Program Outcomes:Pages 19 – 24 of the Consolidated Application
Outcome Requirements by Funding SourceCDBG-Public Services
Develop 1-3 outcomes that best capture the impact of your program
Outcomes should relate to the consolidated plan goal that you are applying under
ESG
Use ESG performance measures
Do NOT add or change these performance measures
Outcome Definition
An outcome is a problem-related, attainable, and measurable statement of a program’s intended effect on its client’s knowledge, skills, attitudes, behavior, or condition.ORThe measurable effect on someone as a result of what they did in your program.
A number & percentage of clients will accomplish Y for/in Z amount of timeFor example: 75% (10) of participants will report a 40% increase in how often they read to their children
Traits of good outcomes Focused on what the client will gain
Measurable and define how it will be measured
Specific tells who, how and when
Attainable and can be attributed to that program
Understandable to someone outside of the program
Writing Outcomes in the ApplicationComponent Underlying Logic
Outcome Why does this program exist? What change are we hoping to see happen?
Strategies What steps will you need to take to achieve your desired outcome?
Measurements How do I know a client has succeeded/ the outcome was met?
Target Dates What timeframe to we anticipate seeing results?
Rationale Why is this outcome appropriate for participants? How did you set the target? Do you have a benchmark?
Long-Term Effects
How will achieving your outcome(s) benefit the community? The client? This may extend beyond the terms of the grant.
ESG OutcomesESG Performance Measure Location of data
The % of persons who exited to permanent housing (subsidized or unsubsidized) during the operating year.
Bowman’s ServicePoint report 625, question 36.
The % of persons age 18 or older who increased their total income, (from all sources) as of the end of the operating year or program exit.
Bowman’s ServicePoint report 625, question 36.
HP and RRH only - The % of households who remain in permanent housing six months after program exit.
Bowman’s ServicePoint report 625, question 36.
Strategies Can list outputs such as “conduct 15 classes
with 150 total participants.” Measurable is better. Evidence-based is better.
Measurement Methodology Self-reported by clients– surveys, interview Evaluation tools (i.e. SSOM, SPDAT) Report of changed status such as housing,
income
Rationale for Outcome and Target How did you determine the measurement
and/or target numbers? A 2% increase over last year?
