Lift Station Rehabilitation General Construction Services Pre-Bid Meeting MINUTES January 03, 2019 2:00 pm

1. Introductions:

2. Mandatory Meeting: Sign In: Sign-In Sheet will be record of attendance.

3. Intent: Inform bidders about project. Point out items of interest and ensure bidders understand what the project

entails. Provide an opportunity for each bidder to visit each site. Nothing said in this meeting changes the Bidding Documents. Bid what you see.

4. Scope: A general services contract for the rehabilitation of operating wastewater lift stations. Work to be performedincludes but is not limited to demolition and removal of existing piping, fittings, valves and structures andinstallation of new piping, fittings, valves, structures, resetting existing pumps, rehabilitation of interior walls,relocation and installation of electrical equipment, general site work and all associated labor, materials, equipment,permits and maintenance of traffic to result in a complete and functional installation as shown on the constructionplans and specifications.

5. Bid Date/Time: January 15, 2019 at 2:00 p.m. local time in the Governing Board Room, 2500 Jupiter Park Drive,Jupiter, FL 33458

6. Engineer’s Estimate: $250,000 annually for a 5 2 year period and optional 3 annual renewals.

7. Contract Time Initial Contract Time 24 Months with option of three 12-month renewals Each individual lift station rehabilitation: 60 Days from receipt of purchase order.

8. Renewal Option with Escalator: The contract allows for a price increase at each optional renewal. Increase is basedon US Department of Labor, Bureau of Labor Statistics Consumer Price Index (CPI-U All Items Not SeasonallyAdjusted.

9. Special Conditions, Paragraph 11. Insurance Required: Note coverages and additional insured requirements.Insurance requirements apply to Contractor and all Subcontractors.

10. General Requirements: 1.1.7 Regulatory Permits and Permit Conditions. District will reimburse application andpermit fees. Contractor is responsible to obtain permits.

11. General Requirements. 1.1.15 Working Hours: 7 a.m. to 4 p.m. excluding weekends and District observed holidays.

12. General Requirements. 1.1.35 Shutdowns and Bypass: Read carefully, contractor is responsible to reimburse theDistrict for costs associated with extended shutdowns or bypasses. Times shown are maximum allowable, shorterduration shutdowns and/or bypasses are included in other specifications.

13. Traffic Regulation: MOT plans and all materials and labor for implementation contractors responsibility.

14. Erosion and Sediment Control: Site erosion control applicable to all project sites.

15. Dewatering: Contractor is responsible permitting.

16. Trench Safety Act: Contractor is required for compliance with Chapter 553 of the Florida Administrative Code.

17. Other Standards and Specifications: Applicable standards and specifications are included by reference.

18. Testing – The District will provide testing services for density and concrete.

19. Owner Furnished Equipment: Various materials included in the work will be Owner Furnished Materials. Read the measurement and payment section for each pay item for specifics.

20. Measurement and Payment, 8.1.6 Quantities: Unit Price Bid Sheet Quantities are estimates based on the initial 2 year contract period. Quantities may vary significantly and District may increase/decrease quantities without invalidating the unit price.

21. Questionnaire – Required for all Bidders. Must be complete. Qualification requirements included in questionnaire.

25. Schedule Draft construction schedule is due prior to issuance of an individual PO. Contractor shall prepare and

submit for review. Schedule with District to implement station bypasses a minimum of 14 days in advance. Contractor shall anticipate up to 48 hours after bypass is initiated before release of the lift station for

demolition. Only one station at a time may be removed from service (i.e. bypassing of multiple stations simultaneously

will not be allowed.) Questions issued during the bid and at the prebid meeting.

1. There are 10 mobilizations shown. If there are more than 10 lift station rehabilitations will this quantity be increased? Yes.

2. There are items in the measurement and payment section that are not included in the Bid Response. Correct, this addendum includes corrections to the Bid Response and the Measurement and Payment Section.

3. Is bonding required on this project. No, bid, construction and maintenance bonds are not required. 4. Please provide additional information on what is the electrical scope of work or if it is to be provided by the

owner. The bid form there is 2 panel racks and 2 panel installations. Is this a completely new panel rack with all new meter can, disconnect, terminal J Box, etc. as shown on Sheet SD-4? These are two different pay items on the Bid Response Form and Measurement and Payment Section. The descriptions in the Measurement and Payment Section have been revised. Please see Addendum 1.

5. Please provide additional information on what is the valve vault scope of work. The bid form there is 5 valve vaults. What is the sizes along with there depth? What is the aluminum hatch dimensions for each vault? Are the hatches lockable or require fall protection features? See revised Bid Response Form in Addendum 1.

6. Sheet SD-7 shows chimney replacement with new top slabs. Does this apply to this bid? if so how many and what size is the existing lift station? What would be the riser vertical dimensions for each lift station? Does the hatches require fall protection? See revised Bid Response Form in Addendum 1. Yes, fall protection is required.

7. Please provide additional information on what is required for bypass pumping or it is to be provided by the owner. If bypass is required please provide gpm and total head pressure to be quoted. 8.0 Measurement and Payment 8.1 GENERAL 8.1.1 Measurement and payment will be based upon Work completed and accepted in accordance with the

Contract Documents. No separate payment will be made for excavation, trenching, backfilling, leakage tests, surveying, density tests or other incidental items of Work not shown in the Agreement.

8.1.2 EQUIPMENT AND MATERIALS IN STORAGE: Partial payment for materials and equipment in proper

storage at the site of the Work will be made for those items for which the Contractor has submitted the following:

A. Invoice for each item in storage. The invoice shall not exceed the value of the item stored as determined

by the amount paid to the manufacturer (subcontractor fabrication costs excluded).

B. List of items in storage. C. With the following pay estimate, a release of lien for 90% of each item listed as stored materials for the

previous pay estimate shall be provided. Any item for which a 90% release of lien is not supplied shall be considered as having been removed from the site.

No payment for any offsite storage of material will be made. Any payment for stored material shall not

include labor or profit by the Contractor or any sub-contractor. The Contractor is specifically advised that this procedure may require full payment for some stored materials or shop work significantly ahead of the time when payment is issued by the Owner.

8.1.3 MEASUREMENT

A. Payment shall be made at the unit prices as listed in the General Construction Services for Wastewater and Reclaimed Water Maintenance/Repair Unit Price Bid Sheet based on measured quantities as described below.

1. Mobilization/Demobilization: Lump sum price per individual project shall include but not be

limited to all mobilization/demobilization costs associated with mobilizing and demobilizing crews, equipment and materials to the job site, general conditions, permitting, permit compliance, superintendent, administrative duties, locates and any costs associated with the project not specifically addressed elsewhere.

2. Isolation Valve: Cost to remove an existing station isolation valve and install a new station isolation valve. See detail SD-1 and SD-27. Price shall include but not be limited to all labor, equipment, materials, dewatering, excavation, pipe cutting, old valve removal and disposal, installation new fittings, restraints, spool pieces, valve, valve box, backfill, and compaction. The District shall provide a maximum 4 hour shutdown for the contractor to perform the installation and place the station back into service. The District shall provide density testing services for backfill from an independent testing laboratory.

3. Temporary Bypass Connection Assembly: Cost to install a District provided bypass connection assembly. Price shall include but not be limited to all labor, equipment, materials, dewatering, excavation, pipe cutting, installation of bypass assembly, removal of bypass assembly when complete and reconnection of station to force main and/or isolation valve. The District shall provide a maximum 4 hour shutdown for the contractor to perform the installation

4. Wetwell Concrete Lid and Hatch: Price to install a rectangular or round concrete wetwell lid and hatch per Wetwell Concrete Lid and Hatch Detail. Price shall include but not be limited to

submittals, removal and disposal of the existing lid and hatch, new lid, hatch and fall protection, grading, leveling, brick, mortar, inspection coordination, back fill and compaction. The new lid and hatch shall comply with the District’s Manual of Minimum Construction Standards, Section 150 and Lift Station Standard Details SD-31.

5. Demolition of Wetwell Rectangular Chimney and Replacement with Cylindrical Wetwell Section: Price to demolish an existing rectangular wetwell chimney, lid and hatch and replace with a cylindrical Wetwell Section. The wetwell section shall comply with the District’s Manual of Minimum Construction Standards, Section 150, Section 121 and Lift Station Standard Details SD-31.

6. Valve Vault: Price to provide a new concrete valve vault per the Valve Vault detail. Price shall include all labor, materials and equipment for the installation including but not limited to excavation, grading, compaction, placement, backfill, vault, lid, hatch, sleeves, drain (including wetwell core and piping) and grout. The new valve vault shall comply with the District’s Manual of Minimum Construction Standards, Section 150, Section 121 and Lift Station Standard Details SD-31.

7. Valve Vault Piping Assembly: Cost to remove the existing lift station piping assembly (buried, above grade or in an existing vault) and replace with new valve vault piping and valving as detailed in the Valve Vault Piping Assembly details.

8. Above Grade Piping Assembly: Cost to remove the existing lift station piping assembly (buried, above grade or in an existing vault) and replace with new above grade piping as detailed in the Above Grade Piping Assembly detail.

9. Discharge Piping Assembly: Cost to remove the existing discharge piping assembly, brackets, supports and anchors complete and install the new Discharge Piping Assembly as detailed in the Discharge Piping Assembly detail. Price shall not include new Angle Support Brackets.

10. Angle Pipe Support: Price to provide one new Angle Pipe Support as shown on the Angle Pipe

Support Detail.

11. Alternate Angle Pipe Support: Price to provide one new Angle Pipe Support as shown on the Alternate Pipe Support Detail.

12. Leveling Floor: Cost to clean and prep the existing wetwell floor and install a min. 6” thick 4000 psi concrete leveling floor with reinforcing as detailed on the Leveling Floor detail. Cost shall not include but not be limited to removal and reinstallation of base plates, pump bases or piping required for the leveling floor to be installed, pressure washing, scouring of existing concrete surface, reinforcing, inspection coordination, concrete, concrete placement and concrete finishing. Concrete shall not drop more than 4’ when placed. Contractor shall use a tremie hopper and chute or other approved method for placement. All placed concrete shall be slumped tested. Slumps shall not exceed manufacturers mix design limits. All concrete shall have 4 compressive strength cylinders taken from placed concrete. Cylinders shall be broken on day 3, 7 and 28. One cylinder shall be held in reserve. Any concrete not meeting 70% design strength in 7 days shall be removed and replaced at the Contractor’s expense. The District shall provide testing services from an independent testing laboratory.

13. Base Plates: Price to install one (1) new base plate. Price shall include all materials and labor including but not limited to all labor, equipment, materials, base plates, anchors, epoxy and grout pad.

14. Pump Bases: Price to install one (1) new pump base on a base plate. Price shall include removal of the old and installation of the new pump base.

15. Drop Bowl Assembly (less than 10’ drop): Price to install a drop bowl assembly per the Drop Bowl Assembly detail. Price shall include all labor, materials and equipment including modifications to the fillet as required to accommodate the drop.

16. Abandon Outside Drop (less than 10’ drop): Price to abandon an outside drop with grout. Price shall include temporary plugging of upstream main, grout and all labor, materials and equipment to grout outside drop. At a minimum grout should extend from the influent piping down the drop 12” and extend from the wetwell wall into the drop 12”. The remainder of the drop may be filled with clean fill or suitably plugged to the satisfaction of the District.

17. Guide Rails Removal Installation: Price to remove and install guiderails per the guide rail detail. Price does not include the cost of the guide rails, brackets, anchors or fasteners.

18. Guide Rails: Price to provide new guide rails, brackets, anchors and fasteners as shown on the Guide Rails detail.

19. Panel and Panel Rack: Price for removal of existing meter, disconnect, junction boxes, panel rack and panel and installation a new panel rack per details provided in Electrical Panel Details and installation of new or existing disconnect, junction boxes, meter and panel on the new panel rack at the same or similar location within 25’ of the original location. Price to include all permitting, labor, equipment and materials including but not limited to aluminum pipe, stainless steel uni-strut and hardware, concrete base, PVC caps, conduit, cable, terminations including junction box terminal strips. New panels, disconnects, junction boxes and meters shall be owner provided and comply with Lift Station Structural and Mechanical Standard Details SD-31 through SD-34. This pay item shall include cost of permitting through the applicable agencies but shall exclude permit fees. Permit fees charged by the applicable agencies shall be reimbursed to the Contractor by the District.

20. Panel Installation: Price for removal of an existing panel, junction boxes, disconnect and meter and installation of the removed or new panel, junction boxes, disconnect and meter at the same or similar location within 25’ of the original location. Price shall include but not be limited to all labor, equipment, material, conduit, cable, terminations including junction box terminal strips. New panels, disconnects, junction boxes and meters shall be owner provided and comply with Lift Station Structural and Mechanical Standard Details SD-31 through SD-34. This pay item shall include cost of permitting through the applicable agencies but shall exclude permit fees. Permit fees charge by the applicable agencies shall be reimbursed to the Contractor by the District.

21. Concrete Demolition Slab: Price for demolition, removal and proper disposal of existing concrete slabs up to 8” thick with or without reinforcing. Price shall include but not be limited to labor, materials, equipment, hauling and disposal fees.

22. Concrete Demolition Structures: Price for demolition, removal and proper disposal of existing concrete structures up to 12” thick with or without reinforcing. Price shall include but not be limited to labor, materials, equipment, hauling and disposal fees.

23. Concrete (no reinforcing): Price to provide poured in place concrete pads without reinforcing, 3,000 psi. Price shall include but not be limited to submittals, grading and compaction, form work, permitting, inspection coordination, concrete and finishing. All placed concrete shall be slumped tested. Slumps shall not exceed manufacturers mix design limits. All placed concrete

25. Concrete (#5 @ 12” O.C.E.W., Top and Bottom): Price to provide poured in place concrete pads with #5 @ 12” O.C.E.W., Top and Bottom reinforcing, 4,000 psi. See Generator Slab Detail. Price shall include but not be limited to labor, equipment, materials, submittals, grading and compaction, form work, permitting, inspection coordination, concrete, reinforcing and finishing. All placed concrete shall be slumped tested. Slumps shall not exceed manufacturers mix design limits. All placed concrete shall have 4 compressive strength cylinders taken from placed concrete. Cylinders shall be broken on day 3, 7 and 28. One cylinder shall be held in reserve. Any concrete not meeting 70% design strength in 7 days shall be removed and replaced at the Contractor’s expense. The District shall provide testing services from an independent testing laboratory.

26. Stabilized Grass Driveway Section: Price to provide a stabilized grass driveway. See Stabilized Grass Driveway Section Detail. Price shall include but not be limited to labor, equipment, materials, grading, compaction, crushed limestone and sod. The District shall provide density testing services for subgrade and base from an independent testing laboratory.

27. Grass Paver Driveway Section: Price to provide a grass paver driveway. See Grass Paver Driveway Section Detail. Price shall include but not be limited to labor, equipment, materials, grading, compaction, crushed limestone, top soil, bahia plugs and concrete grass paver. The District shall provide density testing services for subgrade and base from an independent testing laboratory.

28. Adjust or Replace Manhole Frame and Cover: Price to adjust elevations on existing manhole frame and covers or to replace an existing manhole frame and cover with an owner furnished manhole frame and cover. See Precast Manhole Detail and Manhole Frame and Cover Detail for frame and cover installation requirements. Price shall include but not be limited to all labor, equipment, materials, brick, mortar, permit compliance and inspection coordination. Maintenance of Traffic for work in a roadway shall be paid under Item 27 – 30.

29. Ductile Iron Fittings: Price to install miscellaneous ductile iron fittings not otherwise accounted for in this bid and measurement and payment section. Price shall include all labor, equipment and materials including restraints as detailed in Force Main Thrust Restraint Chart and Force Main Thrust Restrain Detail

30. Relocation Radio Telemetry Unit (RTU): Price to relocate existing RTU within 25’ LF of control panel and wetwell. Price shall include but not be limited to labor, equipment, materials, conduit, cable, grounding, terminations in RTU panel and control panel. See RTU Detail.

31. Sod/Seed and Mulch

a. Sod: Unit price per SY to furnish and install all labor, equipment and materials for sod (Bahia or St. Augustine) as shown on the construction plans in conformance with FDOT Section 570.

b. Seeding and mulch: Unit price per SY to furnish and install all labor, equipment and materials for seeding mulch in conformance with FDOT Section 570.

32. Dewatering w/Wellpoints

a. First Day: Lump sum price for the first day for all labor, equipment and materials to furnish,

install and operate a complete dewatering system. Lump sum price shall include all costs specific to mobilizing and demobilizing dewatering labor, equipment and materials to the project site, permit compliance, installation, operation and maintenance and removal after no longer in use, abandonment of well points via grouting or other acceptable means to the Engineer and any other incidentals not specifically covered elsewhere for dewatering equipment and materials to provide a dry trench for work.

b. Each Additional Day: Lump sum price per day past the first day for all labor, equipment and materials to furnish, install and operate a complete dewatering system. Lump sum price shall include all costs specific to permit compliance, operation and maintenance and removal after no longer in use, abandonment of well points via grouting or other acceptable means to the Engineer and any other incidentals not specifically covered elsewhere for dewatering equipment and materials to provide a dry trench for pipe installation.

33. Maintenance of Traffic – Signage Only (work within FDOT, PBC, Martin County, Village of Tequesta, Town of Jupiter and/or Town of Juno Beach right of way with permit), Thoroughfare Road. a. First Day: Lump sum price to furnish and install all labor, equipment and materials for a

complete traffic control system in compliance with all applicable regulatory authorities. Lump sum price shall include all costs specific to mobilizing and demobilizing labor, equipment and materials to the project site, permit compliance, installation, operation and maintenance and removal after no longer in use.

b. Each Additional Day: Unit price per day past the first day to furnish and install all labor, equipment and materials for a complete traffic control system in compliance with all applicable regulatory authorities. Lump sum price shall include all costs specific to permit compliance, operation and maintenance.

34. Maintenance of Traffic – Signage Only (work within FDOT, PBC, Martin County, Village of

35. Maintenance of Traffic – Flagger: Lump sum price per day to provide two flaggers per day under

7.1.3 Measurement A.10. Cost shall include additional signage for lane closures or work in intersections.

36. Maintenance of Traffic – Sign Board: Lump sum price per day to provide one sign board per day under 7.1.3 Measurement A.10 and /or A.11.

37. Line Stops: Lump sum price per line stop to furnish and install all labor, equipment and materials

for installation of a line stop on an existing main. Payment shall include only cost of line stop contractor, materials (line stop fittings) and equipment (tapping and line stop machine) to perform the line stop. All other labor, equipment and materials associated with preparation for the line stop shall be paid under other applicable line items in the unit price bid schedule.

38. Trench Safety Act: Lump sum price per project for compliance with Chapter 553 of the Florida Administrative Code (Trench Safety act). Payment shall include all equipment, materials and labor necessary for compliance.

8.1.4 PAYMENT: Payment will be made at the lump sum or unit price for each item shown in the Unit Bid Price Schedule, stored and/or installed and accepted, which price and payment shall constitute full compensation for furnishing all materials and performing all Work in connection therewith and incidental thereto. The following schedule shall be adhered to:

A. Last Day of the Month – Cut-off date, confirm quantities with inspector. B. 5th of the Month - Pay estimate to the Engineer C. 10th of the Month - Pay estimate to the Owner. D. 20th of the Month - Payment by the Owner

When the cut-off date occurs on a holiday or weekend, the date shall be the last work day preceding the

end of the month. 8.1.5 PAY ESTIMATE FORMS: The Engineer will supply a pay estimate form for the Contractor to use for

submittal. The Contractor shall make copies to be used for submittal of the pay estimates. Failure of the Contractor to sign the pay estimate or attach appropriate documentation shall be grounds for returning the pay estimate with no action by the Owner or Engineer.

8.1.6 QUANTITIES: The Contractor is specifically advised that quantities listed in the Unit Price Bid Sheet

are estimates. Actual installed quantities may vary significantly. Further, quantities may be increased or diminished without invalidating the unit price in the Unit Price Bid Sheet
