Am I eligible for coverage?

You qualify if you are an active full-time employee working at least 30 hours a week. You must be working in an eligible group as defined by your employer.

When does my coverage begin?

When does coverage become effective?*

The "Guaranteed Issue Amount" is the most coverage you can get without having to submit Evidence of Insurability (EOI). Coverage up to the Guaranteed Issue Amounts will begin on 01/01/2017. *You must be actively-at-work for your coverage to begin. Other rules may apply. Please review your policy documents for more information.

Do I have to provide proof of good health (EOI) to enroll?

An EOI Form (medical questionnaire) is required to enroll in the Supplemental Life insurance if you are a late applicant during this enrollment period. If you are currently covered, you may be able to increase coverage without EOI.

When will coverage that requires proof of good health (EOI) begin?

Coverage will begin after Aetna reviews and approves your EOI. If EOI is not approved, coverage will be limited to any "Guaranteed Issue Amounts" that apply.

How much coverage does my Employer provide?

Employer Paid - Term Life Your employer pays for a benefit in the amount of: • You: 1.5 times your basic

annual earnings rounded to the next higher $1,000 to a maximum of $1,000,000 or $50,000

Life insurance provides your loved ones with financial protection if you die. It can help pay your final expenses like funeral and burial; everyday living expenses like mortgage or rent, car payments and groceries; future expenses like college tuition payments.

Employer Paid - AD&D Ultra®

Your employer pays for a benefit in the amount of:

AD&D Ultra provides additional financial protection for the unexpected. AD&D Ultra pays a benefit if you are dismembered as the result of an accident, or an additional benefit to your life insurance benefit if you die as the result of an accident. It will also provide benefits for accidental; loss of eyesight, hearing or speech, loss of a limb, loss of limb use due to paralysis, disfiguring third-degree burns, or coma. Your basic coverage may begin to reduce when you reach a certain age.

Can I buy coverage and how much will it cost?

You can buy coverage called Supplemental Life insurance for yourself and your spouse and children. You can purchase Supplemental AD&D Ultra® coverage for yourself and your spouse and children.

What additional features should I know about?

Accelerated Death Benefit Provision

You may be eligible to receive up to 80% of your (combined basic and supplemental) life insurance coverage if diagnosed with a terminal or serious medical condition.

AD&D Ultra® Features A benefit is paid to your surviving spouse/domestic partner or dependent children if you die in an accident.

Seatbelt/airbag benefits: If you or your dependent die from a motor vehicle accident while wearing a seatbelt, a benefit is paid. An added benefit is paid if an airbag inflated. Educational benefit: For your spouse and each eligible dependent child under 23. Childcare Benefit: For each dependent child under 13 to help pay for childcare. Repatriation of Mortal Remains: If you or your dependent die in an accident 200 miles or more from home, a benefit will be paid to transport the body to your hometown funeral home.

Conversion If your coverage ends or is reduced, you can convert your term life policy to a Whole Life Policy.

You may convert your basic and/or supplemental coverage into a W hole Life Policy with rates based on your age at that time by paying premiums directly to Aetna. W hole life insurance is generally more expensive than term insurance so a change in your premium may apply. You will have 31 days to convert your coverage without answering any medical questions.

Portability If you leave your employer, you can take your term life plan with you.

You have an additional option to conversion. You can continue your basic and/or supplemental life insurance as a term policy by paying premiums directly to Aetna. Term insurance is generally less expensive than W hole Life insurance but your rates will increase as you reach higher age bands. You will have 31 days to convert or port your coverage without answering any medical questions.

Learn the steps in choosing who will get your life insurance payment

You have a life insurance plan from Aetna Life Insurance Company. It’s a great step in helping to protect the security of your loved ones. Now you have an important choice to make: choosing a beneficiary.*

A beneficiary is the person or legal entity who receives the life insurance payment if the insured person dies.** An example of a legal entity is a trust fund you may have set up.

If you are ready to choose a beneficiary, here are some points to think about:

1. You can name as many beneficiaries as you want.Specify whether you want each to be one of the following:

• Primary beneficiary

• Contingent beneficiary

The contingent beneficiary receives a payment only if there is no primary beneficiary entitled to payment.

Just remember to provide each person’s full name, social security number and his or her relationship to you.

2. You can designate a trust as a beneficiary.When the insured person dies, the life insurance payment can go to the trust as beneficiary.

3. You can decide how you want the beneficiary payment divided.

Use percentages. Make sure the total adds up to 100 percent. You can also choose to have the payment evenly divided among beneficiaries. Avoid using dollar amounts since your coverage amount may change.

4. There may be a court process if you designate your estate as a beneficiary.

That process, known as probate, may be used to settle an estate. It will:

• Resolve all claims and distributes property under a valid will

• Protect the deceased’s instructions

• Confirm who is the personal representative of the estate

• Protect the interests of family members who may have claims against the estate

• Protect the personal representative against claims and lawsuits

5. There may be a court process if a minor is a beneficiary.If the beneficiary is a minor, a court may need to decide who should be the guardian or conservator of the minor’s property before payment can be made.

6. It’s good to review your beneficiary choices from time to time.

Check to see if the decisions you made still fit your plans, especially after major events like a birth, marriage or divorce.

Just remember this: If you do not name a beneficiary, your benefits will be paid according to the beneficiary provision in your contract. Please see your plan documents for details.

Your Long-Term Disability insurance coverage will begin on your hire date if elected within 31 days of your hire date. Coverage for late applicants will begin when approved by Aetna. You must be actively at work for your coverage to commence.

Proof of good health, known as "Evidence of Insurability (EOI)" is required to enroll for coverage if you enroll after your 31 day initial enrollment period, or to increase your current coverage in the Voluntary Long-Term Disability insurance plan. You will need to submit a medical questionnaire (EOI Form) and be approved by Aetna before your coverage will become effective. A choice of plans that pay a monthly benefit based on a percentage of your Pre-disability Earnings* for a covered disability. You must submit a claim and be approved by Aetna to receive benefits: *Generally, Pre-disability Earnings include your total income before taxes and any deductions for pre-tax contributions. When am I considered to be disabled?

You are considered to be totally disabled after a significant mental or physical change resulting from a disease, or injury, causes you to be unable to perform the substantial and material acts necessary for your own occupation. Also, as a result, your earnings are 80%, or less, than your adjusted Pre-disability Earnings. Your benefit will extend beyond 24 months only if you cannot perform the material duties of any reasonable occupation that you could be expected to perform satisfactorily in light of your age, education, training, experience, station in life, and physical and mental capacity. Also, as a result of your continued disability, your earnings are 60%, or less, than your adjusted Pre-disability Earnings. If your occupation requires a professional license or certification, you will not be considered disabled solely because you lose your license or certification.

Are there any offsets that may reduce Long-Term Disability?

Offsets Your benefits may be reduced if you are receiving income from other sources. Examples include: Employer sources: Government sources: • Any disability or retirement benefit

received under a retirement plan • Disability benefits received from any

statutory disability plan • Payments received from accumulated

sick time or salary continuation program related to your current employer

• Temporary disability benefits received under any state or federal workers' compensation law

• Any governmental retirement system earned as a result of working for your current employer

Are there any exclusions that apply to Long-Term Disability?

Exclusions You will not receive benefits under certain circumstances. Examples include: • Your disability results from an intentional self-inflicted injury; or you became injured while

committing a criminal act or driving under the influence of alcohol/drugs. • You are not under the regular care of a doctor when requesting disability benefits. • You are receiving payment under a salary continuance or retirement plan sponsored by your

employer. Pre-existing Conditions A pre-existing condition is an illness, injury or pregnancy-related conditions for which, during the 3 months before your coverage became effective: • You were diagnosed or treated; or • You received diagnostic or treatment services; or • You took drugs that were prescribed or recommended by a physician. The plan does not pay benefits for a disability that is caused, or contributed to, by a pre- existing condition, until you have been covered under the LTD Plan for 12 consecutive months (six months if you are 65 or older).

Are there any limitations that apply to Long-Term Disability?

Limitations • You can receive benefit payments for Long-Term Disabilities resulting from mental illness,

alcoholism and substance abuse for a total of 24 months per occurrence. This time period may be extended if you are confined to a hospital.

Is there anything else I should know about my plan?

Recurring disabilities If you return to work and become disabled again from the same illness or injury, it may be considered the same disability. You will only have to satisfy one elimination period and may be eligible to begin receiving benefits immediately if: The disability recurs during the elimination period and within 30 consecutive days of work or the disability recurs after the elimination period and within 6 consecutive months of work. Partial disabilities Partial disability benefits allow you to work, earn income and continue receiving benefits so you can receive up to 100% of your income during the first 12 months of your disability. You are considered partially disabled if, due to an injury or illness: • You are unable to perform the main duties of your own occupation • And you are earning 80% or less than your Pre-disability Earnings After the first 12 months, partial disability benefits can continue based on a formula that you will find in your policy documents. Vocational Rehabilitation and Return to Work Our goal is to help you return to gainful employment. Consultants will review each claim to determine if rehabilitation services would be appropriate and effective. We will work with your employer to provide reasonable accommodations to help you return to work. You may even qualify for an increase in your benefits by participating in a rehabilitation program.

What additional features should I know about?

Survivor Benefit If you die during a period when you qualify for disability benefits your eligible survivor will receive a lump sum equal to 6 months of your gross disability benefit.

Conversion If your employment ends, you may be eligible to convert to a disability conversion contract as a

temporary policy until you become insured under another group plan. This conversion plan can provide $4,000 of LTD coverage.
