    Madison Avenue Elementary School is a two (2) story structure totaling 35,154 s.f. The original facility of 12,554 s.f. was constructed in 1932. A small classroom addition and aseparate kitchen addition were consecutively constructed in 1946 and 1953. The additions totaling 6,036 s.f. were minor in scale and for the convenience of this report areidentified as one addition. A classroom wing of 11,774 s.f. was constructed in 1963. The media center addition totaling 4,790 s.f. was constructed in 1994. The facility contains o(1) gymnasium of 1,485 s.f. which also serves as student dining, one (1) kitchen of 661 s.f. and one (1) media center of 1,260 s.f. Madison Elementary currently provides space fgrades K-5. The complex provides partial accessibility but does not fully conform to the provisions of the Americans with Disabilities Act Guidelines. The second floor of the schofacility is not accessible. Typical classrooms throughout the facility are undersized and do not conform to the current standards established by the State of Ohio. The school facilis located on a 11.6 acre site in a residential area. School buses load/unload along the main entry drive which shares access with vehicular traffic. The existing structure isconstructed with load bearing masonry walls with supported deck and slab on grade construction. The structure is outfitted with a motion sensor security system and a fi re alarmsystem with manual pull stations. The complex does not contain a suppression system. A portion of the 1993 addition is equipped with an air conditioning system.

    Facility Assessment

    A. Heating System

    Description: The existing system for the original building, the 1946 addition and the 1963 addition is a gas fired low pressure steam boiler system. The boilerinstallations were updated in 1998. The existing system for the media center addition is a hot water boiler system which is original to the date ofconstruction. A limited portion of the 1994 addition is equipped with air conditioning. Typical classrooms throughout the complex are equippedwith unit ventilators.

    Rating: 3 Needs Replacement

    Recommendations: Provide for complete replacement of the entire heating, ventilation, and cooling system throughout the entire complex due to condition, use of

    dated equipment, and lack of proper ventilation and air conditioning. Convert to fully ducted system to conform to OSDM standards and currentventilation requirements.
