About this sectionThis section helps with checking the structural require-ments. The load capacity of the ceiling plays just as much of a role here as the available space or the kind of opera-tions that are performed.

 If the operating room does not meet one of the requirements, it does not mean that the installa-tion is impossible – it simply means that additional clarifications must be made. Most problems can be solved by adapting standard components.

What happens after the clarification?Every operating room is unique, and the responsible sur-geon knows exactly what is important to him or her. There-fore send the completed checklists and data sheets by fax or e-mail to your contact person at Leica Microsystems. He or she will closely examine the state of affairs and discuss with you how to proceed.

Radius of action

This is a recommended standard positioning. In each case, however, the surgeon must check that proper work is ensured in this position. (Taking into consideration the patient's position, the kind of operation, operating room table, operating room lamps and the position of the assistants and nurses. Also particularly critical is the use with MRI systems.)

This is also the range of movement of the arms and weights of the ceiling mount. Ideally, there should not be any secondary devices in this range. A part of the range can be protected from collisions by rotation stops on the ceiling mount. This “dead” range can be freely selected via two stops (minimum 90°).

Work is not allowed without a rotation stop, since otherwise the cables are in danger.

Overview of the Control Unit locationand the cabling of the system:

The Leica OHC system requires installation of the control unit in the wall. For this reason, an opening of exactly 340×730 mm must be created with a mounting hole pattern in accordance with the drawing next to this paragraph. The depth must be at least 300 mm.

The transformer is placed on the base, which requires a height of the CU opening of at least 600 mm .

All cables needed for the system operation, incl. Leica FL400 / Leica FL800, are provided along with the Leica OHC. The cables are 20 m long. If the distance from the CU to the stand is longer than 20 m, Leica Microsystems must be notifi ed (see contact information on page 6).

The standard cables are long enough if the stand control unit and the documentation system (Leica MDRS, HDMD) are located at the same place. Some cables are too short for other placements. In this case, contact Leica Microsystems (see page 6).

About this sectionThe Leica ceiling mount OHC serves one purpose only: It should make work as easy and comfortable as possible for the surgeon. Every doctor has his or her way of operating, with a unique style and an arrangement that is best suited to him or her.

For these reasons, it is of great importance that the doc-tor's needs and desires be taken into consideration during the planning. Where is the illumination located? The anes-thesiologist? The assistant? Even the difference between left and right-handedness can be significant.

Unlike most other fixtures, the position of the Leica OHC cannot be changed again. The better the setup is planned, the more pleasant it will be to work with the ceiling mount.

General questions

Question Explanation

What kind of operations is the Leica OHC4 used for? The response helps determine the optimum positioning. In any case, it must be established that the operating fi eld lies within the range of the Leica OHC and that the operating room illumination is not cast in a shadow.

Is the operating room table moveable?■ Yes ■ No

A moveable table grants more freedom for different procedures and is very advantageous in any case.

Is the operating room table fi xed on a base?■ Yes ■ No

A base limits the table's movability or even prevents movement entirely. This requires exact consideration of all possible procedures. In addi-tion, it must be judged whether the combination of the Leica OHC and the position of the operating room lamps enable these procedures.

In what range is the operating room table moveable? From which direction will the surgeon operate?

The operating room lamps cannot be oriented to-wards the operation area from the side of the Leica OHC, since otherwise collisions with the Leica OHC may result. Therefore lateral procedures on the skull with operating room illumination are not pos-sible on the side of the Leica OHC.

Vibrations in general

Vibrations of the microscope itself are absorbed by the damping material between ceiling mount and horizontal arm. Vibrations of the ceiling are reduced, because the ceiling mount does have a patented damping system for vibrations in the range of 10 – 60 Hz. The microscope allows a verti-cal amplitude of less than 0.4 mm (peak to peak value) for all used working distances and the whole zoom range with eyepiece 10×.

Ϙ Extrapolated to a vibration of the ceiling, the amplitude of the ceiling related to the floor must be smaller than ±1 mm.

Ϙ Short wavelengths are worst, longer wavelengths of a few seconds duration will reduce the perceptible effect.

Ϙ For better efficiency and reduced costs we recom-mend to absorb vibrations directly at their source (compressors, elevator machines, etc.).

Vibration absorber

The vibration absorber is used for maximum stabi-lization of the microscope. Reducing the vibrations ensures a stable, clear image.

Only vertical vibrations are absorbed, not lateral vibrations.

If unobstructed positioning of the stand cannot be ensured due to exist-ing fixtures, other clarifications must be made first.

Does the operating room in question have a concrete or steel con-struction?■Concrete ■ Steel

With a concrete construction, a standard solution is possible in most cases. A steel construction requires a custom solution (see the sketch “Basics of calculation for the ceiling plate construction”).

Basics of calculation for the ceiling plate construction

With steel constructions, it is particularly important that sufficient rigidity is achieved in order to prevent vibrations and residual shaking. The solution proposal above has been tried in practice and provides for a reliable, vibration-free mount.
