LCP member transportation services

Welcome!

On behalf of Anthem Blue Cross and Blue Shield, thank you for your service to members enrolled in

Hoosier Healthwise, Healthy Indiana Plan (HIP) and Hoosier Care Connect.

General information

What is available and how to schedule member transportation services • LCP Transportation provides transportation for members enrolled in

Hoosier Healthwise, HIP and Hoosier Care Connect • All nonemergency transportation services must be scheduled between

8 a.m. and 8 p.m., Monday through Friday • Trips may be scheduled up to 45 days in advance • To schedule transportation, including transportation for after-hours or urgent

care, members can call 1-800-508-7230, select a language, select Anthem Medicaid and select option 1; for return trips, select option 2

• When calling for pick up times, please have all the information necessary to schedule the trip; when calling for return time frames, please give the member’s name who had the trip instead of the escort

Guidelines for members using wheelchairs and long-distance trips

• All trips, both nonemergency ambulatory and nonemergency nonambulatory (wheelchair) trips must be scheduled by 8 p.m. at least two days in advance.

• LCP recommends scheduling with two weeks’ advance notice due to availability in certain geographic areas.

• Trips more than 50 miles must be scheduled by 8 p.m. two business days in advance.

• Pick-up times for long-distance trips may be changed the day before to coordinate multiple long-distance trips. LCP notifies member before combining long distance trips.

Urgent care transportation

• Urgent care trips can be scheduled the same day if the primary medical provider (PMP) confirms that the trip is related to urgent care.

• Urgent care trips have a two-hour window for pick up from the PMP’s confirmation time.

• Urgent care long distance trips have a four-hour window for pick up from the PMP’s confirmation time.

LCP policies for transporting children and minors

• Children under the age of 18 must have a parent or guardian escort them or provide proof of legal emancipation.

• Car seats are required by law for children under the age of 5.

• LCP provides booster seats for children 4 years of age who weigh at least 40 pounds and up to 8 years of age who weigh up to 80 pounds.

• Prior approval is required for elective procedures such as:

— Bariatric procedures

— Out-of-state trips

Member eligibility verification

• LCP only provides transportation to Anthem members for Medicaid-covered or Anthem-authorized services.

• Anthem members referred by their PMP to another facility or health care professional must provide the facility name, address and telephone number when scheduling transportation services with LCP.

Anthem – LCP covered services

• 24-hour live agent dispatchers for urgent care requests

• Transportation for appointments: — Health care and health education — Vision and dental — Redetermination — Women, infants and children — Interpreter services are offered for any non-English speaking

members — All Anthem members: trips to the pharmacy after doctor

appointment (third-leg)

Indiana public transportation

In these cities, LCP offers members the option to utilize Indiana’s public transportation in lieu of curb-to-curb service. This is an option only offered in cities which have reliable public transportation.

Vendor evaluation program

• The vendor evaluation program is based on a 10-point scale

• Determines vendor priority level for trips by county

• Ensures each vendor uses the same criteria and has the ability to manage an increase in demand for services

• Vendors are evaluated on a number of important criteria:

— Pricing

— Dependability

— Customer service

Vendor dispatch and cancellation procedures

• LCP requires no-shows and cancellations to be communicated immediately to LCP dispatch.

• Faxed/uploaded trips to vendors are confirmed as received via telephone by vendor staff.

• Vendors who are unable to perform a previously agreed-upon trip should always communicate this change directly to LCP. This unexpected change should not be communicated to the member.
