Dear friends!

Cancer - not a verdict but a diagnosis, Weve beatencancer, Cancer can be cured these slogans are rather

common both in printed and electronic media. But the

Cancer registry data tell a different story: even though

cancer is a diagnosis, and weve beaten and cured,

there are more people dying of cancer in Ukraine than in

the developed Western countries. They die of a potentially

curable illness, while observance of certain conditions

could save many lives. Those are not mere conditions for

LISSOD, they are the laws to which activities of the clinic

have been subjected since its establishment. I want you

to know about them because competent treatment of

information is life-saving today.

Required are knowledge and experience of physicians, their

non-indifferent attitude to the patient, a comprehensive

approach to treatment, modern equipment, use of

innovative technologies and international medical and

diagnostic standards. The indispensable combination

of all of these factors lies in the heart of the hospital

activities. Many Ukrainian health facilities today have

Clinical oncologist is neither a surgeon nor a

chemo- or radiotherapist. It is a specialist withprofound knowledge of modern methods

of cancer diagnosis and treatment, tumors

biology and immunology, molecular biology;

well-grounded in pathological anatomy and

histopathology basics as well as instrumental

and laboratory methods of research. Clinical

oncologist makes a plan of examinations, makes

diagnosis and determines disease stage on the

basis of test results; predicts the disease course, determines the therapeutic tactics taking into account

the sensitivity of tumor and the feasibility of radio-, chemo-, biotherapy and / or surgical treatment;

well-grounded in rehabilitation basics. It is far from being the complete list of knowledge and skills the

LISSOD clinical oncologists possess. Our patients rst appointment is held with the leading foreign

oncologists as well as high-class Ukrainian physicians who have undergone training at major cancer

centers in Europe and Israel.

In difcult and controversial cases we always gather Tumor Board involving several clinical oncologists,

chemo-, radiotherapists and histopathologists. It allows excluding the possibility of human mistake.

With the use of the hospital telecommunications system we hold consultations with the world leading

oncologists in real time.

Individual approach to each patient is the core principle of LISSOD. An administrator, available 24/7

Dr. Bernstein made a signicant contribution to the development of radiotherapy,

having worked for many years in the Department of Oncology of Rambam Medical

Center (Haifa, Israel). He formulated the fundamental principles in planning three-

dimensional therapy of gastric cancer. Dr. Bernstein's works are used as guidelines by

the European Organization for Research and Treatment of Cancer (EORTC). In 2008,

Dr. Bernstein was awarded the honorary certicate of the Israel Cancer Association

for his contribution to the development of oncology.

ELIZAVETA DUDNIK-ILSTEIN

Worked as an oncology consultant at the following clinics and departments:

Hillel Yaffe Medical Center (Hadera , Israel), Internal Medicine Department and

immediately after internship he defended his PhD thesis on Methodological aspects

of endovideosurgical formation of organ anastomoses in portal hypertension.

Surgical Oncology. In 1993 he graduated with honors from the Bogomolets NationalMedical University. After internship in surgery he worked in the Gastrointestinal

Bleeding Center at Kiev City Hospital #12 under the supervision of Prof. V.D. Bratus

and Prof. P.D. Fomin.

Over the last 2-3 years there has been a signicant

progress in cancer treatment, comparable to a revolution

in oncology. At present we can tell with condence that

more than 60% of patients can be cured. This is true for

absolutely all cancer patients, not only for patients with

early stages of cancer (data of the U.S. National Cancer

Institute). These results can be achieved only through

the use of international medical standards in cancer

treatment. Full application thereof at LISSOD places

Ukraine in one row with the most developed Western

countries.

In radiotherapy we use linear accelerators of the latest

generation. The use of linear accelerators in combination

with three-dimensional planning, involving computed

tomography and sophisticated computer programs

enables getting a three-dimensional image of the tumor

and healthy organs. The necessary dose of radiation

is focused on the tumor while the inuence on the

surrounding organs and tissues is minimized. Therefore,

the frequency of side effects and complications after

radiation therapy is gradually reduced.

Ensuring quality radiotherapy treatment requires

not only linear accelerators but also specially trained

specialists. For 5 years already, starting from March

2007 a team of highly qualied radiation therapists,

technicians, medical physicists and engineers trained

abroad has been working in LISSOD.

Electron irradiation provides accurate treatment of

supercial tumors without damaging organs located

deeply into the body. Additional advantage of this type

of radiation therapy is the possibility of intraoperative

radiation.

In terms of air cleanliness, the linear accelerator rooms

are built on the level with modern operating rooms,

allowing for the transfer of the patient under anesthesia

for irradiation of the tumor bed. Thus, the surrounding

tissues are not damaged and chances of cure increase

signicantly. Such system is used in treatment of breast

cancer, pancreatic cancer, sarcomas of soft tissues, ofthe pelvic organs (rectum, cervix), etc. For instance, in

the case of breast cancer the patient can avoid everyday

treatment for 5-7 weeks: the long course is replaced by

a single session of irradiation during the operation.

Only a few health facilities in Europe have such a

technology and this fact puts LISSOD on the leve

with the most prestige cancer clinics.

The hospital provides brachytherapy involving

high-power sources, namely iridium. The source is

delivered to the tumor by remote control without

a touch of hand, ensuring full radiation safety

for the patient. Brachytherapy together with

computer planning provide high cure rate with

minimal side effects in comparison with outdated

technologies. Uterine and cervical cancer, breast

cancer, prostate cancer, head and neck malignant

tumors as well as soft tissue sarcomas (without the

need for limbs amputation) - brachytherapy treats

these tumors promptly, effectively and without

hospitalization. Specially equipped rooms with

the operating rooms level of air cleanliness allows

to prescribe high-dose chemotherapy when

necessary.

The strategic idea of clinical oncology at LISSOD is

multidisciplinary planning of the medical process

involving surgery, radiotherapy, chemotherapy,

hormonal and immunological treatments. This

approach enables us to closely follow the principle

of organ preservation in the treatment of cancer

without compromising its radicalism.

Over the 5 years of LISSOD functioning

more than 20 000 sessions of radiotherapyhave been carried out.

Operating rooms are Class 10000 and equipped with a sophisticated system of

automatic removal of narcotic gases. The work of top-class specialists and the

use of the latest equipment allow performing general and laparoscopic surgeries

of any complexity at LISSOD. Advanced anesthesia and respiratory devices with

the use of the latest generation screens provide accurate monitoring of patients

organs and systems condition during the operation, which, in turn, enables the

use of minimally sufcient anesthesia, reducing the frequency of complications

both during and after surgery.

The Intensive Care Unit is equipped in accordance

with the leading world standards in this eld.

Mobile equipment makes it possible to apply

minimal anesthesia for complete pain and

discomfort relief during diagnostic procedures,

which are commonly thought to be painfu

and unpleasant (bronchoscopy, gastroscopy,

colonoscopy, prostate examination, gynecological

examinations, etc.).

The frst Modern Breast Center in Ukraine was set

up on the LISSOD basis. High qualied specialists of

different elds - clinical oncologists, oncology and plastic

surgeons, breast physicians, radio- and chemotherapists,

histopathologists and gynecologists - are involved in its

work. The most advanced technologies and standards are

used in the Center. In particular, the Stereotactic Breast

Biopsy Unit by Lorad (USA) allows detecting even minimal

deviations from the norm in the three-dimensional image

in real time. A special Vacuum-Assisted Breast Biopsy

device enables removal of benign breast tumors up to 2

cm without surgical interference. Oncotyping (the latest

test method) helps determining the efciency of different

treatment methods for each patient. The treatment thus

gets more individual - inuence on the organism reduces

while efciency stays the same.

Principles of plastic surgery as well as help of plastic

surgeons themselves are used for preservation and

restoration of breasts in their full attractiveness. The

Centers specialists were the rst in Ukraine to introducethe organ preservation technique of sentinel lymph node

biopsy, which makes it possible to preserve the unaffected

lymph nodes and prevent such complications as chronic

lymphostasis. They were the rst to successfully use the

radio frequency identication of nonpalpable breast

cancer, with which even the smallest tumors are removed

accurately and completely. The radicality of tumor removal

during operations is always controlled.

More than 250 different breast operations have

been performed at the LISSOD Breast Center.

The ultrasound-guided prostate biopsy is held

when pathological changes in prostate are

detected during the nger rectal examination

or in case of increased level of PSA tumor

marker in blood. Special equipment with a

thin needle is used to differentiate benign

from malignant prostatic hyperplasia. This

absolutely painless examination is done

on an outpatient basis with the use of the

latest achievements of minimally invasive

anesthesia.

> Gynecology

The Department of Gynecology performs

tests to detect malignant tumors and pre-

cancerous diseases. The LISSOD principle

of gynecological cancer treatment lies in

combination of the necessary oncological

radicality with the organ preservation

treatment. The clinical staging performed

during the operation allows choosing the

most optimal protocol for post-operative

care.

All painful procedures are performed with

the use of minimal anesthesia.

The Endoscopy Department is equipped with the most advanced equipment and is organized

on the level with the best endoscopy departments of the Western hospitals. What is most

important for patients - all endoscopic observations including invasive ones are performed

with the use of anesthesia, thus eliminating unpleasant feelings. The video endoscopes used

in our hospital have minimum diameter, while video cameras improving the image quality

allow detecting any deviations from the norm. We also use echo-endoscopes which enable

the puncture of different neoplasms. LISSOD patients can undergo endosonography with the

possibility of ne-needle aspiration biopsy (an endoscopic ultrasound observation of organs

in chest and abdomen directly through the stomach or esophagus). This technique is truly

revolutionary and serves as a substitution to an open biopsy.

The computed tomography (CT) is performed in accordance with the world

leading protocols. The department is equipped with Philips multi-image CT

scanner and a MedRad contrast media injector. CT-tests of the whole body

are performed with the use of different techniques depending on medical

necessity. CT-tests are normally done with intravenous and oral contrast unless

there any contraindications. Apart from standard CT-tests CT-urography,

CT-enterography, lung cancer screening are performed. CT-guided biopsies of

different complexity are carried out.

Over the 5 years of LISSOD work more than 4 000 CT-tests have been

performed.

At the Nuclear Medicine Department

with the use of gamma-camera

dynamic planar and three-dimensional

tomography tests are held. These tests

detect additionaly 30 % of pathological

changes comparing to the regular bone

scintiography.

Lymphoscintigraphy, for instance, is

used to detect affected lymph nodes

during the operation in order to avoid

the unnecessary surgical invasion and

minimize post-operative complications.

Modern scintillation gamma-camera

allows getting, storing and processing

images of a separate organ, system or

the whole body in the static, dynamic

and graphical modes.

The Chemotherapy Day Unit provides the most

advanced chemotherapy without hospitalization

In order to ensure the maximum comfort we

use specialized chairs with an option of body

massage. Every patient can watch the favorite

Research in morbid anatomy is the cornerstone of oncology.

Its results show whether the patient has cancer or not, show

the form of disease. Mistakes here may turn fatal if treatment

starts without necessity for it or if nothing is done when

there is urgent need for it. Histological conclusion is the

guidance for the oncologist in choosing treatment tactics,

its components, and chemotherapy agents.

Great attention is attached in LISSOD to the verication

of the oncological diagnosis. The verication is held at

the world leading morbid anatomy laboratories in Israel

and Germany, whereto all the tissues samples received

during operations, biopsies (endoscopic, breast, etc.), and

aspiration punctures, are sent.

The samples of tissues removed in the course of operations

are sent to the LISSOD morbid anatomy laboratory

equipped with modern facilities for express diagnosis for

cancer cells, which are invisible to surgeon. It allows the

surgeon to remove the minimum of healthy tissues, and on

the other hand to reduce the disease relapse and metastases

appearance.

More than 40 000 laboratory research has

been carried out over the 5 years of the LISSOD

existence.

Medical support

Individual rehabilitation plan as welas the treatment plan is devised by

the interdisciplinary Tumor Board.

Endocrinologists, cardiologists,

urologists, gastroenterologists,

physiotherapists, exercise phy-

siologists get involved in the

recovery process depending on

the problems concomitant to the

disease.


A general practitioner (GP) and an exercise therapist work permanently with our patients. Their

help is an important component of the comprehensive treatment.

Depending on the situation the GP provides recommendations as for the maintenance treatment

of cardiovascular, respiratory and gastroenterological diseases. Our patients can get dietary

recommendations for many illnesses such as diabetes mellitus, heart diseases, diseases of the GIT, etc.

All the rehabilitation therapeutic measures planned take into account the oncological treatment.

Problems connected with lack of exercise, excess weight, osteochondrosis, ischemic heart disease may

arise in post-operative patients and those limited in physical activity as a result of the underlying disease

complications. That is when the help of an exercise therapist is required. Therapeutic exercises are

therefore very important in post-operative period.

The exercises can be both individual and in groups. The exercise therapist takes into consideration the

disease features and character, and establishes the intensity. Exercises reduce the recovery time.

Psychological support

We believe that success in cancer treatment is directly related to the emotional state of the patient.What is more, patients relatives are often in need of such support. Restraint and calmness, awareness,

adequate view of the situation and interaction with doctors is very important to them. Our 5-year

experience and that of the leading Western hospitals proves that psychological support multiplies the

treatment results.

The psychological support in LISSOD is free of charge and is an important part of comprehensive

treatment.

Professional psychologist holds private and group counseling.

The psychological support groups work successfully at the clinic and are open for all comers.

Specialists in psychology consult patients relatives on a regular basis. They help them to cope with

anxiety and to build relations in the way to provide support to their loved ones.

Psychological support helps the recovery of patients by lling their lives with warmth, understanding,

sense and condence.

Aside from classic rehabilitation procedures the LISSOD

patients can take a course of Ayurvedic massages.

Ayurveda (knowledge of life in Sanskrit) - is a Hindu system

of health procedures aimed at physical and emotional

recovery of the person. The practice of Ayurveda therapy

is adopted by the World Health Organization and is widely

used in the USA, the EU countries, etc.

Ayurvedic massages promote vigor and relieve stiffness,

improve emotional state, blood circulation and lymphatic

drainage, accelerate toxin elimination (which is especiallyimportant during the course of radio- and chemotherapy).

Having general positive impact on organism the massages

help to endure the cancer treatment easier and to recover

faster.

Prescription for Ayurvedic massages at LISSOD is made

only under control of the clinical oncologist while the

manipulations are performed by high class specialists

from India.This allows accounting for all possiblecontraindications, ensuring maximum effect of treatment

and recovery procedures.
