It is with great pleasure that we welcome you to our 2nd European Symposium

and the 12º Simposio Ibérico.

Throughout this past year, BIOMET 3i has celebrated its 25th Anniversary of supporting

clinicians’ needs in restoring patients’ smiles. During these past years, implant dentistry

has evolved to become a mature discipline, giving us great confidence in the future that

lies ahead. All the key learning and research of the past years have been applied in the

development of the new 3i T3™ Implant which we will launch at the Symposium. The 3i T3

Implant is a contemporary hybrid implant with a new multi-surface topography designed

to deliver aesthetic results through tissue preservation. Over the past months some of the

most recognized clinicians have been using this implant as part of the preparation for

launch and will share some of their experiences with you during the two day event.

Since its foundation in 2001 as the Iberic Symposium, this meeting has been recognized

as an excellent continuing education opportunity and now with worldwide attendance

from over 23 countries, has evolved to be the host of the European Symposium. With this

historic review, I invite you all to participate in this Symposium which promises to be an

exceptional event.

We look forward to greeting each one of you in Madrid.

3 Dear Colleagues,

Implant dentistry is changing rapidly. Today, both clinicians and patients want to achieve

optimal aesthetic restorations as quickly as possible. We have learned during the past

decades that sustaining aesthetic results over time requires both knowledge and

commitment. Bone and soft tissue around implant(s) must be managed expertly at the

time of surgery, during the restorative phases, and thereafter during maintenance. This

must be accomplished against the backdrop of patients’ natural and growing desire to

avoid protracted and time-consuming treatment protocols.

This 2nd European Symposium provides an extraordinary opportunity to focus on these and

other important topics. Some of the most innovative and experienced implant practitioners

in the world are gathered to discuss recent research findings and the treatment strategies

developing from them, including the use of the new 3i T3™ Implant, a contemporary

hybrid implant designed to help clinicians deliver optimal, predictable aesthetic results

through tissue preservation.

In the following pages, the breadth of the program as well as the world-class caliber of the

speakers is evident. We are pleased you have joined us here in Madrid to listen to what

Since placing their first dental implant in 1986, the presenters have incorporated and developed many different techniques, materials, and products. This presentation will discuss the evolution that has occurred, in pursuit of not only aesthetic and functional success but also simplicity and long-term treatment results.

11.50–12.20 Selection Of Augmentation Materials Does Matter! From Guidelines To Decision Making Dr. Nicola De Angelis

After a short introduction to the types of bone defects and their prognoses, this presentation will focus on surgical treatment. All techniques that enable an increase in new bone and/or soft tissue should be considered bone augmentation techniques. Strict parameters for surgical preparation are essential. Particular attention will be given to minimally invasive surgical approaches and appropriate biomaterials. Long-term clinical and histological results for various bone substitutes and collagen membranes will be reviewed.

12.20–12.50 Crestal Bone Remodeling And Its Clinical Significance - Dr. Stefan Fickl

Postrestorative reductions in crestal bone height have been acknowledged to be a consequence of implant therapy involving two-piece implants. Many different reasons, including mechanical and biological factors, have been postulated to be the causative factors for this clinical phenomenon. Platform switching seems to have the potential to limit crestal bone loss around dental implants, and other factors influence the stability of tissues in this area. The goal of this presentation is to demonstrate factors involved in crestal bone remodeling, strategies to avoid it, and its clinical significance.

Digital dentistry is allowing dental professionals to optimize their daily practices by standardizing the control of quality and workflow. Dental treatment is now being delivered in a manner that only a few years ago may have been seen as science fiction. Implant surfaces may vary in roughness at the micrometer level. Animal research has verified that so-called moderately rough surfaces (characterized by Sa levels of between 1 and 2 micrometers) show the strongest bone responses. Clinical data support the use of moderately rough surfaces rather than smoother or rougher ones. (An exception is implants placed between the mental foraminae in the mandible; there good clinical results have been reported with most types of surfaces). Surfaces may further vary with respect to nanometer-scale roughness, chemistry, and physics, but evidence in this area is limited to animal research. This presentation will explore what is known about the ideal parameters for implant surfaces.

09.10–09.40 The Clinical Effects Of Implant Surface Topography Prof. Dr. Markus Huerzeler

Implant surface topography has been evolving over the last 25 years. Subtractive rather than additive techniques have shown very positive results since the introduction of modern microtopographic surfaces. Recently these surfaces have been refined, further increasing the clinical benefits. This presentation will discuss the clinical aspects and ideal biologic range of implant surface topography, particularly as it relates to compromised bone situations.

09.40–10.10 Immediate Implants: Considerations For Achieving Optimal Peri-Implant Architecture - Dr. Bruno Negri

Immediate placement of implants into extraction sites is a proven technique. However, modifications in the surrounding bone following extraction and implant osseointegration can make it challenging to obtain predictable aesthetic results. This presentation will discuss methods for achieving and maintaining a natural tooth-like structure after immediate implant placement. Selection of biomaterials that will assist in attaining aesthetic results will also be covered.

10.10–10.40 Accelerated Treatment Concepts With Implants In The Aesthetic Zone. When And How? - Dr. Stavros Pelekanos

Clinicians who create implant-supported restorations have found it challenging to choose the appropriate methodology, abutment design, and restoration type necessary for achieving optimal aesthetics and avoiding complications. Moreover, the choice of materials that will come in contact with the soft tissue (acrylic, base alloy, gold, titanium, zirconia) further complicates decision-making. This presentation will focus on implant placement (immediately and delayed) in aesthetically demanding situations, immediate loading procedures, and abutment selection (customized vs. prefabricated abutments, screw- vs. cement-retained), with an emphasis on biological responses to the various material choices. New approaches to immediate abutment placement will also be discussed.

17.10–17.40 Global Aesthetic Therapy Using Wide-Diameter Implants - Dr. Frédéric Chiche

Achieving a predictable aesthetic result in the maxillary anterior area can be helped by using wide-diameter implants in combination with platform switching to preserve bone. The use of wider implants helps prevent ridge collapse during post-extraction remodeling but must be combined with platform switching to preserve the crestal support. This presentation will demonstrate the aesthetic results that can predictably be achieved with this combination of unique components. Surgical and prosthetic procedures will be discussed.

Interest in intraoral scanning techniques has grown during the last years. Are we ready to adopt this new digital impression technique or should we wait? This presentation will review different intraoral scanning systems and describe the workflows involved in using them to capture digital impressions of BellaTek Encode Healing Abutments.

The integrity of the dental-implant-abutment junction (IAJ) has clinical relevance due to the potential detriments associated with an inferior seal. To build upon the research in this area, a novel, dynamic fluid leakage testing protocol was established and executed. This presentation will highlight the results of the study including data on BIOMET 3i, Straumann, Dentsply (Astra Tech), and Thommen implant connections.

This presentation will discuss strategies for establishing and sustaining aesthetics of implant-supported restorations throughout the course of years and even decades. Many factors contribute to the achievement of aesthetic restorations with immediately placed implants and sustaining those results over time. This presentation will review guidelines for immediate implant placement in the aesthetic zone, including patient selection, treatment staging, provisional restoration, soft-and hard-tissue preservation, and other considerations for achieving successful outcomes.

14.35–15.00 Does Size Matter? Sinuslift Or Short Wide Body Implants? Prof. Hugo De Bruyn, Moderator

Today dental implants have good predictability in a large range of treatment applications, thanks to improvements in implant surface and body design, along with a better understanding of soft- and hard-tissue biology. From a conservative delayed-loading protocol requiring a lengthy treatment time, implant treatment has evolved towards immediate loading, with benefits for both patients and clinicians. However, treatment of the maxilla and especially the posterior area has always been associated with higher implant-failure rates predominantly because of the bone morphology and quality, as well as the anatomical limitations in that region. To overcome these drawbacks, bone-grafting procedures and sinus lifting have been advocated. The overall clinical outcome of maxillary implants will be discussed in this presentation, along with the scientific evidence regarding short and wide implants as an alternative to sinus lifting or bone grafting.

15.00–15.30 Expanded Indications Of Intraoral Scanning. A Sustained 25 Year Success History Dr. Andreas Bindl

Since its introduction, more than 25 years ago, the intraoral scanning technology for chair side fabrication of all-ceramic restorations has enjoyed successful expansion in the Dental offices across the globe. Development has been focused on inlay, partial crown and crown restorations in a single appointment, mainly through chair side milling units. However, clinicians focused on dental implant treatments overlooked this technology which was not providing any implant prosthetic solution. During recent years, indications were expanded to the initial crown and bridge offering, including restorations such as implant abutments. In this context, healing abutment impressions through intraoral scanning has integrated the best of both worlds, helping clinicians to avoid the soft and hard tissue recession associated with components swapping inherent to restoring an implant case, with the ease of use and patient friendliness of intraoral optical impressions.

15.30–16.00 Immediate Implants, Myth Or Reality - Dr. Alfredo Machín

The advantages of placing implants immediately after tooth extraction include avoidance of overheating caused by cortical bone drilling and greater preservation of the keratinized tissue. Reduced bone resorption is another potential benefit that has been widely discussed. On the other hand, systematic practice of this technique precludes the possibility of posterior bone augmentation surgeries or soft-tissue management. If restorations can be simultaneously placed along with implants placed immediately post-extraction, the benefits to patients can be substantial, and better treatment plan acceptance should result. This presentation will review many of the aspects involved in immediate implant placement.

16.00–16.30 Predictable Protocols For Optimal Anterior Implant Aesthetics - Dr. Ion Zabalegui, Dr. Eva Berroeta

Anterior implant placement has always presented a challenge, due to the aesthetic requirements, difficult osseous anatomy, and hard- and soft-tissue changes occurring during healing. Since the labial bone plate resorbs in every direction after tooth extraction, even when an implant is placed immediately, patients may end up with compromised aesthetics. This presentation will describe a protocol that has been adapted for different clinical situations in order to achieve predictable functional and aesthetic results after anterior maxillary tooth extraction. The technique involves placement of an implant immediately after tooth extraction in the best three-dimensional position relative to the definitive restoration, as well as reconstructing the alveolus by means of a flapless modified socket seal procedure. Results from up to 10 years of follow-up will be presented.

12.40–13.10 Immediate Implantation And Immediate Restoration For Better Aesthetic Results Prof. Dr. Dr. Robert Haas

Patient acceptance of implant treatment is never higher than it is immediately after tooth loss. Standardized implant-placement protocols and current easy-to-use technology allow for even patients who are edentulous in one or both arches to be restored within three hours. Single-tooth cases can be treated even more quickly. This presentation will focus on treatment modalities that facilitate immediate implant placement and immediate function. Results that document the outcome of implants placed in this manner, as well as patient reactions to such treatment also will be presented. Long-term aesthetic outcomes will receive particular attention.

14.25–14.50 Mastering Aesthetics In Post-Extraction Site - Dr. Ronnie Goené, Moderator

Being able to place implants immediately in fresh extraction sites and restore them immediately can offer many benefits, as patients demand implant solutions that can be delivered as quickly and non-invasively as possible. Such restorations also have high survival rates. However, questions have remained about the aesthetics achievable for such implants. A protocol will be presented for achieving a high level of aesthetic success for implants placed in fresh anterior maxillary extraction sockets and immediately.

14.50–15.20 Implant Positioning When Replacing The Four Maxillary Incisors: A Platform-Switched Treatment Option - Dr. Víctor Méndez

Patients do not view dental implants as an object of desire; rather, they seek a way to replace teeth that will be as cost-effective and minimally traumatic as possible. Nowadays, anterior fixed partial dentures can provide an aesthetic result that is difficult to distinguish from the natural dentition. Consequently, any implant-supported prosthesis will be compared to the aesthetic and functional standards set by conventional tooth-supported restorations. The restoration of the four maxillary incisors by means of an implant-supported prosthesis is one of the most challenging situations in implant dentistry. The questions of how many implants should be placed and where they should be positioned are especially important for achieving a superior end result. In this presentation, a protocol will be presented for placing two platform-switched implants in the central incisor positions as a means of achieving the correct biomechanical behavior of the prosthesis, along with the best possible aesthetic results.

15.20–15.50 A Digital Approach To Minimally Invasive Surgery That Maximizes Treatment Outcomes Dr. Tommie Van de Velde

This presentation will describe the indications and limitations related to digital dentistry in the context of implant guided surgery and restoration. The rationale for the use of guided surgery in clinical practice will be described, and results of studies scrutinizing the clinical aspects of guided surgery will be related to existing treatment protocols. To illustrate the technical and biological benefits of digital dentistry, cases also will be presented.

Restoration of implants in the aesthetic zone remains one of the greatest challenges in implant dentistry. With the aid of appropriate bone and soft-tissue engineering, clinicians can create restorations that blend in perfectly and emerge from the peri-implant sulcus in harmony with the neighboring teeth. But quite often bone loss can occur over the short and long term, with consequent unfavorable soft-tissue changes. This presentation will focus on the capacity of implant-abutment connection designs to prevent unfavorable bone remodeling around the implant collar.

Dental patients today often are seeking treatment for a wide range of aesthetic and functional issues. Successful oral rehabilitation of such patients thus must integrate various dental specialties; dentists must be able to work as part of a multidisciplinary team. This presentation will review the requirements necessary for the delivery of optimal treatment.

Predictable clinical outcomes of immediately placed and provisionalized implants in the aesthetic zone depend upon multiple factors. These include: atraumatic tooth extraction, position and stability of the implant, and proper maintenance of the hard and soft tissues. Furthermore, attention to the crestal bone anatomy and tissue biotype is required for obtaining optimal aesthetics. This presentation will review guidelines for obtaining sustainable aesthetics with this treatment modality.

12.10–12.40 New Techniques And Technologies In Guided Surgery - Dr. Jacobo Somoza

In the past few years, the introduction of CAD/CAM technologies dramatically changed dentistry, including the areas of implant treatment planning and guided implant-placement surgery. This presentation will review several new designs for surgical stents, immediate loading protocols, and radiological diagnostic appliances. The “Virtual Key” system for calibrated guided surgery, comprised of both techniques and appliances, will receive special attention. Virtual Key appliances include a unique radiographic appliance that is automatically recognized by the planning software. Using it, all pre-surgical diagnostic information such as the prosthetic plan, wax try-in, photographic mapping onto stone models, and diverse STL files are automatically aligned with the case.

16.30–17.00 Preserving Anterior Maxillary Labial Bone: The Foundation For Aesthetic Success Dr. Yasukazu Miyamoto

The long-term stability of the peri-implant tissues is critical to achieving lasting aesthetic results. Often tissue stability in the aesthetic zone is related to the stability of the labial bone. Predictable results using platform switching to preserve critical labial bone will be demonstrated with cone-beam computed tomographic (CBCT) scans and long-term post-operative assessment.

17.00–17.30 Overall Success Rates Using Short Implants In Atrophic Posterior Mandibular Sites Dr. Pietro Felice

Implant therapy reliably yields good long-term outcomes. However, severe bone resorption in the partially edentuous posterior mandible may hamper implant placement. Various bone-regeneration techniques have been devised in recent years to allow for prosthetic rehabilitation using the longest possible implants. However, evidence has been increasing that these procedures entail significant post-operative morbidity. Current clinical research is thus focusing on surgically less invasive solutions using short implants. Recent literature reports have documented short implant success rates similar to those of long implants. This presentation will review the results of three clinical trails designed to compare the efficacy of short implants versus long implants inserted in partially edentulous patients with posterior atrophic mandibles following interpositional bone augmentation.

This presentation will focus on different methods for hard- and soft-tissue augmentation to achieve long-lasting functional and aesthetic treatment results. The use of tenting screws, absorbable thermoplastic polylactic acid membranes, and classic augmentation techniques will be discussed.

18.00–18.30 Peri-implantitis: From Diagnosis To Treatment - Dr. Helena Francisco

Peri-implantitis is characterized by an inflammatory process in the mucosa surrounding a functioning implant, with attendant loss of the supporting bone. The subgingival biofilm that forms on implant surfaces does not differ from that on tooth surfaces. But despite the close similarities between tissue reactions around teeth and implants, the lesions of peri-implantitis and periodontitis may differ both in extent and rate of progression. This presentation will review the etiology, pathogenesis, and diagnosis of peri-implantitis, along with the relevant literature regarding the role of current implant surfaces in spreading inflammation. Clinical implications and therapy will also be discussed.

A BRIEF guIdE FoR EXPloRIng MAdRId

Madrid is one of Europe’s most cosmopolitan and multiethnic cities.

It is a business and government centre, housing the Ministries, the Central Government, the Spanish Parliament and is also the place of residence of the Spanish monarchy.

With a population of 3 million and another million of commuters, Madrid is also a major centre for cultural and artistic activities.

Its wide variety of monuments, the diversity and character of its historic districts and its Museums put it on a par with any other major European capital.

The climate of Madrid is cold and dry in winter. The average temperature in January is 6ºC but the skies are often cloudless and blue, with a welcoming warmth from the sun at midday.

Gastronomy in Madrid is culture, leisure and business all in one. The range is enormous, from traditional home cooking to highly innovative, ground-breaking cuisines.

The best way to get around Madrid by public transport is the Metro. With its 12 lines, it can take you quickly and conveniently to any part of the city.

Madrid, a multifaceted city:

• The “old” Madrid of the Habsburgs is to be found in the districts that make up the city centre; narrow streets lined with shops selling traditional products that open on to the Plaza Mayor and from there to the Puerta del Sol, “the city’s forum” and kilometre 0 of the Spanish road network.

• The Madrid of “Las Letras”, a maze of streets that lead to the Plaza de Santa Ana, where bars, restaurants, theatres and nightspots coexist harmoniously.

• The Madrid of “the Austrians” takes us to the city and court of Carlos V, with the impressive Palacio Real and the Plaza de Oriente and the Opera building.

• The Madrid of “tapas” was built by the Habsburgs. Originally a working-class district, it hosts a multitude of bars and taverns offering ‘tapas’ that will delight even the most demanding gourmets. A popular open-air market (El Rastro) is held here every Sunday.

• The “picturesque” Madrid; the Lavapiés district is a warren of narrow streets overlooked by balconies filled with geraniums. A large number of art galleries have their premises in this district.

• The Madrid for “smart shopping” is without doubt the Salamanca district, built by Queen Isabel II in the second half of the 19th century.

• The “financial” Madrid, with the Plaza de Colón as its hub. This is where the Ministries, Banks and other government buildings are located.

• The “traditional and avant-garde” Madrid is without doubt the district of Chueca, next to the historic centre. Lining its narrow streets, you will find traditional family-owned shops alongside fashion boutiques. The architectural heritage of this quarter has been well-preserved.

• The old “residential” Madrid is centred around Rubén Dario, a district preferred by the aristocracy in the second half of the 19th century. Today, it is where you will find Embassies, offices and luxury apartments.

• The “entertainment” Madrid is the Gran Vía, lined with cinemas and theatres from Plaza de España to Callao.

Climate:Madrid is 600 m above sea level. Winters are cold and dry, with temperatures often dropping below 0ºC.

Public transport: The Metro, with 12 lines, is the quickest and cheapest (€1.50) way to travel around the city.
