WHAT’S NEW FOR 20211. Mandatory Active Enrollment, October 29 - November 20, 2020 for Core Benefits (medical, dental,

vision, Health Care Flex Spending Account (HFSA), Dependent Care Flex Spending Account (DFSA), and ALL Voluntary Benefits (except for Voluntary Group Term Life Insurance). This will be an ACTIVE enroll-ment. YOU MUST WAIVE OR ENROLL IN BENEFITS FOR 2021.

� If you do not re-enroll in benefits your current coverage will end on December 31, 2020.YOU MUST COMPLETE THE ENROLLMENT PROCESS REGARDLESS IF YOU CURRENTLY HAVE COVERAGE. Current benefits will NOT automatically rollover (except Life Insurance).

� To enroll, continue, make changes, OR decline medical, dental, vision, HFSA, DFSA, ARAG Legal, or any of the Aflac policies you MUST complete the new online enrollment process. There are 2 options:

a. Speak with a Professional Benefits Counselor/Enroller by scheduling a telephonic/virtual enrollment appointment at www.myenrollmentschedule.com/lauderdale or by calling 1-866-998-2915.

b. Self-enroll or waive all core and voluntary Aflac and ARAG coverage at www.benselect.com/enroll. Please see the new online open enrollment instructions provided with the enclosed benefits rate sheet or scan the QR code:

� Enrollment MUST be completed by November 20, 2020 to have benefits for 2021 (except for Voluntary Group Term Life Insurance which automatically continues).

2. Selerix BenSelect Online Enrollment System:

� You must complete the online enrollment process (via Professional Benefits Counselor/Enroller or self-enroll) using Selerix BenSelect to have benefits for 2021.

� Selerix BenSelect provides benefit eligible employees the ability to self-enroll in ALL benefits during the annual Open Enrollment period, update Beneficiary designations, and make changes for mid-year Qualified Life Events.

� Self-enrollment at www.benselect.com/enroll simply requires your employee ID and a four digit PIN (your birth month and last two digits of your birth year). Please see the on-line open enrollment instructions provided with the enclosed benefits rate sheet.

� The enrollment system is accessible 24-hours a day and can be accessed using a computer, tablet or smart phone. 3. Beneficiary Designation Required

� All eligible employees who have a City Paid Group Term Life Insurance policy or a Voluntary Group Term Life Insurance policy will need to login to Selerix BenSelect at www.benselect.com/enroll to complete their beneficiary designation.

4. Voluntary Aflac Benefit Plans & ARAG Voluntary Legal Insurance - No Rate Changes

� All voluntary benefits will end effective December 31, 2020. You must re-enroll to continue these benefits.

� Enrollment in all voluntary benefits will be included through the new Selerix BenSelect enrollment system.

� It is recommended for employees requiring information regarding these products to speak with a virtual Professional Benefits Counselor/Enroller or visit www.cofl.fbmcbenefits.com.

� Aflac Short -Term Disability is changing from an Individual policy to a Group policy effective January 1, 2021. Employees currently enrolled in an Individual Short-Term Disability plan will receive a letter from Aflac explaining their options.

5. Biometric Screening Surcharges Waived for 2021 Plan Year

� Due to COVID-19 (Coronavirus), regretfully, onsite biometric screenings organized and held in City facilities will not be available for employees and covered spouses/domestic partners enrolled in one of the City’s Cigna Medical Plans.

� Biometric screening surcharges will be waived only for the 2021 plan year.

� Biometric screenings will remain a required activity for eligible employees who wish to participate in the City’s voluntary Wellness Incentive Program (WIP) in order to receive the $500 (taxable) incentive.

� Employees and covered spouses/domestic partners are encouraged to complete a biometric screening annually so they can be alerted of potential medical issues and to seek appropriate intervention to improve their health and well-being.

6. Cigna Medical � There will be no increase in employee medical contributions effective January 1, 2021.

� There will be an 8.1% City contribution increase for employee medical coverage effective January 1, 2021.

� All employees and dependents enrolled in Medical for 2021 will receive a NEW Medical ID card. Current Medical ID cards will no longer be valid for 2021.

� Use your NEW Medical ID card every time you fill a prescription. It has important information on it that the pharmacy needs to process your prescription(s).

� Effective January 1, 2021 Express Scripts will be Cigna’s new pharmacy platform. This upgrade will result in some pharmacy/prescription changes (i.e.: how prescriptions will be filled; updated payment information required for home delivery; etc.) If you have questions, please contact Cigna at 1-800-244-6224.

7. Cigna Dental � There will be no increase in dental premiums and no plan design

changes effective January 1, 2021.8. Cigna TheTeleDentists

� TheTeleDentists is Cigna’s telehealth dental program.

� Consult with a dentist in the convenience of your home 24/7 if you are experiencing dental pain, oral sores, lesions, swelling or infections.

� No copays and no coinsurance.

� The DPPO plans will incur an approximate charge of $49 applied towards your annual maximum.

REMINDERS FOR 2021 1. Benefits Education Workshops

� Education Workshops will be conducted October 19 - 23, 2020 via Zoom Video Conference. Vendor representatives and City staff will be available to answer your questions. This is your opportunity to learn more about 2021 benefits options. Please see the Benefits Education Workshop schedule provided in this newsletter or visit www.fortlauderdale.gov/benefits or LauderShare.

2. Dependent Eligibility Documentation � You must provide documentation (i.e., birth certificate, marriage certificate, certification of domestic partnership, etc.) of

eligibility for each new eligible dependent that you enroll by uploading the document into Selerix BenSelect. You will also be required to include Social Security numbers for each new dependent. Please be sure you have this information and documentation readily available when you enroll. The Affordable Care Act requires all employers to report Social Security numbers to the IRS for all employees/dependents offered medical insurance. If you are not able to upload the documents into Selerix BenSelect, you can submit them to Benefits, HR. Please remember to write your employee ID number on each document submitted. All documents must be received in Benefits Section, HR by November 20, 2020. New dependents will not be covered without the required eligibility documentation.

3. Domestic Partner, Child(ren) of a Domestic Partner and Adult Children (ages 26-30) are subject to Post-Tax deductions per the Internal Revenue Code (IRC)

� Premiums attributable to adult children are deducted post-tax.

� Premiums attributable to domestic partners, and the children of domestic partners, are deducted post-tax unless it is established that they are qualified tax dependents as defined by the IRC. Employees must complete the “Domestic Partner Certification of Dependent Status Affidavit” form annually to re-register a domestic partner as a qualified dependent under the Internal Revenue Code.This form is located in Selerix BenSelect and at www.fortlauderdale.gov/benefits or LauderShare.

� Employees are subject to Imputed Income for non-tax qualified dependents. 4. Tobacco Use

� Employees who use tobacco have until November 20, 2020 to complete a City authorized Tobacco Cessation Program to avoid paying a $25 post-tax per paycheck surcharge starting with their first paycheck in 2021.

� This only applies to employees who are enrolling in one of the City’s medical plans.5. Health Care and Dependent Care Flex Spending Accounts (FSA) - Benefits Outsource Inc. (BOI)

� Current FSA participants who re-enroll should continue to use their current BOI benefits debit card for the 2021 plan year until the card’s expiration date, at which time BOI will mail a replacement card.

� The maximum election for the Health Care FSA for 2021 is $2,750.

� Please refer to the 2021 Benefits Handbook at www.fortlauderdale.gov/benefits or LauderShare for details and specific information.

6. Wellness Incentive Program � You must complete the three required goals and earn at least 200 additional points through wellness related activities

every calendar year between January 1 and December 31 to receive an annual $500 (taxable) wellness incentive, payable after March 31 of the following year.

� Please refer to the 2021 Benefits Handbook at www.fortlauderdale.gov/benefits or LauderShare for details and specific information.

7. Domestic Partner and Adult Dependent Coverage Requirements

� To enroll a new Domestic Partner and/or Child(ren) of a Domestic Partner, an Domestic Partner Affidavit Form must be completed and submitted. These documents must either be uploaded into Selerix BenSelect or received by Benefits, HR by November 20, 2020. Newly enrolled dependents will not be covered without the required eligibility documentation.

� If your domestic partner is a qualified dependent under the Internal Revenue Code, then the City may provide coverage to your domestic partner without taxation. This also applies to coverage of your domestic partner’s children. The employee must complete the Domestic Partner Certification of Dependent Status Affidavit and must re-certify annually during open enrollment.

� To cover an eligible Adult Child Dependent (ages 26-30), an Adult Child Dependent Affidavit must be completed annually. 8. Cigna Telehealth Connection

� Cigna Telehealth Connection is Cigna’s telemedicine program (provided by MDLIVE) that includes live appointments with board-certified doctors and pediatricians via video or phone who are able to diagnose and prescribe, when appropriate.

DENTAL PLANSCigna DHMO and DPPO (excluding IAFF): The City offers two dental plan choices – DHMO and DPPO. Go to www.cigna.com to view a list of participating dental providers. For DHMO providers select Cigna Dental Care Access. For DPPO providers select Cigna DPPO.

Dental ID cards will be issued to newly enrolled employees and employees who change dental plans.

1. Cigna Dental Care Access DHMO: Go to LauderShare or www.fortlauderdale.gov/benefits for more information including covered services, limitations and exclusions.

Network: P410X DHMO Plan Highlights:

• In-network coverage only.

• No referral for specialty services.

• Must select a participating primary care dentist or facility. If you do not select a dentist who participates in the Cigna Dental Care Access DHMO network, Cigna will automatically assign a default dentist based on your home ZIP code. You can change your dentist by contacting Cigna directly at 1-800-244-6224.

• Covered services based on fee schedule, all other services received at a discount.

• Orthodontia coverage for children and adults.

• No claim forms to file.

2. Cigna DPPO: Go to LauderShare or www.fortlauderdale.gov/benefits for more information, including covered services, limitations and exclusions.

In-Network: Cigna DPPO Highlights: Cigna Dental PPO plan features schedules of benefits for preventive (100%), basic (100%), major (60%) and orthodontic services and are subject to exclusions and limitations.

• Extensive national network.

• Maximum annual benefit of $1,500 per person combined in or out of network.

CITY OF FORT LAUDERDALE HEALTH AND WELLNESS CENTER (OPERATED BY MARATHON HEALTH) The City Health and Wellness Center provides employees and their families (children ages 6+) who are enrolled in the City’s Medical Plans with high quality primary, preventive and acute care professional medical services for no cost (with the exception of drawn specimens sent to an external lab for testing). We are investing in your health because, over the long term, it will help mitigate costs to both you and the City. The services provided will help you understand your health and to manage it successfully.

NEW! - Virtual (video) and telephonic appointments available: The Health and Wellness Center is also offering virtual (video) and telephone visits for many of their regular services, including medical visits, comprehensive health reviews and health and nutrition coaching.

The Center follows the same rules and privacy regulations that protect your privacy at your personal physician’s office, a hospital, or other health provider. In fact, the privacy of your personal health information is protected by the Health Insurance Portability and Accountability Act (HIPAA) Privacy and Security Rules.

The Center’s staff is licensed to diagnose and treat a wide variety of common illnesses and injuries as well as prescribe various medications. Your health is the priority of the Center. Their mission is to put you in the driver’s seat of your health. The staff provides you with the personal health reports, education, and guidance to help you understand your health.

With little to no wait times, your Marathon Health center focuses on time spent with your provider developing a personalized health plan. You’ll get healthier and they will help you celebrate with programs like Healthy Like Me cheer your steps toward better health.

Summary of Services Lab Services Many lab tests can be processed at the Center, including hemoglobin A1C, lipid panel, stools for occult blood, fasting glucose, random glucose, rapid strep, urinalysis, oxygen saturation levels, influenza A and B, mononucleosis tests, pregnancy tests, and more. All tests processed at the Center are at no charge to employees, retirees, and their family members enrolled in one of the City’s medical plans. All other laboratory tests (i.e., urine culture, strep culture, complete blood count, chemistry profile, TSH) can be drawn by Marathon Health providers, but will be sent to an external laboratory for processing. The external laboratory will submit a claim to your medical plan for this service, and you may be responsible for a portion of the bill.

Prescription Medicine Provided at Time of Treatment The City’s Health and Wellness Center stocks a supply of 30 to 40 prescription medications that the medical staff may dispense as part of your medical care. A list of the current medications provided at the is available online at www.fortlauderdale.gov/benefits or www.fortlauderdale.gov/laudershare. The Health and Wellness Center is not a pharmacy. Prescriptions written by another physician cannot be filled at the Health and Wellness Center, but in many cases, and in the context of care the staff provides, you and your family may be able to get certain medications at the Center at no cost.

Wellness InitiativesThe City’s Health and Wellness Center is not just for when you are sick. The medical staff helps eligible employees, retirees, and their family members stay healthy, achieve health goals, and manage chronic conditions by working with individuals to create a personalized, step-by-step health plan. These plans empower people with chronic conditions to be more active and prevent medical conditions from becoming more serious in the future.

The City’s Health and Wellness Center or your personal physician may conduct a biometric screening and review the data on a personal and confidential basis directly with you to develop an action plan to improve your health.

Tobacco Use: This initiative only applies to employees and retirees participating in one of the City’s medical plans. Newly eligible employees who are tobacco users have 60 days from the date of their event (i.e., date of hire) to complete a City authorized Tobacco Cessation Program (if applicable) to avoid paying a $25 biweekly post-tax surcharge. The City’s authorized Tobacco Cessation Programs are:

The role of the Coordinator is to educate employees, retirees, and their covered dependents about Cigna benefits, resolve plan-related issues, and facilitate City wellness events. VOLUNTARY AFLAC BENEFIT PLANS &ARAG VOLUNTARY LEGAL INSURANCE

The City offers Voluntary Benefits that are administered by FBMC Benefits Management to all eligible active employees. These are guaranteed issue optional benefit plans that are paid 100% by employees and typically have the advantage of preferred rates, not available to individuals on their own, and the convenience of paying premiums through payroll deduction.

All Voluntary Benefits will end effective December 31, 2020. You must re-enroll to continue these benefits in 2021:

• NEW! - Group Short-Term Disability Insurance, Aflac (Post-Tax): Provides financial benefits in the event of a qualified disability.

• Group Hospital Indemnity Insurance, Aflac (Post-Tax): Provides added financial resources to help with medical costs.

• Group Accident Insurance, Aflac (Pre-Tax): Provides financial benefits for covered accidents.

• Group Critical Illness Advantage Insurance, Aflac (Pre-Tax): Provides benefits when diagnosed with a covered critical illness.
