2019 Quad Cities ATHENA Leadership AWARD Nomination Instructions Each year, Women Lead Change Quad Cities recognizes the accomplishments and contributions of women in the community and as in years past, IWLC is giving organizations and individuals the opportunity to honor inspiring women in their lives. Nominations are now being accepted for the 2019 ATHENA Leadership Award. The 2019 ATHENA Leadership Award Finalist (Honorees) and Recipient will be honored at a luncheon awards program on Thursday, April 4, 2019. The ATHENA Leadership Award is presented to a woman who is honored for excellence, community service and for actively assisting women in their attainment of professional leadership excellence and leadership skills. Do you know a woman in the Quad Cities that consistently supports, develops and honors women leaders ? Does she inspire other women to achieve their full potential ? Does she strive to create balance in leadership ? It is our desire to recognize the outstanding female leaders in this region and honor those who encourage others to reach for the stars. CRITERIA ATHENA Leadership Award Nominees must meet each of the following criteria: o Demonstrate excellence, creativity and initiative in their business or profession o Provide valuable service to improve the quality of life for others in their community o Actively assist women in achieving their full leadership potential QUALIFICATIONS o Represent either the profit or not-for-profit sector and must work and/or live in the Quad Cities o Previous ATHENA Leadership Award Recipients are not eligible for nomination; however, past nominees may be nominated again o There is no minimum or maximum age qualification that must be met COMPLETING THE NOMINATION FORM o You are encouraged to work closely with your nominee to complete the nomination form, unless you are not notifying your nominee of their nomination. If that is the case, you are encouraged to work closely with individuals who know your nominee well and can provide sufficient information to complete the nomination form

Each year, Women Lead Change Quad Cities recognizes the accomplishments and contributions of women in the community and as in years past, IWLC is giving organizations and individuals the opportunity to honor inspiring women in their lives. o Nominations for the ATHENA Leadership Award must be submitted using this nomination form and formato You may include THREE (3) additional pages of supporting documents (articles, testimonials, etc.) with your

completed nomination formo Deadline for application submissions is: Feb. 15, 2019 by 12:00pm

THE PROCESSA Selection Committee, made up of previous years ATHENA Leadership Award Honorees will review all nominations and select five finalists, “Honorees.” The five finalists are then tasked with writing an essay with a specific topic. A second Selection Committee, made up of the previous years’ ATHENA Leadership Award Recipient, together with a diverse group of community leaders, will review the finalists’ nominations, together with their completed essay and select the ATHENA Leadership Award Recipient.
