Proposed research: While most patients with childhood-onset chronic kidney disease (CKD) nowadays survive into adulthood, their cardiovascular (CV) morbidity and mortality at young adult age is excessive due to extensive cumulative exposure to non-traditional CV risk factors associated with uremia. Therefore, the prevention, diagnosis and treatment of early CV disease is becoming a primary objective of pediatric CKD management. While early CV disease has been observed in children with even moderate CKD, the causative factors and natural evolution of CV morbidity and their relationship to CKD progression are incompletely understood due to a lack of prospective studies. Due to the absence of vascular pathology secondary to ageing, diabetes and smoking, the pediatric CKD population should permit a uniquely sensitive assessment of associations between early CV lesions and biochemical and genetic risk factors. The ESCAPE Network, a consortium of pediatric nephrology centers in 13 European countries, has previously generated fundamental information about CV abnormalities in pediatric CKD. Building on this expertise, the Network is launching the Cardiovascular Comorbidity in Children with CKD (4C) Study, a long-term observational study exploring the prevalence, extent and progression of CV pathology and its association with CKD progression. The morphology and function of the heart and large arteries will be monitored by sensitive non-invasive methods in 700 children aged 6-17 years with eGFR 10-45 ml/min/1.73m². Age-specific reference cohorts will be studied in parallel. Arterial biopsies obtained on occasion of access or transplant surgery will be characterized pathoanatomically and pathochemically. Multiple potential clinical, anthropometric, biochemical and pharmacological risk factors will be monitored and related to the evolution of CV status. A genome-wide association study will be performed to identify common genetic variants associated with CV disease and/or CKD progression. Monitoring will be continued when ESRD is reached to study effects of different renal replacement therapy modalities.

Aim of the research: The 4C Study is an observational longitudinal study in children and adolescents with advanced chronic kidney disease designed to address the following research topics: • Distribution of indices of large artery morphology (carotid intima media thickness, cIMT) and function (aortic pulse wave velocity, PWV) in healthy children • Prevalence of left ventricular hypertrophy (LV mass index) and myocardial dysfunction (cardiac strain) in children with CKD • Prevalence of abnormal large artery morphology (cIMT) and function (PWV) in children with CKD • Change of cardiac and arterial morphological and functional abnormalities during CKD progression • Impact of dialysis and transplantation on cardiac and arterial pathology in children with end-stage renal disease • Impact of conventional, uremia- and dialysis-associated risk factors on cardiac and arterial abnormalities in children and adolescents • Predictive value of established and novel serum and urine biomarkers for cardiac and vascular abnormalities in children with CKD • Association of common genetic variants with progression of CKD and cardiovascular disease in children and adolescents with CKD (genome wide association study) • Impact of uremia and dialysis on arterial tissue morphology, gene and protein expression in children with end-stage renal disease •Identification of novel risk factors and biomarkers of kidney disease progression in children with CKD •Identification of common of genetic variants associated with kidney disease progression in children with CKD

Progress and results as of July 2017: Reference values for cIMT and PWV aged 6 to 17 years were obtained in 1,000 healthy children in Germany, Turkey and Poland. Eight regional coordinators were jointly trained in performing cardiovascular assessments and equipped with mobile cardiovascular ultrasound and oscillometric PWV measurement devices. A total of 704 children and adolescents (aged 6 to 17 years) with CKD stages 3b to 5 were successfully enrolled in the study by the end of 2011 at 55 pediatric nephrology centers in 12 European countries. The patients have been undergoing 6 monthly clinical examinations and annual echocardiographies, ambulatory blood pressure measurements, cIMT sonography and PWV measurements, together with regular blood and urine sampling. A central biorepository has been established in Heidelberg. In June 2016 approximately 60% of the enrolled patients have had four-year follow-up visits. 290 patients have dropped out of the study, mostly due to loss to follow-up or transition to adult care. Approximately 40% of the patients have started renal replacement therapy: 201 patients received renal transplant, 101 and 85 started hemo- and peritoneal dialysis, respectively. Approximately 70-90% of patients have completed cardiovascular assessments (carotid artery sonography, echocardiography, pulse wave velocity and analysis) at the annual visits. Out of the wealth of data emerging from the study, the following key findings are particularly noteworthy:

• According to ABPM 9% of children with CKD had uncontrolled, 7% masked and 30% drug-controlled hypertension. Only 39% were normotensive without treatment.

Isolated nocturnal hypertension in children with CKD was associated with increased IMT SDS and PWV SDS, but not LVMI.

Children with CKD have impaired systolic and diastolic cardiac function as evident by tissue Doppler imaging, which is related to kidney function and hypertension. Additionally, progressive mechanical dysfunction of the left ventricle was shown during a 4 year follow-up period, as measured by strain echocardiography.

Arterial biopsies from children with CKD revealed increased calcium content and IMT. Gene transcription analysis shows significant differences in the regulation of osteogenic proteins, calcium-transporting proteins, inflammatory cytokines, and especially extracellular matrix proteins. These findings most likely reflect an early stage of a CKD-specific calcifying arteriopathy.

• LVH was present in 33%, 47% and 58% of children with CKD stage 3, 4 and 5 respectively; the LV mass index increased significantly with observation time and was correlated with blood pressure, eGFR and BMI. The cardiac strain analysis revealed regional anomalies of LV wall contractility despite preserved global systolic function (ejection fraction).

• At the first visit cIMT was elevated in 40% and PWV in 23% of patients. cIMT steadily increased with time, indicating progressive arteriopathy in children with CKD.

• Renal transplantation led to rapid normalization of PWV, whereas IMT and LV mass did not change within the first post-transplant year.

• Metabolic acidosis was shown to be associated with poor growth and hyperparathyroidism but not with disease progression.

• suPAR, the soluble urokinase receptor, was identified as a novel predictor of CKD progression in children with mild-to-moderate CKD. The association of suPAR with CKD progression was independent of baseline eGFR, proteinuria, blood pressure and the type of underlying kidney disease.

Numerous biomarkers including a bone marker panel, vitamin D metabolites, inflammatory markers (CRP, fetuin A…), uremic and vasculotoxins (indoxyl sulfate, p-cresyl sulfate, homoarginine, ADMA, homocyteine…), and markers of carbonyl, oxidative and cardiovascular stress (cTnT, BNP, GDF15, ST2) have been measured and correlated with the cardiovascular endpoints.

Genome wide SNP arrays (Illumina Omni 2M) have been run in 700 patients. A Genome Wide Association Study (GWAS) was performed in a total population of 1200 children with CKD, combining the 4C Cohort with the ESCAPE trial cohort and the North American CKiD Cohort in the NIH co-funded PediGFR Study. This landmark study yielded several novel loci associated with kidney survival in children with CKD.
