Note to ApplicantsThis Full Application Form and Instructions (Application Form) has been designed to comply with the Delta Conservancy’s 2018-2019 Grant Guidelines. By completing this application, its attachments, and the requested supplemental documents, the applicant will be compliant with the requirements of the Delta Conservancy’s Grant Guidelines.

Applicants are advised to carefully review and consider the Grant Guidelines and full proposal evaluation criteria when preparing their applications. The evaluation criteria and their point values are summarized in the Evaluation Criteria table below. In addition, throughout this document, the criterion that will be satisfied by the information provided in a particular section is noted below the header of that question. Multiple responses may relate to a single criterion or a single response may relate to multiple criteria.

Applicants who are selected to receive funding may be required to submit additional information about their projects during the development of the grant agreement. The Delta Conservancy reserves the right to request more information to clarify and complete grant agreements.

Document Submission Process and Deadline

This Application Form and all relevant attachments and supplementary materials must be received by the Delta Conservancy by 5:00 pm on December 18, 2018. All files should be submitted electronically via large file upload (instructions will be provided separately to all concept proposal applicants). The system will not allow for any materials to be uploaded later than 5:00 pm on the application due date.

Please submit the Application Form as a Word document. The Application Form should not exceed 50 pages, including all the text provided in the blank form. Do not delete or modify any text in the blank form. Attachments and supplemental documents are not included in the 50-page limit. Each applicant is required to complete all fields in the Full Proposal Application Form unless noted exceptions apply.

2 Project Description

2.1 Project Description: Purpose and Implementation Included in criterion 1. Include a detailed project description that can serve as a statement of work for a grant agreement:

1) Explain the purpose of the project, and the need for the project. 2) Explain the history of the project. If the proposed project is part of a multiphase project, outline

addresses project objectives. The objectives should be the same as those described in Attachment 7: Performance Measures Table.
