Dear Mr. Ayala,

Thank you for your timely and helpful response to my request for information.

I have received the 80303 form that the Lennox school district sent to the CTC on March 21, 2012, andhad a few follow-up requests based on the contents of the letter.

This letter is to request access to records in the possession of the Lennox School District for purpose of inspection and copying, pursuant to the California Public Records Act, Government Code Section 6250 etseq. The records I am seeking are all alluded to in the aforementioned 80303 form. They include: * The unusual incident/injury report filed by employees* Written statements by those employees regarding the teacher's conduct* The letter notifying the teacher of the contents of Section 80303* The Notice of Intent to Release from Substitute Service that was sent to the teacher in question* The Notice of Release from Substitute Service that was sent to the teacher in question* The name of the teacher
