A special thank you to the NOTABLES of Miami Jewish

Health Systems, who recently concluded their organization’s

amazing 27-year run. Their generosity allowed us to expand

our network of services geared toward caregivers and

those with memory disorders, such as dementia, cognitive

impairment and Alzheimer’s disease. Support of these

programs and services is needed now more than ever, as an

estimated 44 million people around the world are currently

struggling with these memory disorders. To learn more,

please turn to page 18.

Through the generosity of donors, we have come a long

way from our original 23-bed facility. Miami Jewish Health

Systems® is now one of the largest and most innovative

senior healthcare providers in the Southeast United States.

Strong donor commitment is not only an acknowledgement

of our proven achievements, but an investment in the

tremendous potential we have to make the world a better

place. We are sincerely honored to count you among our

family of supporters.

Thank you for all you do!

Dear Family and Friends:

Excitement ills the air here at “The Home” as we gear up to celebrate our Diamond Jubilee!

With our 75th anniversary rapidly approaching and many

exciting projects unfolding, we are pleased to announce a

new initiative: Friends of The Home. Our Friends of The Home donor-recognition society has been established to

recognize donors and express our gratitude to those who

have oだered generous support of “The Home” through their ongoing and signiicant gifts. (Please see the centerfold article

for a full description).

The Hazel and Irving Cypen Heritage Society was also

launched earlier this year, honoring those donors who have

included Miami Jewish Health Systems Foundation as a

beneiciary in their wills, or those who have made other planned gifts through qualiied giving instruments. As always, your gifts have an immense impact on our

endeavors. With the support of our benefactors this year, the

members of the Girsh Memory Enrichment Institute attended

two art-appreciation classes at the Lowe Art Museum on the

University of Miami campus, and the residents of Hazel Cypen

Tower (HCT) attended a Mozart concert at the Adrienne Arsht

Center for the Performing Arts. The Love on Wheels patient

transport program also took residents to social and

entertainment gatherings, plus music-therapy classes are

available to everyone on campus.

“Beautiful young people are an accident of nature,

beautiful old people are a work of art.”

Why We Do What We Do

Dear Miami Jewish Health Systems,

I wanted to take this opportunity to thank all of you for the wonderful care you provided my mother, Rae Levenson. Mom

passed away on December 27, 2013 at the age of 94. Her last two and a half years were spent in 4 Chernin where she got

wonderful, attentive, compassionate, professional care. My family and I were regularly kept informed of her status, and our

questions were always answered without hesitation.

Thanks to all of you—the doctors, nurses, social workers, dieticians, therapists and aides—my mother’s stay was illed with positivity and love. I am most appreciative of your services and will always remember you with great warmth.

Sincerely,

David Levenson

To Whom It May Concern:

During my husband’s stay at the rehabilitation facility, your staだ conducted themselves with the utmost professionalism and integrity, as well as respect and consideration, to me and my husband.

In addition to being extremely conscientious and exhibiting a consistently professional demeanor, they were kind-hearted,

caring indivudals who went out of their way to help anyone in need. They always had a sincere smile for everyone they met

and brightened the day of everyone they encountered.

I believe your organization is very fortunate to have a staだ so professional, and we feel blessed to have them helping us get through this diぢcult time.

Sincerely,

Lourdes and Oscar Reguera

As a long-time FOUNDER, I wish to comment upon the care that my wife Marianne received at the Toppel South rehab building.

Upon her arrival via ambulance, she was welcomed and shown to her third-loor suite by the unit concierge Alba Jaramillo. She, and the several nurses attending her, were completely dedicated to my wife’s well-being throughout her stay.

The rehab group was undoubtedly responsible for the progress that led to her recovery. Her initial injuries included ive broken ribs and a delated left lung – now she is nearly fully recovered.

All in all, her stay at Toppel was a total success.

Yours truly,

Warren Gilbert

Donor Next Door: LEAVE A LEGACY® LuncheonHonoring Hildine Potashnick

Hildine Potashnick is both a FOUNDER and Star Club

member of Miami Jewish Health Systems. In 1996,

she joined the Greater Miami Women’s Auxiliary, now

known as the Miami Jewish Health Systems Auxiliary.

Recognized for her dedication and natural leadership

abilities, Hildine was soon elected to its Board of

Directors. Eighteen years later, she continues to shine

as a beacon of light, spreading the word about Miami

Jewish Health Systems and its ability to enrich lives.

She also serves on the executive committees of the

Latin Auxiliary and the elite Gift of the Heart Society.

In 2001, the Greater Miami Women’s Auxiliary named

her one of its “Leaders of Distinction,” and in 2005, she was honored as the Latin Auxiliary’s “Woman of the

Year.” Born and college-educated in Cuba, Hildine and her husband Jerry now reside in Miami Beach. They

have six grandchildren and two great-grandchildren.

And, as the epitome of l’dor v’dor (from generation to

generation), her son Ronald Fieldstone is a FOUNDER

and member of the Miami Jewish Health Systems

Board of Directors.

On May 15th, we honored Hildine at LEAVE A LEGACY’s®

13th Donor Next Door Luncheon for her generosity of

time, talents and charitable gifts. LEAVE A LEGACY’s®

mission is to promote philanthropy, assist philanthropic

organizations, and provide a link between the critical

needs of our community and the 1,100 nonproits in South Florida. Hildine is a wonderful embodiment of

this program’s purpose.

Fourth Annual Solomon Garazi Memorial Day at the Races

The Latin Auxiliary held the Fourth Annual Solomon Garazi

Memorial Day at the Races on August 10, 2014 at the Ten

Palms restaurant at Gulfstream Park Racing & Casino.

Miami Jewish Health Systems Foundation sincerely

thanks the Garazi Family – Esther Garazi, Belina Garazi, Dr. Isaac and Anita Garazi, and Blanqui and Richard

Schoonover – for serving as the event’s chairmen.

For more than 40 years, Solomon Garazi served as a

pillar in our community - a role model who promoted

generosity, leadership and passion for the welfare of all

humanity. In gratitude for his legacy of selless service and charitable support of “The Home” and the Latin

Auxiliary, Miami Jewish Health Systems named Day at

the Races in his memory.

The funds raised from Day at the Races allows Miami

Jewish Health Systems to maintain and expand the

Latin Auxiliary’s Music Therapy Program. Because of

the Latin Auxiliary, we are fortunate to have a full time

music therapist who provides weekly, personalized

music therapy to every patient and resident at The

Home. The highlight of the day came when the bugle

sounded and the named race began. Whether your

horse was irst at the inish line or not, everyone in attendance had a winning day!

Additionally, all gifts of $500 and above will be recognized

with a new, personalized 75th-anniversary commemorative

brick, commensurate with the gift amount. (See page 20

for further details.)

the Miami Jewish Health Systems website at the following

giving levels, based on annual contribution amount(s):

Advocate - gifts from $50 to $999 Diplomat - gifts from $1,000 to $4,999 Guardian - gifts from $5,000 to $9,999 Ambassador - gifts from $10,000 to $24,999 Champion - gifts from $25,000 to $49,999

Gifts will be designated to the following initiatives

according to your desires:

Your help is needed more than ever. Alzheimer’s disease

is ultimately fatal and its cause is not yet understood.

It kills nerve cells and tissue in the brain, causing it to

shrink, aだecting a person’s ability to communicate, think, function, and eventually, even breathe. An estimated

44 million people worldwide are now struggling with

Alzheimer’s disease and other dementias; by 2030,

the number will increase to 76 million. Because of your

generosity, we can increase our research, treatment

and care of those aづicted, as well as provide education and support to their loved ones and caregivers.

The Rosomoだ Comprehensive Rehabilitation Center®

The internationally renowned Rosomoだ Comprehensive Rehabilitation Center treats patients who have complex

and chronic pain, including amputees and those with

spinal-cord injuries. We receive the toughest cases of

chronic pain and enrich lives every day. Here, it’s not

just about pain management, it’s about healing. With

more than 30,000 patients to date since 1974, and the

irst recipient of the American Pain Society Centers of Excellence in Pain Management Award, the Rosomoだ Comprehensive Rehabilitation Center can help you live

pain- and narcotic-free. Join us in making a pledge to

ease the pain.

Brucker BiofeedbackUsing computer technology and innovative behavioral

procedures, this program helps return function to

individuals with paralysis due to disease or injury in the

brain and spinal cord. It trains the brain to ind alternative connections to improve function. Patients, ive years of age and older, undergo a comprehensive evaluation and

an individualized plan is developed for such conditions

as: Bell’s palsy; brain damage; brain-stem injury; brain

tumors; cerebral palsy; encephalitis; multiple sclerosis;

spina biida; strokes; etc. Please make your donation

today so that Miami Jewish Health Systems can

help improve the outcomes of our youngest and our

most vulnerable patients.

The Memory CenterLeading the way in Alzheimer’s care and research is

our one-of-a-kind Memory Center. Marc Agronin, M.D.,

a board-certiied geriatric psychiatrist and nationally known expert in Alzheimer’s disease, leads the Memory

Center at Miami Jewish Health Systems. He and his

team specialize in the treatment of memory disorders,

such as dementia, cognitive impairment and Alzheimer’s

disease. Gifts to this important initiative continue

the funding provided by the previous donor group

known as NOTABLES.

Included in the Memory Center is the Girsh Memory

Enrichment Institute. The Girsh Memory Enrichment

Institute is a comprehensive program for individuals

with cognitive impairment and their caregivers. Each

program lasts 12 weeks and classes are open to 10

to 15 individuals per session. It is designed to help

delay the progression of memory disorders and provide

strategies for memory improvement.

Cognitive impairment, dementias and Alzheimer’s

disease will ultimately impact all of us. Please help

us ind a cure!

Love on Wheels

Love on Wheels is a vital, on-campus transportation

program that turns ordinary days into extraordinary days,

enriching the lives of “The Home’s” elderly residents and allowing them to participate in a full roster of events

and activities throughout our 20+ acre campus. For that

purpose, the Miami Jewish Health Systems Auxiliary

helps underwrite the salaries of part-time wheelchair

transporters, whose sole responsibility is to move

residents to and from various venues on our grounds.

With your donation, we can continue to help people

move forward.

As Miami Jewish Health Systems enters its 75th year, we are more determined than ever to deliver person-centered care – an approach to care that respects and values the uniqueness of the individual.

Your generosity enables us to ind innovative solutions that help people age safely and graciously, ease pain and discomfort, and enjoy more enriching lives.

We are pleased to invite you to Friends of The Home.

Join us in making a positive impact in the lives of those we serve.

Music Therapy ProgramThe Music Therapy Program provides individual

and group music-therapy services to “The Home’s” residents, tenants and daycare participants. The mere

opening lines of a song can trigger countless memories

from decades ago or prompt the recollection of

priceless events in one’s lifetime, all of which may be

locked away and never released if not for the incredible

power that music has to recover what was once lost.

The generosity of the Latin Auxiliary allows Miami

Jewish Health Systems to maintain and expand the

Music Therapy Program. Each gift, regardless of size,

adds to the crescendo and harmony of our beloved

Music Therapy Program.

Gift of the Heart®

The Gift of the Heart Society is an exceptional group

of people who, by their participation, provides support

for the premier programs and services oだered by Miami Jewish Health Systems. By proudly wearing

a Gift of the Heart pin, you become the Heart of The

Home. In addition to the current selection of diamonds,

and in celebration of our 75th Anniversary Diamond

Jubilee, we have added the option of completing the

pin’s “M” with precious stones of colors corresponding to our Miami Jewish Health Systems logo. Gift amounts

will vary depending on the price of the selected stones

(i.e. rubies, emeralds and sapphires). The price for the

pin remains $600 and $500 for each diamond. Funds

raised from this initiative are unrestricted. A Gift of the

Heart can be described as an outward expression

of caring that comes from within. It is, above all,

a relection of a generous spirit and the perfect reason to pay tribute to your loved one.

Help Us Pave the Way to Success, One Brick at a Time!Miami Jewish Health Systems Foundation invites you

to add your own personal touch to “The Home.” By purchasing a paver, you’ll have a meaningful reminder of

your aだection for “The Home”; honor a family member or inluential person in your life, or celebrate a loved one’s birthday, holiday or milestone.

There are two sizes of customizable stone pavers

available. A $500 gift enables you to write three (3) lines of

text on a 4x8-inch paver. There are also 8x8-inch pavers

that hold six (6) lines of text, available for a $1,000 gift.

Your donation will be designated to the initiative of your

choice, or the area with the greatest need.

We hope you will be a part of this wonderful opportunity

to leave your legacy with “The Home,” brick by brick, to share with generations to come. Miami Jewish Health

Systems Foundation reserves the right to approve all

text prior to inscription.

Unrestricted GiftsUnrestricted gifts are used to help areas of “The Home” where the need or the opportunity is the greatest. Your

gift in any amount may be allocated by leadership

to an area with the greatest need. These will include

donations for memorial and honorarium recognition

(unless otherwise designated), as well as Gift of the

Heart contributions.

Hazel and Irving Cypen’s commitment to Miami Jewish Health Systems, formerly known as the

Miami Jewish Home and Hospital for the Aged at Douglas Gardens, made it what it is today, a

“home for beginning again.” Judge Irving Cypen* and his “beautiful bride,” as he fondly referred to Hazel, were known as the “President” and “First Lady” of Douglas Gardens.

Miami Jewish Health Systems is honored to name its planned giving society the Hazel and Irving

Cypen Heritage Society, commemorating the Cypens’ philanthropic achievements at “The Home”.

The Hazel and Irving Cypen Heritage Society is a recognition society for our planned giving donors.

Making a planned gift to “The Home” gives you the opportunity to combine your philanthropic philosophy with tax-planning strategies that will keep your legacy alive long after you are gone.

A planned gift to Miami Jewish Health Systems Foundation is an investment in the future—

in clinical research, treatment and education toward Alzheimer’s disease and other memory-

related illnesses, in the expansion of facilities, and in securing the latest healthcare technologies

to meet the needs of a rapidly growing mature generation.
