1.0 PURPOSE

1.1 To establish the requirements for Post Maintenance Re-qualification Testing within the Maintenance Department.

2.0 SCOPE

2.1 Applies to all Maintenance Groups and Sections specifying Post Maintenance Re-qualification Testing.

3.0 DEFINITIONS AND ABBREVIATIONS

3.1 Definitions

3.1.1 Corrective Maintenance – Actions that restore, by repair overhaul or replacement the capability of a failed SSC to function within acceptable criteria.

3.1.2 Deficient – Any condition that deviates from the design of a system or component of equipment and results in a degraded ability to accomplish its intended function.

3.1.3 Periodic Test – A functional test of installed equipment and/or systems to satisfy technical specification operability requirements.

3.1.4 Post-Maintenance Requalification Test – Any appropriate testing performed following maintenance to verify that a particular piece of equipment or system performs its intended function based on its design criteria and that the original deficiency has been corrected, or that preventive work has not had a degrading effect on the piece of equipment or system.

3.1.5 Preventive Maintenance – Actions that detect, preclude or mitigate degradation of a functional SSC to sustain or extend its useful life by controlling degradation and failures to an acceptable level.

3.1.6 Repair – The process of restoring an item by welding, brazing, metal removal or other means, such that existing design requirements are met.

3.1.7 Replacement – Process of substituting a component or part there of with a spare refurbished or modified item.

3.1.8 Work Package – A collection of maintenance documents used to control and record work in the plant from the time of identification until the satisfactory completion of the corrective and/or Preventative maintenance action and return to service.

5.0 REQUIREMENTS

5.1 General

5.1.1 The scope of Post Maintenance Requalification Testing shall be based on the extent of preventive and/or corrective maintenance performed.

5.1.2 As a minimum all CSR, SR and AR plant equipment shall be requalified and tested following maintenance.

5.1.3 Post Maintenance Requalification Testing requirements shall be determined and the test performed for all corrective and preventive maintenance activities and troubleshooting activities that might have impaired proper functioning of the component.

5.1.4 Where possible tests shall be conducted under conditions that represent normal operating parameters, such as flow, differential pressure, level, temperature, input signal values and fluid type. Where this is not possible testing shall be performed either in accordance with acceptable industry practices, OEM instructions or instruction from the applicable engineering group. Examples of types of maintenance activities requiring post-maintenance testing:

• Maintenance that affects the integrity or operation of a fluid or gas system, or components within those systems.

• Maintenance that affects the wall thickness of pressure boundaries or affects mechanical strength of components or fittings.

• Maintenance that affects electrical distribution equipment.

• Maintenance that affects electrical control circuitry or electrical components.

• Maintenance that affects instrument detectors or components in an instrument loop.

• Maintenance that affects the engineered function of a system or component.

5.1.5 In some cases, tests may be conducted under conditions that represent abnormal or emergency operating conditions. For example, testing thermal overload relays on switchboards thus simulating a motor overload condition.

NOTE: All non-routine testing must be done in accordance with KAA-647.

5.2 Determining, Specifying and Reviewing the Post Maintenance Requalification Test to be Conducted

5.2.1 Determining the tests to be conducted.

5.2.1.1 During preparation of a maintenance work package the scope of testing required is determined by the requestor using sources such as testing requirements contained in existing authorised procedures, Equipment History, Vendor Manuals and consultations with Operating, Monitoring, Inspection or Testing Personnel.

5.2.1.2 As a minimum, the tests recommended for the maintenance activities listed in Appendix 1 and Appendix 2 (welding) of this Standard must be considered.

NOTE: The actual required testing must be tailored to the specific maintenance performed. Once tests are specified, they will be considered mandatory.

5.2.2 Specifying the Tests to be Conducted

5.2.2.1 The required Post Maintenance Requalification Testing must be specified in accordance with KAA-648.

5.2.3.1 Before work is authorised to begin, a final review of the Requalification Testing identified must be conducted to determine if the specified testing is adequate for the intended work, i.e. the tests verify that the deficiency has been corrected.

NOTE 1: As required by KAA-648, all permit to work requests are reviewed to verify the adequacy of the specified post maintenance testing and identify additional testing as required.

NOTE 2: The primary purpose of this operating review is to ensure that testing restores technical specification operability and imposes no adverse effects on the system or equipment.

5.2.3.2 If the scope of work or testing expands beyond the original scope in the work package, work must be stopped and new or additional instructions must be written and authorised.

5.2.4 Troubleshooting Maintenance Work:

5.2.4.1 For troubleshooting maintenance work, the test requirements normally cannot be determined until the troubleshooting is complete. Thus, a detailed log must be kept of work performed during troubleshooting to ensure that post maintenance testing covers the troubleshooting scope. (Document in troubleshooting plan).

5.2.4.2 The maintenance work package with scope changes must be resubmitted for review once the problem has been determined. Post maintenance/Requal testing requirements must be included in the review.

5.3 Requalification Testing Procedures or Work Instructions

5.3.1 Dynamic Requalification Tests shall be conducted in accordance with written authorised procedures to verify that maintenance was performed properly, the initial deficiency was corrected by the maintenance activity and that the equipment/system/component will continue to perform as designed.

5.3.2 The procedures shall measure performance and allow for documentation and review of test data for the equipment and/or system. The data may become baseline data depending on the extent of the repair.

5.3.3 The Post Maintenance Testing procedure must be written in such a manner as to verify the satisfactory completion of the maintenance and verify system/equipment operability.

5.3.4 Test procedures must include details (as appropriate) such as initial conditions and prerequisites, hold points, precautions, personnel safety requirements, clear acceptance criteria and post-test restoration.

5.3.5 Instructions in the test procedure shall be as specific as possible and will avoid using vague criteria such as “verify proper operation” or “check for excessive temperature”.

5.3.6 Test equipment will be specified and provision made for recording the equipment identification.

5.4 Use of Existing Requalification Testing Procedures for Post Maintenance Requalification Testing

5.4.1 If an applicable periodic test procedure exists or if testing requirements are contained in an authorised maintenance working procedure, then that procedure may be used to verify operability of the system or equipment.

5.4.2 If an existing Requalification Testing Procedure is used, the existing procedure must meet the following requirements:

• The existing procedure must include details such as initial conditions and prerequisites, hold points, precautions, personnel qualification requirements, personnel safety requirements, clear acceptance criteria and post-test restoration.

• The existing procedure must prove system operability by testing components and features that were either directly or potentially affected by the subject maintenance activity.

5.4.3 Using Portions of an Existing Requalification Testing Procedures:

5.4.3.1 If applicable sections of existing procedures are to be used, these applicable sections must be specified in the appropriate section of a separate “controlling” Requalification Testing Procedure which must be written and authorised.

5.4.3.2 This is necessary to ensure that the system status, line ups, precautions and prerequisites apply to the testing of the subject maintenance activity at the current point in time.

5.5 Control of Testing

5.5.1 Control of Post Maintenance Requalification Testing and the control of the resulting test documentation will be part of the Work Package.

5.5.2 If more than one group is involved in requalification, Requal Testing will be co-ordinated by the requestor of the work.

5.5.3 If the test cannot be completed immediately after maintenance is performed, the Work Order must be held as an open maintenance Work Order until such time as the component can be tested; or a new notification/work request must be generated and cross referenced to the main work order.

5.5.4 A satisfactory test is one that verifies the ability of a particular component or system to perform its intended function, the original deficiency has been corrected and no new or related problems have been created by the maintenance activity.

5.5.5 If the test is unsatisfactory, deficiencies identified during testing will be documented and corrected by generating a new notification/work request. The root causes of unsatisfactory tests must be analysed so further rework is minimised. Any identified rework must be tracked in accordance with KGA-081.

5.5.6 If rework is performed, test requirements for the rework must be evaluated to determine if testing must be repeated due to the rework.

5.5.7 All testing will be conducted within the requirements of the Plant Safety regulations.

5.5.8 All systems testing will be co-ordinated.

5.5.9 System operability will be assured by system testing.

5.5.10 Maintenance Components listed in SAP as ISIPRM shall be subjected to statutory requalification by Inspection and Test as required by KSA-021.

5.5.11 For further clarification on IST testing requirements, refer to KWR-TP-IST-001.

REPAIR/REPLACEMENT OF ELECTRIC MOTOR

(1) Perform insulation resistance checks, winding resistance, polarisation index.

(2) Verify proper direction of rotation and proper phase relationships.

(3) Operate the equipment and verify there are no abnormal noises.

(4) Obtain baseline vibration analysis data.

(5) Measure bearing temperatures.

(6) Measure starting and running current for each phase.

(7) Check oil level.

(8) Check air filter cleanliness.

(9) Measure winding temperatures.

NOTE: Operation/notification must be raised for review of the thermal overload settings (should the motor manufacturer or model number change).

REPAIR/REPLACEMENT OF CIRCUIT BREAKER

(1) Verify adjustment of circuit breaker trips.

(2) Measure phase-to-phase and phase-to-ground insulation resistances.

(3) Measure resistance across each main contact.

(4) Perform auto functional test on the breaker (open and close on required signals).

(5) Measure breaker response time.

(6) Verify operation of auxiliary trip devices and relays.

(7) Perform manual operation.

(8) Breaker parameter check (e.g. breaker operating voltage, current, control power, status lights).
