• To provide guidance on the standardized process of

entering student information on the Literacy Education

Information System (LEIS) form, the standard data

collection tool for North Carolina Community Colleges’

(NCCCS) College and Career Readiness (CCR) providers.

• To ensure federal, National Reporting System (NRS),

and state required information are collected on the

LEIS forms.

LEIS forms are required to be completed for every

student enrolled in an adult education/literacy

program beginning each program year and at the

start of each Period of Participation (PoP) within the

program year.

Forms are available in electronic and printable format on

the CCR Program Quality & Accountability webpage.

Both versions must be maintained in a secure location.

– Physical forms must be in locked storage.

Student Colleague/ LACES Id are collected on Pages 1-3.

• Course/Section# is only collected on Page 1 (optional).

• SSN, Date of Birth, Last Name and First Name are needed for data

matching.

• SSN is necessary for WIOA employment goals.

• Provider Name – Enter your organization name.

• Program Year – Enter the current PY e.g. 2017-2018. A program year runs

July 1 – June 30.

• Period of Participation (PoP)for Program Year – Check the applicable PoP.

• Separation date for this PoP – Enter Separation Date for the POP. This is

displayed in Colleague or as Left Date in LACES.

A student is automatically separated and a POP ends if

• a student has not attended any classes in 90+ days, and

• no future service is scheduled within the next 120 days after the last

date of attendance.

**The last day of service cannot be determined until at least 90

days have elapsed since the participant last received services;

services do not include self-service, information-only services,

activities, or follow-up services. This also requires that there are no

plans to provide the participant with future services.

• After 90+ days with no instructional hours, even if the student overall status is Active or Enrolled

• After 90+ days with no enrollments or hours, even if student overall status is prospective.

• Until 90+ days after the last hours date in a Period of Participation, even if the student is

no longer enrolled and overall status is Left or On Hold (LACES)

Attendance.

• A student is automatically separated and a POP ends if

• a student has not attended any classes in 90+ days, and

• no future service is scheduled within the next 120 days after the last

• Users are allowed 14 days to enter attendance before separation is

automatically calculated. Attendance records and registration are checked

for future attendance prior to separation.
