• More than 2 million heart attacks and strokes each year• Cause 1 of every 3 deaths (800,000 deaths a year)• Leading cause of preventable death in people <65 • $444B in health care costs and lost productivity• Greatest contributor to racial disparities in life expectancy• 15,773 Georgians died of heart disease and 3564 died of
stroke (2011)• 37 percent of blacks and 33% of whites in GA have
hypertension.

Introduction: What is Million Hearts®?
• A national initiative to prevent 1 million heart attacks and strokes by 2017.
• Brings together communities, health systems, nonprofit organizations, federal agencies, and private-sector partners from across the country to fight heart disease and stroke.

Introduction: What is 100 Congregations for Million Hearts®?
Launched February 21, 2013, to encourage faith-based organizations to increase awareness and action around heart disease prevention with a focus on high blood pressure (hypertension).
Million Hearts® Congregations
Can we recruit 5 Georgia-based congregations to join 100 Congregations for Million Hearts® by December 2015?
Research Question
Support faith-based organizations in raising awareness of, and preventing, heart attacks and stroke with a focus on hypertension control
Project Goals
o Designate advocate to be a resource for heart health information
o Establish/strengthen relationships with local experts and resources (community pharmacists, health centers, and health workers.)
o Disseminate messages about hypertension control;
o Promote Heart Health Mobile app
o Distribute blood pressure tracking wallet cards
