 371-08 WAC. “Date of receipt” is defined in RCW 43.21B.001(2). To appeal you must do the following within 30 days of the date of receipt of this decision:

• File your appeal and a copy of this decision with the PCHB (see addresses below). Filing means actual receipt by the PCHB during regular business hours.

• Serve a copy of your appeal and this decision on Ecology in paper form - by mail or in person. (See addresses below.) E-mail is not accepted.

You must also comply with other applicable requirements in Chapter 43.21B RCW and Chapter 371-08 WAC.
