LEAD GUITARS in STEM Project Participation Agreement

I understand that as part of the institute commitment to support the learning activities in the National Science Foundation Advanced Technological Education (NSF ATE) #1304405 “LEAD GUITARS in STEM Project,” I, _______________________________________ (print name), am committing to the following. Implementation Intention Please initial the following:

_____ I have discussed the “LEAD GUITARS in STEM Project” with my administrator. _____ I plan to implement the STEM Guitar program, or parts of the institute curriculum, in my program.

_____ I understand that I am required to participate in a pre-institute webinar and follow-up activities [To include any of the following: attend follow up webinar, develop an activity for use in your classroom, including a quiz; follow up assessments; present to other teachers; formal presentations; STEM expos; newsletter; media coverage; survey; interviews; targeted evaluations; mentor; learning community; and assessment tool usage].

Implementation Commitment

I understand that in order to receive [financial compensation, stipends, guitar equipment credit, coupons, vouchers, etc.], I agree to implement a minimum of two STEM activities from the LEAD GUITARS workbook and associated online assessments, or assist a teacher at my school, to integrate two activities in their program.

Implementation Form

Following the institute, I understand that in order to receive [compensation], it is my responsibility to provide and complete the following: • An implementation form that lists at least two STEM activities from the LEAD GUITARS workbook; • Provide associated online student assessments from those activities; • Participate in follow-up assessments • Administrator and instructor signatures acknowledging implementation which generates the final the online

instructor/student surveys (required to receive final compensation).
