In some geographic areas, patients can wait up to six months for an appointment. By utilizing virtual technology, Sutter can provide patients with improved access to quality dermatology care in a faster, more convenient, and affordable way. Sutter Patient Family Advisors provided input on the parts of this platform that connect with the patient prior to launching this service to the public. Connecticut hospitals, through Patient and Family Innovation Units across the state have led the way in partners with the very people who can lead us through these challenging times successfully – our patients. Jenna Burke, Randel Horton Patient Family Advisors (PFAs) are instrumental in co-creating tools and resources with Health Services Advisory Group (HSAG) to help increase quality of care and improve patient outcomes. As the Medicare Quality Improvement Organization for Arizona and California, HSAG has the unique opportunity to incorporate PFAs across healthcare settings. This presentation will share the perspective of one HSAG PFA and highlight how PFAs have been successfully engaged in work with hospitals, post-acute providers, and physicians. No One Dies Alone, is a story of compassion. Compassion drove community hospital to partner with Patient Family Advisors to design, implement and participate in a program that honors the dignity and reflects the needs of patients who may not have families or access to loved ones at their end of life; particularly during COVID-19 pandemic restrictions. Being creative and resourceful; tools recruitment processes, training and implementation of a hospital-wide initiative was co-designed.

Just before stay-at-home orders and cancelled travel plans a group of PFCC-passionate people met to strengthen how we measure quality in healthcare. Some were more practiced with empathy; some were more practiced with analysis. All had a desire to merge stories and data to better understand quality. Respect, partnership, inclusivity, mentorship, support, and investment were some of the missing ingredients. Hear reflections from participants on how these priorities have become even more relevant to facing today’s challenges.

Patient Advisory Councils (PACs) are most effective when leader, physician, administrator/clinician, and patient advisor co-chairs all are active participants. This session will focus on how this integrated approach and our proven 5-Step PAC “Pathway to Success” have been instrumental in transforming our Kaiser Permanente PFCC program to become more effective in improving strategic initiatives, and in ensuring that patients advisors, physicians, and other leaders feel their work on the PAC is truly improving care.

The UCLA Resnick Neuropsychiatric Hospital and Department of Psychiatry formed the PFAC in 2014 to facilitate partnerships between patients/families/friends and the administration and staff covering inpatient, partial hospitals and outpatient programs. The presentation will focus on how to successfully engage a Patient and Family Advisory Council, the important influence and role of the group on hospital and outpatient clinic operations, and the key successes of the committee, including the development of a patient education and orientation app.

Patient Family Advisors in Merger and Acquisition

Healthcare-Associated Infections (HAIs) are the most frequent adverse event in healthcare. The environment plays a critical role in transmitting pathogens causing HAIs. The physical environment is also important to hospitalized patients and family members. Patient satisfaction frequently includes measures of cleanliness of the physical environment. Patients provide a unique perspective for targeting environmental surfaces for cleaning. Patient/family engagement in environmental cleaning processes may lead to improved practices for reducing HAI transmission as well as improve patient satisfaction.

In this interactive didactic session using evidence-based information and our own lived experiences, we will describe the importance of health literacy in healthcare and research. We will explore key concepts of health literacy and ask participants to apply these concepts to real-world examples including patient education materials, research consent forms and data collection instruments. Finally, we will provide practice guidance in plain language writing.

During the turbulent times of transition, the PFAs in an urban safety net hospital levered their partnership to generate organization stability. Participating in the required M&A activities became creative during the COVID-19 pandemic restrictions on gatherings and physical distancing.
