NP Tiana Williams, The Little Clinic at North College Hill I wanted to share this encounter given by Tiana’s patient, not only because of the difference she made is this patient’s life, but because of the conviction and appreciation in the patient’s voice as she told the story that she said saved her life. The female patient came in for a visit a couple weeks ago and as Tiana began to examine her, she insisted that the patient go to the hospital for possible pneumonia. The patient said she was very resistant about going, but Tiana continued to express her concern and the importance of her going to the hospital. The patient returned to the clinic six days later looking to thank Tiana for everything she had done for her. She proceeded to tell me that today, after six days in the hospital, was her first day out after being kept on 10L of O2 and told that she could have possibly died had she not came in when she did. She asked me to thank Tiana a million times for her and stated that she could not express her gratitude for her. Patient Safety Events From Martha George, Compliance and Accreditation Manager

A patient safety event is an event, incident, occurrence or condition that could have resulted or did result in harm to a patient. A patient safety event can be, but does not have to be, the result of a defective system or process design, a system breakdown, equipment failure or human error. Patient safety events are often classified into the following categories: Adverse Event – a patient safety event that resulted in harm to a patient

• Example: A patient comes to TLC to receive their flu shot. Patient is on exam table. Provider gives flu shot and leaves the area to dispose of the empty syringe in the sharps container. While provider has back turned to patient – patient passes out and falls off the exam table face first, breaking his or her nose and three front teeth. Patient is disoriented, bleeding and 911 is called. The patient is taken to the ER and admitted for observation.

No-Harm Event – a patient safety event that reaches the patient but does not cause harm

• Example: A patient comes to TLC to receive their flu shot. Patient is on exam table. Provider gives the flu shot and stays with the patient for a few minutes. The patient passes out but is caught by the provider.

Near-Miss Event – a patient safety event that did not reach the patient

• Example: A patient comes to TLC and is diagnosed with Otitis Media. The patient is allergic to penicillin. The provider prescribes the antibiotic Augmentin. When the pharmacist reviews the Augmentin prescription she notices that the patient is allergic to penicillin and calls the provider. The provider realizes his or her mistake and prescribes another antibiotic.

Hazardous Event – an unsafe condition or circumstance (other than the patient’s own disease, process or condition) that increases the probability of an adverse event

• Example: A patient comes to TLC because they have been vomiting off and on for the past 12 hours. The clinician asks the patient if someone brought them to the clinic and the patient says yes. The clinician gives a Phenergan injection and allows the patient to leave the clinic without verifying that the patient has a ride and is not driving.

*All of the examples are taken from actual incident occurrences at The Little Clinic. A Sentinel Event is any unanticipated event in healthcare setting resulting in death or serious physical or psychological injury to a patient or patients, not related to the natural course of the patient’s illness. This will be addressed in more detail in next week’s News Blast. All Patient Safety Events must be reported via the electronic Occurrence Report System.

NP Betty Hahn at The Little Clinic on Stapley Drive in Mesa, Arizona

The clinic was short staffed this morning so Betty Hahn was running it by herself. She was helpful, professional and apologetic for the short delay we experienced as a result. I can’t thank her enough for the terrific service and care she provided us today and at other times when she has provided care for my family.

Each time we have visited the clinic we experienced great care and have always found the staff helpful and personable.
