The gold standard of H. pylori screening is recommended by the Gastroenterological Association of Thailand
UBTSimple screening method, widely used in developed countries, but not in Thailand due to its high costs (2,500 Baht/case)

Conclusions 1. ยดึตามแนวทางปฏิบติัของสมาคมแพทยร์ะบบทางเดินอาหารแหง่ประเทศไทยในหน่วยบรกิารท่ีมคีวามพรอ้มในการตรวจทางหอ้งปฏิบติัการโดยเฉพาะ RUT แต่ในโรงพยาบาลท่ีไมม่อุีปกรณ์ในการตรวจวนิิจฉัยเชื้อ H. pylori ก็สามารถใชว้ธิ ีtherapeutic diagnosis ในการรกัษาผู้ป่วยท่ีมีอาการ dyspepsia ซึ่งไมต่อบสนองต่อการได้รบัยาลดกรดและยารกัษาแผลในกระเพาะอาหารได้เลย แต่อยา่งไรก็ตาม การรกัษาด้วยวธิ ีtherapeutic diagnosis น้ีอาจต้องคำานึงถึงผลขา้งเคียงท่ีอาจทำาให้เกิดภาวะเชื้อด้ือยามากขึน้ในอนาคตด้วย
2 . ลดต้นทนุในการตรวจวเิคราะหเ์ชื้อ H. pylori โดยเฉพาะ RUT ซึ่งถกูแนะนำาใหเ้ป็น gold standard ทัง้น้ีเพื่อใหก้ารตรวจวนิิจฉัยทางหอ้งปฏิบติัการก่อนการใหก้ารรกัษามคีวามคุ้มค่าทางเศรษฐศาสตรม์ากขึ้น นอกจากนี้ มีความจำาเป็นอยา่งยิง่ในการเพิม่จำานวนและศักยภาพของหอ้งปฏิบติัการในระดับภมูภิาค เพื่อตรวจวนิิจฉัยการติดเชื้อ H pylori เพิม่มากขึ้นด้วย
