    1.0 INDUSTRIAL ATTACHMENT PROGRAM REQUIREMENTS

    1.1Brief description

    Industrial Attachment (INFO 4996) is a pre-condition for graduation of BITprogram. Effectively beginning from Semester 3, 2011/12, ONLY FINAL

    YEAR students in final semester are allowed to undergo for the Industrial

    Attachment Program.

    Students are NOT allowed to take any other course(s) during the period of

    Industrial Attachment Program. This is to ensure their full commitment

    towards completing the task(s) that has/ have been assigned to them. The

    Industrial Attachment Program is available all semesters for each academic

    year.

    Generally, students are encouraged to find their own placements; in case ofdifficulty, the Department will assist them in getting the place. Either case,

    students must make sure that they are assigned to IT projects, or at least

    attached to an IT department or unit. During the training, each student will be

    assigned a supervisor from the Kulliyyah i.e. a DIS lecturer as a liason person

    between the department and the organization. The Kulliyyahs Supervisor

    will visit the student, and at the end of the period, he/she will assess the

    student. In addition, assessment will also incorporate the companys feedback

    (by Companys Supervisor) and the students report.

    1.2Project Objectives

    The purpose of this training is to provide students with an exposure to real

    work environment and how projects (especially IT projects) are implemented

    in organizations.

    1.3Project Scope and Duration

    The duration o f the Industrial Attachment must be within 24 weeks for INFO

    4996. The scope of the project must be for 6 months worth of work and

    relevant to IT. The project must involve either working in existing IT

    system/work or developing system/multimedia project.

    1.4Report

    Students are required to submit a report (Industrial Attachment Report) at the

    end of the Industrial Attachment session. The purpose of this report is to

    provide information on the type of task assigned to them, what is/are the

    learning outcome(s), obstacle(s), lesson(s) learnt etc during the period of

    Practical Training in the companies.

    1.4.1 Industrial Attachment Report

    Every student must prepare a written report about the Practical

    Training Program that he/ she has undergone. This is to ensure that

    the students write about the task given to him/her during the

    program.

    Industrial Attachment Report must be attached together and ring

    bounded. The Log Book (Refer 1.4.2) must be attached in the

    Industrial Attachment Report in the Appendix Section.

    Submission MUSTbe made to the Kulliyahs supervisor after two

    weeks upon completion of Industrial Attachment Program. If the

    student fails to submit the report on time, a reminder will be sent to

    the student. The student will be penalized if he/she fails to submit

    Industrial Attachment report within a month from completion date.

    Final grade for Industrial Attachment will be accumulatedexcluding report assessment.

    1.4.2 Log Book

    Every student is required to keep a Log Book of work where

    he/she will account all the jobs undertaken.

    Students must document their activities in a Weekly Log. The

    Weekly Log must be certified by the Companys supervisor, and

    contains the following sections:

    Week Number

    Description of activities (Include problems encountered and

    solutions)

    Name of supervisor

    Remarks

    The certified copies of the Weekly Logs are to be kept by the

    students and included in the Appendix of the Industrial Attachment

    Report.

    1.4.3 Plagiarism and Cheating

    Suspect Plagiarism

    If a student is caught or suspiciously suspected of plagiarism, a

    detail and thorough investigation will be conducted by the

    department.

    1.5Assessment Forms

    There are TWO(2) assessment forms given to each student. Those forms are

    Companys Supervisor Assessment Formand Student Assessment Form.

    1.5.1 Companys Supervisor Assessment Form.

    Students are required to give his/ her Companys Supervisor this

    form on the presentation/visit day by the Kulliyyahs supervisor.

    Every student must observe the confidentiality of the supervisors

    student assessment form. The form must be returned to the

    Kulliyahs supervisor during the final visit.

    1.5.2 Student Assessment Form.

    Students are required to fill in the form by the end of the Practical

    Training Program and return it to the Kulliyahs Supervisorwithin

    the first week of the new semester.

    2.0PRE-REQUISITES AND PROCEDURES

    2.1Procedures to Participate in the Industrial Attachment Program

    2.1.1 Application

    Every student should fill up a form known as Application Form, in

    his/her first week of an active semester prior to starting of the IA

    program in the next semester.

    Students will have to do their Industrial Attachment in companies

    within Malaysia only. For outside Malaysia, student needs to obtain

    approval from the department and pass the verification on the

    company.

    Students also will have the options of either to seek companies ontheir own, or by selecting the company from the Departments

    selection. Either option does not guarantee a placement for the

    student. It totally depends on the company. (Selection is usually based

    on these criteria; project availability, students academic excellence

    and curriculum activities involvement).

    Student should submit this form within the second week of the

    semester to the Industrial Attachment Program Assistant.

    Once they have submitted the Application Form, it is their SOLE

    responsibility to ensure of their placement. Students will have to playan active role in contacting the companies and getting feedbacks. The

    confirmation of placement must be informed to the Department not

    later than 4 weeks before the actual program begins.

    Students will be penalized with the amount of RM50 for any late

    confirmation. Failing to secure a placement without concrete reason(s)

    will cause student to postpone the program and again, this will affect

    the study duration.

    The Kulliyyah will not be responsible in securing or replacing

    placement for students (no backup from Kulliyyah).

    2.1.2 Registration

    A departmental committee will evaluate the entire students

    application.

    The departmentwill proceed with the registration for the subject

    once the application has been approved, (by the Department and the

    company).

    2.1.3 Accepting Offer of Placement

    Before accepting the offer of undergoing the Industrial Attachment

    Program, every students will be given an opportunity on whether to

    accept the selected company or reject it.

    The department will take the responsibility to communicate with the

    company after receiving letter of acceptance. Students are given one

    week to accept/reject the offer.

    If a student decides on accepting the offer, he or she must sign a

    Confirmation Attachment Form as an agreement between the student

    and the Kulliyyah within one month. Once signed, the student must

    perform duty as mention on the said agreement.

    Any changes of placement will not be entertained once the agreement

    has been signed. But, in any emergency or unavoidable case, studentwill need to start the Industrial Attachment Program again for the new

    placement. Only ONEchange is allowed. Changes of placement might

    also take place in an event where the department is not satisfied with

    the current workplace upon first visit by the Kulliyyahssupervisor.

    2.1.4 Reporting for duty

    Students are required to fax or e-mail (scan) their Report Duty Form to

    the Industrial Attachment Coordinator before the end of their first

    week in the company.

    2.1.5 Commitment to Program

    Every student must perform his/her training with full dedication and

    concentration and must complete the whole program according to the

    agreement stated between the company and Kulliyyah. Once report

    duty, every student must commit to completing the program towards

    the end. If a student fails to do so, he or she will be penalized with the

    amount of RM100 and will be given the grade FAIL, and he/ she will

    have to repeat the program.

    2.1.6 Insurance Coverage

    Every student will be covered automatically with the University

    insurance coverage. For further detail, refer STADD.

    2.1.7 Preparation of Visit Program

    Kulliyyah supervisor will visit the students and company supervisors at

    the beginning and at the end of the program.

    The Kulliyyahs supervisor should meet company supervisor during

    the first visit and discuss about students project scope for the entire 6

    months.

    Student will have to set the date of the visit him/ herself by callingKulliyahs supervisor and Companys supervisor availability.

    2.1.8 Presentation

    Students are required to do a short presentation In doing so, they are

    required to prepare a presentation material (Power Point Slides etc).

    Student must present to both companys and Kulliyyah supervisor

    on the task given.

    The 15-20 minutes presentation will include the companysbackground, unit and job allocation, and explanation about the task

    given in detail.

    Presentation Marking Criteria

    2.1.9 Submission

    Students are required to submit the documents below to the department

    within two weeks after the completion of IA:

    1) The Industrial Attachment report2) Log Book is attached in the appendix of the Industrial Attachment

    report

    3) The assessment forms (Companys Supervisor Assessment Form)4) Student Assessment Form

    2.1.10 Assessment

    Students are graded based on the Universitys grading schemeand

    the assessment is comprised of:

    Assessment Method Mark (%)

    i. Company Evaluation 35%

    ii. Industrial Attachment Report 25%

    iii. Oral Presentation & Supervisor Visit 30%

    (Students presentation, Q&A,interview session)

    iv. Monthly Report 10%

    Total 100%

    Based on the above assessment method, students will be awarded a

    grade (A, A-, etc.)

    Kulliyyahs supervisor is given a month to submit the grades prior to

    submission of all requi red documents by the students.

    2.1.11 Important Note:

    In order for the report to be considered complete, the following criteria

    MUSTbe met:

    Submission of ring bound/hardcopy and soft copy of the report before

    the due date as specified in 2.2.9. For softcopy, please include CD

    cover that contains similar information as stated at the front cover of

    the ring bound report.

    Student must observe the procedures and policies and to ensure all the

    penalties have been settled or result can be hold until everything hasbeen settled.

    3.0Industrial Attachment Program Policies

    3.1.1 Every student will be assigned a supervisor i.e. a lecturer in the

    Department of Information System to supervise the training.

    These supervisors from the Kulliyyah will be appointed on rotation

    basis. This is to give chance to all lecturers to have the experiencein supervising students in the Industrial Attachment Program.

    3.1.2 Every student must obey and respect his/her Industrial Attachment

    supervisor throughout the program. He/she must also abide by the

    rules and regulations of the agency/company providing the training.

    3.1.3 Every student must make himself/ herself available throughout theworking hours as specified by the agency/company concerned. If the

    student failed to do so, the student will fail the program. Additionally,the Industrial Attachment student is entitled to take a day leave on

    monthly basis during the 6 months period.

    3.1.4 Every student is not allowed to reveal any classified informationpertaining to the agency/company where he/she is attached.

    3.1.5 Every student must behave and wear proper attire during the IndustrialAttachment period, in line with the Islamic religious requirements. As

    for the non-Muslim students, the Islamic codes and conduct must be

    respected.

    3.1.6 Every student must avoid causing damage on any property belongingto the agency/company concerned.

    3.1.7 Every student should bear all cost pertaining to the IndustrialAttachment; the agency/company of his/her attachment may, if it so

    wishes, pays an honorarium.

    3.1.8 Any violations on any of the sections from 3.3.1 to 3.3.7, the case willbe forwarded to the board of the department.

    4.0GENERAL PRESENTATION GUIDELINES FOR THE REPORTS

    4.1 Language

    The language of the project paper is English.

    4.2 Paper

    Size - A4 (21.0cm X 29.7cm)

    Quality - Acid-free paper of at least 80gm weight

    Color - White

    4.3 Font Size and Type

    Students MUSTuseArial. Other fonts areNOT ACCEPTABLE.

    Font Size:

    For Headings and subheadings, use 12-point font.

    For Table of Contents, use 11-point font.

    For Text , use 11-point font.

    For Tables and Figures, use 11-point.

    For Footnotes, use 9-point font.

    This format is just a guideline. The actual finalized report should be made as being

    advised by the supervisors except for the Preliminary pages where all the items

    must be in sequence and format as stated above must be followed.

    When student are writing a piece of work it is essential that student provide detailed

    and precise information on all the sources student have consulted. Always remember

    to record the details about an item as student use it. The use of public material without

    acknowledgement is plagiarism for which the penalty will be failure of the

    dissertation.

    5.1Setting out References

    There are two methods by which references can be displayed, the British Standard

    and Harvard. Once a method has been selected it is important to be consistent in
