Implementation of LOTO procedures Provide and Maintain Facility / Installation Plot Plans that indicate hazardous locations and the type of

permits that are being used to control tasks Scope of work received from execution team. Preparation of permit to work (PTW) as per Scope of work of execution team. Preparation of its relevant documentation and attachment i.e. MS, JSA and others as required. To attend the risk assessment (RA) meeting for critical jobs. Ensure the appropriate Task Based Risk Assessment process is in place and being implemented for PTW

activities Ensuring that required Critical Lifting Plan for rigging jobs in or around QP Plant Facilities have been

reviewed and approved by STI Department. Reviews job Method Statements, Drawings (PFD’s & P&ID) and Job Hazard Analysis. . Submission of Permit to Work to PTW office for concurrence and approval. Attend the Permit to work meeting for explanation of work description Receive permit to work from Permit to Work office after concurrence and approval. Submission of Permit to Work to Operational Head of that Area for final approval. To make sure all requirements are fulfilled as per PTW.
