Cerebral palsy (CP) is a diagnosis of considerable concern to obstetricians and parents alike. Evidence points to a pregnancy and/or perinatal origin of many cases of CP. It is likely that no other neurodevelopmental disorder is so strongly tied to phenomena occurring in pregnancy and the perinatal period. At the same time, evidence isnow overwhelming that the labor process, andthe intervention or nonintervention of practitioners inthatprocess,isonlyaminorfactorintheoverallCP picture. The importance of CP stems from its high frequencyinthepopulationanditsseverity.Witha prevalence of about 1 in every 500 school-age children(1),CPisamongthecommonestofthechronic disabilities of childhood. About 30% of children with CP cannot walk, and nearly 20%walk only with an assistive device (1). About half of the children with CP have one or more additional disabilities, most commonly a seizure disorder,impairment of cognitive capacity, or difficultieswith hearing or vision (1). CP is also a critically important outcome in very low birth weight or premature infants. Because of the rapid evolution and application of perinatal and neonatal intensive care in recent decades, verypremature infants now survive in unprecedented numbers, and have contributed to an increasein the population prevalence of CP (1). The most common causes of CP are hypoxic, ischemicbrain injury, periventricular leukomalacia orintraventricular and/or parenchymal hemorrhage that occur in the first year of life (2). What iscrucial to know is that to date no cure for CP is available and current treatments are targeted to maintaining function, relieving contractures,improving nutrition and providing developmental

Itis therefore very encouraging that reports are emerging in the successful use of adult stem cells inCPandneonatalhypoxicbraininjury.OurTHAI StemLifefamilystemcellbankwasthefirstbankintheworldtopublishinapeerreviewedjournal,two cases of own cord blood stem cell use along with G-CSF in CP (6) where both children noticed improved mobility and decreased spasticity! Other groups have also published their results regarding safety and feasibility in using own cord bloodinCP(7),whileothersreportedencouragingimprovements in traumatic brain injury with own adult stem cells (8). Published reviews recognize theneedtoexploretheuseofowncordbloodinneonatalischemicbraininjury(9)andinThailandTHAI StemLife is in the process of organizing a multicenter study to further elucidate the potential of using own stored cord blood to alleviate symptoms of the condition. All these interesting and groundbreaking developments will be available to all THAI StemLife parents soon. TSL : Could you please tell me about the different

conditions in children causing intellectual disability

syndromes at birth?

Prof. Dr. Teerachai : There are several types of

intellectualdisabilitysyndromes,butthemajorones

can be categorized into two groups.

The first are those with intellectual disabilities that

become more apparent as the children grow.

The development of the brain does not go hand

in hand with their age; they learn and develop

more slowly compared to other healthy children.

At a certain point, their development might stop

completely. These conditions result from the lack or

accumulation of certain substances; it may be lack

ofgrowthhormone,whichpromotesgrowthofbody

tissues or hormones such as those in the thyroid

gland.Alternatively,accumulationofsubstancesdue

to errors in biochemical pathways of the metabolism

can be other underlying causes.

The second type comprises those with

intellectual disabilities that remain fairly stable over

a child’s life span. There are many possible causes

for this including complications or incidents during

pregnancy or during birth. This may lead to an injury

to the brain or head resulting in permanent physical

impairment, known asCerebral Palsy (CP). Two in

1,000 live births or 0.2% are diagnosed with this

condition.

The difference is that in the second group,

provided that incidents such as injuries or

abnormalities to the brain are not visible, cerebral

palsy will not be diagnosed immediately after birth.

However,itmightbecomeobviouswhenchildrenget

older.Forachildtobediagnosedwithcerebralpalsy,

they must be outside the normal ranges for a child’s

age,growthanddevelopment.Theeffectsofcerebral

palsy fall on a continuum of motor dysfunction for

instance,abnormalitiestothebraincanresultinthe

inability to walk, difficulties with visual or auditory

processing,difficultiesinrelationshipwithpeers,joint

deformities and contractures, speech control, and

abnormal muscle tone.

TSL : Is it possible to precisely identify the cause of

aintellectualdisabilitysyndromeatbirth?Ifnot,what

are some of the possible causes for this disability? Is

thereawaytopreventthis,andhow?

Prof. Dr. Teerachai : Obvious identifiable causes

include low thyroid or other hormone production,

injuries to the head, infections, or encephalitis.

However, many other causes are elusive and

unknown because they happen during a pregnancy;

for example, a baby may not get enough oxygen

during laborand/orbirth. Incases like this,precise

causes are not known since what we can observe is

theresultofaparticularcourseofevent;forinstance,

if a baby does not cry or breathe spontaneously after

birth,abreathingmachineforrespiratoryaidisgoing

In case of preterm delivery, a myriad of the

aforementioned situations frequently occurred in

thepast,butatpresent,thesurvivalrateofpreterm

infants is much higher. Nonetheless, they are at

risk for respiratory failure causing lack of oxygen

to the brain. Consequently, insufficient oxygen

and nourishment for cell development in the brain

leads to severe intellectual disabilities and growth

deficiencies.

Another pregnancy complication is that vaginal

delivery can result in babies being born with a nuchal

cord (the umbilical cord wrapped around the baby’s

neck),whichpreventsoxygenandnutrientsfromthe

placenta to reach the baby.

In regards to preventive measures, such are

applicable in only some of the conditions. Detection

of thyroid gland dysfunction, hormone and other

substances can be administered, proper prenatal

care,andgoodnutritionalsupportcanbeeffective

at reducing the risks of preterm birth. Genetic testing

or carrier screening for abnormal genes is also an

efficientapproachtoavoidcertaingeneticdiseases

suchasanemia,whichisoneofthemostcommon

blood disorders nowadays.

TSL : How should parents handle a child with

intellectual disability syndrome at birth?

Prof. Dr. Teerachai : If facedwith this issue, you

need to have your child diagnosed as to whether

his/her condition can be cured and how to prevent

the spread of a possible infection. The key is to

think about what you need to do in order to provide

a good life for your child so they can survive in the

community.Forinstance,youmayconsiderphysical

therapy. The parents need to understand that

such conditions are caused by natural order and

that no one is at fault. Also note that certain kinds

of medications for pregnant women may result in

variousformsofdisabilities;forexample,anti-nausea

medications for pregnancy sickness can cause an

infant to be born with birth defects including no arms

or yellow teeth.

TSL :Owingtotheinnovationsofstemcellresearch,

we can now treat children with intellectual disability

syndrome using their own cord blood cells. Could

you describe the process of this treatment and how

safe it is to be performed on children?

Prof. Dr. Teerachai : Duke University Medical

School in the United States has studied and practiced

medicineinthecelltherapyfieldfortreatingchildren

with inborn errors of metabolism. This research holds

huge promise for children with intellectual disabilities

the development of therapies to compensate for

what they have lost and that further damage can

bearrested. Inotherwords, thesebenefitscanbe

compared to car insurance, which is expensive

and tends to scare people off. But if an accident

did happen, obviously it’d be

great if someone were there to

help you out, similarly to stem

cell treatment. More importantly,

there will be new technology to

advance this process in the very

near future.

Duke University has also

reported that over a course of

stem cell therapy, patients with

inborn errors of metabolism have

shown signs of development,

despite the uncommon nature of

their disease. Nonetheless, a lot

more research needs to be made

in the future.

In Thailand, the Medical

Council has limited the scope of

approved treatments exclusively

toresearch.However, therehave

been reported cases of children

with both motor and intellectual

disabilities using their own stem

cells and side effects were not

present.Infact,theyimprovedso

much that they became almost

not much different from other

healthy children. Thus, if stem

cell treatments are accepted and

approved by the Medical Council

of Thailand, we’re literally talking

about saving and/or improving the

livesof2in1,000children.

TSL : From which part of the body

do they collect the stem cells in

this case?

Prof. Dr. Teerachai : Umbilical

cord blood stem cells are easily

isolated,sincetherearenumerous

in cord blood, so stem cells are

easily collected with a simple

cord blood collection at birth in

newborns. Other methods include

bone marrow aspiration that can

be painful and time-consuming

or peripheral blood cell harvest

that is much easier and preferred

in adults! But for the conditions

we are discussing, cord blood

collection is preferred .

As far as I’m concerned, a myriad of diseases are curable,particularlyconditionsassociatedwithblooddisorders;forinstance,leukemias (abnormal white blood cells), marrow diseases, redblood cell disorders, primary immunodeficiency disorders, breastcancer,andovariancancer.Othermedicalconditionsconcerninggeneralbodydeterioration includemyocardial ischemia,diabetes,and coronary artery diseases. I believe that with today’s rapidly-advancing technology, and newmedical knowledge, doctorswillsoon be capable of developing treatments with stem cells in the near future. There are three different methods to collect stem cells: 1) cord blood stem cells in newborns 2) peripheral blood stem cells in adults and 3) bone marrow stem cells. TheSongkran tradition is the celebration of the traditional ThaiNewYear, usuallybetween April 13-15, every year. Normally, schools and educational institutes willhave already closed for some time and students will be free. This is the moment for family reunionsandcelebrating the festival together.During theSongkranFestival,it’s not only water throwing activities but there will be a lot of traditional activities to promote love and happy feelings among Thai families also. The traditional activities to celebrate the Songkran Festival that Thai people have been practicing for years start by making merit at the temple on April 15th and pouring water to the monks as well as pouring water to the Buddha images at home. In the afternoon, each housewill arrange the pouringwater ceremony to the seniors inthehousetoexpressgratitude.Intheceremony,theyoungermemberswillprepareflowers, incent, candles, new clothes, betel nut and fragrance to present to theseniors.
