SUBJECT: WELLNESS

District staff and The Gehring Group have been working together to explore Wellness options for

District employees. In general, healthy employees are happier, more productive, and have lower health

insurance costs. For all of these reasons, the District desires to implement a Wellness program.

In the past the District had a wellness program that included a Jupiter Hospital nurse coming to the

District on a quarterly basis to conduct wellness screenings for District employees. As a result of this

program, several employees benefited from early detection of various health issues. Therefore, District

staff have seen first-hand the benefits of having a program in place. Unfortunately, this program was

discontinued when Jupiter Hospital began charging for the service.

At this time, I believe the challenge lies in developing a roadmap for implementation of an effective

Wellness program. We are not alone in facing this challenge, e.g.,

A. Good Health Begins with Preventative Care – this flier shows what preventative care is already

covered by the District’s Employee Health Insurance.

B. DRAFT Wellness Program Development Plan – a potential 3 year roll out plan

C. DRAFT Wellness Program Metric Goal Options – some of the most common Wellness metrics

and alternate activities. To my surprise, most metrics are required to have an alternate activity

(e.g., a tobacco user can get the non-tobacco user credit if they complete the alternate activity

such as complete a tobacco cessation program).

D. Two examples of wellness programs, including MCSO Wellness (Martin County Sheriff’s

Office) and Living Well in PSL (the City of Port St. Lucie).

A successful Wellness program will require broad-based support and participation among employees

MARTIN COUNTY SHERIFF’S OFFICE - WELLNESS PROGRAM1

Mission Statement:Martin County Sheriff’s Office is dedicated to improving the health and well-being of its employees and their families. MCSO Wellness was created to provide employees with the resources needed to reach and maintain the healthiest lifestyle possible. Healthy employees will have a positive impact on the organization as a whole, and individual employees will experience the benefits of better health both inside and outside of work. For MCSO Wellness to meet its goals, each employee must take responsibility for their individual health and use the services provided by MCSO Wellness to continue their healthy habits or make a lifestyle change for the better. It is time to move the culture of Martin County Sheriff’s Office to one that fosters employee health and wellness.

Program Overview:This is a voluntary program that is offered to all employees who are enrolled in the Sheriff’s Office medical insurance plan (both PPO and HDHP participants are eligible). Wellness points will be earned for completing various wellness activities, and all employees who earn 100 or more points will be entered to win one of four grand prize drawings. A grand prize will also be awarded to the employee who earns the most points throughout the 2013 – 2014 plan year. Note: All employees who are tobacco free (or complete a tobacco cessation program) will be rewarded with savings in payroll deductions each pay period. New Hire employees on the HDHP will also receive HRA funds upon completion of their Health Risk Assessment and Biometric Screening. See page 3 for monetary incentive details.

GETTING STARTEDThe points incentive program begins on January 1, 2014 and ends September 30th, 2014. Verification of all Wellness Initiatives and activities must be submitted to the Human Resources Unit no later than September 30th, 2014 to qualify for points incentive. MCSO Wellness cards may be used as a form of verification for MCSO Wellness Sponsored exercise programs only. On the card, 15 verified slots (initialed by program instructor) will equal 4 MCSO Wellness points.

FAQsWhat is a Biometric Screening?When you schedule an appointment with the Employee Health Center, you will participate in a general health check, which includes lab work (blood draw) that requires fasting. The lab work will include a metabolic panel, cholesterol, thyroid, and more. The confidential results of the lab work will be reviewed with you during a follow up visit. If needed, you and the physician will create a plan of action to improve your overall health and well-being.

What is a Health Risk Assessment?The Health Risk Assessment is a confidential questionnaire that can be completed online at www.employeewell.com under the “WellSource” tab. This data is used in an aggregate report to help MCSO decide which programs would most help employees improve their health and well-being.

How Do I Schedule an Appointment?All employees and their dependents enrolled in the medical insurance plan have free access to the Employee Health Center. To schedule, visit www.employeewell.com or call (772)872-7304. Please let them know you will be coming in for your Biometric Screening. Informing them of this is important, as some blood work requires fasting.

PHYSICAL ACTIVITY INCENTIVE POINTSTo qualify for this incentive you must provide proof of exercise for 12 days of each month in which you wish to receive credit. Proof may be provided in the form of a monthly calendar template with entries written, a signed verification from an instructor, a print out from your fitness facility, or other equivalent means of verification. You must submit proof for each month for which you are requesting credit.

To qualify for incentive points you must write the name and date of the event on your tracking tool and have your tracking tool signed by the Wellness Committee member present at the event. Other 5k events will qualify for the incentive, but the cost of participation will not be covered. To qualify for incentive points for other 5k events, verification is required in the form of a race bib, or registration receipt, with your walk or run time written on it.

Lunch and Learns: Various topics will be covered during the lunch and learns offered throughout the year. To qualify for these incentive points you must write the title and date of the Lunch and Learn event attended, and have your wellness tracking tool signed by the Wellness Committee member at the event.

Wellness Series: Wellness series may include but are not limited to weight management programs, nutritional counseling, specialized fitness series, etc. Participation is verified by the series instructor confirming your attendance during the series.

Any employee who does not submit the affidavit or does not complete the cessation course will be charged the regular payroll deduction. An employee who falsifies these documents and/or fails to be truthful will be subject to disciplinary action as defined in MCSO’s Standard Operations Procedures.

NEW HIRES ONLY: HDHP 2013 – 2014 WELLNESS INCENTIVENew Hire employees who have enrolled in the new High Deductible Health Plan (HDHP) with Florida Blue can earn money in a Health Reimbursement Account (HRA) for participation in the Employee Health Center’s Biometric Screening and online Health Risk Assessment. Upon completion of the Health Risk Assessment and Biometric Screening at the Employee Wellness Center, New Hire employees on the HDHP Plan will earn HRA funds as listed below:

After New Hires complete the Health Risk Assessment and Biometric Screening, the clinic will give you a completed Wellness Incentive Form. Bring this form to the Human Resources Unit for processing of your HRA money. New Hires have 90 days from their date of hire to complete the criteria and submit the physician signed form to Human Resources in order to receive your HRA funds for the 2013 – 2014 plan year (Please note: These documents are currently included in the insurance premiums and are not savings in addition to the current premium).

Living Well in PSL Wellness Program Creating a Culture of Health & Wellness

The City is committed to your good health and overall wellness. The goal of Living Well in PSL is to create a culture of health and wellness within the City of Port St. Lucie by providing wellness initiative programs and activities throughout the year to engage and encourage employees to develop or maintain healthy lifestyles. The City is excited to provide you with resources that will allow you to maximize your overall health and well being. This is a voluntary program that is offered to all employees who are enrolled in the City’s health plan. You will earn an incentive for each wellness target achieved or each management program completed, and for each wellness incentive activity that you participate in. Incentives can total up to $500, and the total incentive earned may be used through a pre-tax Health Reimbursement Arrangement (HRA) account, as a taxable reduction in your medical payroll deductions for fiscal year 2014 – 2015, or as a taxable one-time lump sum received in October 2014. Getting Started The program begins on October 1, 2013, and you must have your wellness exam, target numbers or management

programs, and wellness incentive activities completed by September 30, 2014. Below is an overview of the

Employees may achieve any or all targets. The total incentive amount earned may be used through a pre-tax Health Reimbursement Arrangement (HRA) account, as a taxable reduction in your medical payroll deductions for fiscal year 2014 – 2015, or as a taxable one-time lump sum received in October 2014. It is the goal of the City to make this wellness program available to all employees enrolled in the City’s health plan. If you have a condition that would make it medically inadvisable or otherwise prohibit you from achieving the targets and therefore realizing the benefits of this program, the City encourages program participants, with the help of their physicians, to present a reasonable alternative plan.

How it works

To participate in Wellness Initiatives (refer to Table 1), schedule a Living Well in PSL wellness program appointment*

with the Employee Family Health Center at (772) 807-4430 and complete a Health Risk Assessment (HRA) at

https://tcma.hra.net/. You will automatically receive an incentive for completing the HRA and biometrics screening

between October 1, 2013 and September 30, 2014. You can earn additional incentives by meeting target numbers or by

participating in the corresponding wellness Management Program described below:

Weight Management Program: This program is coordinated through the Employee Family Health Center and will help

you start implementing behaviors to lose weight and keep it off! The program incorporates diet, exercise and, for some

participants, weight loss medication to reach weight loss goals that are tailored for each individual. Length of program:

approximately 6 months.

Diabetes Management Program: Uncontrolled diabetes increases the risk of a number of complications that can affect

nearly every organ inside the body, including the heart and blood vessels, eyes, kidneys, and nerves. This program is

coordinated through the Employee Family Health Center and will review the importance of understanding your blood

sugar numbers as well as diet and exercise strategies to control blood sugar. Length of program: approximately 6

months.

Cholesterol Management Program: Elevated cholesterol contributes to the risk of heart disease and stroke. This

program is coordinated through the Employee Family Health Center and will provide nutrition and exercise strategies to

lower and control cholesterol levels. Length of program: approximately 6 months.

Hypertension Management Program: Geared for those individuals with high blood pressure or elevated blood pressure,

this program will incorporate diet and exercise to help lower blood pressure. Medication will be utilized when

necessary. This program is approximately 6 months in length and is coordinated through the Employee Family Health

Center.

Tobacco Cessation Program: This program is a varied program designed to find the best way to help you kick the habit and is coordinated through the Employee Family Health Center. Since tobacco use is a risk factor for many cancers, heart disease, stroke, and other chronic illnesses, cessation is necessary to have a healthy lifestyle. Length of program: approximately 6 months. *Living Well in PSL wellness program appointments are specifically wellness appointments only, and are not to be used

for other medical issues such as prescription refills or illnesses.

To participate in Wellness Incentive Activities (refer to Table 2), you will decide which Wellness Incentive to engage in

this year. This year’s Wellness Incentive Activities and completion criteria are described below:

Living Well in PSL Programs: These programs include outside organized fitness events, healthy eating and weight loss

programs, and specialty fitness activities other than Exercise Participation. To receive the incentive, you must complete

2 out of 3 activities in this category. Living Well in PSL Programs are not limited to, but could include activities such as:

Training for and completing a 5k run/walk, triathlon, marathon, bicycle race, strength competition, etc. Proof of completion required.

Participating in Weight Watchers (or equivalent) or nutrition counseling one time per month for at least 6 months. Proof of participation required.

Participating in Yoga, Zumba, Kickboxing classes (other exercise types qualify), or an at-home workout program 3 times per week for at least 6 months. Written confirmation required in the form of an exercise log or sign off from class instructor.

Living Well in PSL Challenges: These challenges will be a fun way for all employees to compete in a fitness or wellness challenge. This is an exciting new way we will encourage employee engagement in wellness programs and create a culture of health and wellness in the City of PSL! These paid challenges are run through Live Healthy America (or equivalent). You can at www.livehealthyflorida.com. Use participant code, “LHPSL”, to access our challenge page. The employee will pay for the challenge up front and will receive a $50 incentive if one challenge is completed.

Challenges will be run during a set time frame and will focus on important wellness initiatives. o October – Step 2 It ○ January – Wellness ○ June – Nutrition ○ August – Lifestyle

To qualify you will have to achieve pre-determined completion standards for one challenge. Exercise Participation: To qualify for this incentive you must exercise 12 times per month for the whole fiscal year. You

can only qualify for one of the following incentive options:

For a paid exercise program membership, you can qualify for a $200 incentive. Proof of participation in a written

confirmation from the program instructor is required. The following local gyms have also agreed to provide reports to

confirm your monthly workouts:

Humana Fitness & Wellness Center at the Civic Center

LA Fitness

Anytime Fitness

YMCA Treasure Coast For personal exercise methods, you can qualify for a $50 incentive. Proof of participation via tracking your daily exercise by completing a monthly workout log and submitting to Human Resources with your signature is required.

‘Did You Know?’ Seminars: The City will be providing educational seminars on a variety of Health, Fitness and EAP

Topics. Levels will range from beginners level to advanced level. You will be asked to sign in for each seminar in order to

receive credit for attendance in each.

All Living Well in PSL Measurements, Activities, and Targets must be completed between October 1st, 2013 and September 30th, 2014 to qualify for the incentive payment. You must be employed and on the City’s health plan to be
