Sir,
I bought my Nokia mobile (Model C1-01) in Poorvika Mobiles,
Mylapore branch on 21-07.2011. (Copy of bill and other details
already submitted). I lost my mobile on 19.12.2011 while driving my
vehicle from palavakkam to neelankarai. I kept my mobile in my
pocket and it felt somewhere which I didn't noticed. I noticed it
after I reached home only. I tried that number and it switched off.
Immediately I lodged a complaint into the nearest police station
(Proof already submitted)
Kindly send me the insurance claim at the earliest.

Note: Since my card is a CUG (Corporate card provided by the
company) I can't able to get the SIM blocked letter from airtel or
from the office.
