Chronic Health Issues are Best Addressed with 3 Visits to Establish an Individualized Treatment PlanInitial visit (#1 of 3): 75-90 minute medical interviewAmple time is spent to develop a working relationship with you, hear your complete health history and story, review current prescription medications and supplements, send home with a 4 day diet/exercise/sleep/stress/digestion diary and nutritional assessment questionnaire (NAQ), give release of records form to be sent to other doctors, order lab if appropriate, make treatment recommendations if appropriate.

Second visit (#2 of 3): 60 minute physical exam plus lab determination Assessment of conventional and nutritional physical exam parameters, receipt of 4 day diary and NAQ, receipt of current lab work and medical records, ordering of conventional and functional lab work.

Third visit (#3 of 3): 60 minute visit to review findings and discuss individualized treatment planA report of overall findings (synopsis of chief complaints, physical exam findings, diet diary info, lab parameters, underlying imbalances) is reviewed and the individualized treatment plan is prescribed and discussed. This typically consists of:
