Dear Friends,Another year has passed and it’s time once again to thank you all for everything you’ve done to support Opportunity House and the people we serve. Without your commitment to our mission we could not have met the needs of 240 individuals and their families.

From the opening of our accessible home on Center Cross Street to sending three Special Olympic athletes to the World Games in Los Angeles, the past year was a busy and productive one. With dedicated staff and board, involved parents, guardians and families, and a generous community we remain financially strong and firm in our commitment to provide the best possible service to adults with intellectual and developmental disabilities.

The past year was a very good one but challenges remain. After a decade in which we’ve seen no increases in reimbursement rates for the services we provide, the infrastructure we need to continue to provide the highest quality programming is eroding. Though we hope to see a turnaround in state funding we aren’t waiting for it and will secure our own future by increasing our capacity for private fund development. We hope you will continue to support us and help us meet the growing community need.

We accomplished a lot this past year and have a lot to be proud of so please take a few minutes and read through the report. Thank you again for helping make this a great year.

• Residential Services opened the Center Cross Home in October, 2014 increasing our capacity for serving mobility impaired individuals. With six accessible bedrooms at Center Cross and eight at our Maplewood home we have the capacity to meet accessibility needs for the foreseeable future.

• Respite Services provided 86 days of needed respite to area families, far exceeding their goal of 60 days.

• Community Support provided needed support and direction to 24 individuals living in their own home, and exceeded goals in maximizing independent living skills.

• OH Industries/Work Services generated nearly $245,000 in revenue by contracting with 16 companies to perform 341 different packaging and assembly jobs, providing 88 individuals with opportunities to earn wages while honing skills for community employment.

• Vocational Services placed 16 individuals in competitive community employment and received 100% employer satisfaction ratings on employer satisfaction surveys.

• Special Olympics sent three athletes to the World Games held July 25 – August 2 in Los Angeles, California. Playing for the American soccer team, Cody Zimmer, Brendan Morrissey, and Matt Sherman brought home bronze medals.

• Fundraising Events, Band Together for Special Olympics and the 6th Annual Golf Outing, raised over $39,500 for unfunded programs. • Developmental Training held a Craft Sale to raise funds for the purchase of a computer to use for client programming, hosted a luncheon to recognize staff and client for their community volunteer service, and achieved 100% compliance on their annual licensing survey.

• The Learning for Life graduated 10 individuals receiving Certificates of Accomplishment from Kishwaukee College at a commencement ceremony in November, 2015, and hosted an Art Show at the Developmental Training Center featuring the artwork of students of the program. • Carla Raynor, Learning for Life Instructor, received the Spirit of Caring Award from the Kishwaukee United Way in March, 2015.

Last year a random sample of 47 consumer satisfaction surveys of individuals, parents, guardians and other family members were conducted. The results showed continued satisfaction with the programs and services we offer.

• 96% of respondents indicated satisfaction with the services they receive.

• 100% indicated that their concerns were addressed in a timely manner.

• 92% of those interviewed would refer OH to other people.

Opportunity House programs were once again recognized for their quality. The DHS Bureau of Licensure, Certification and Accreditation, and the Bureau of Quality

Management conducted unannounced on-site surveys of our Day Program Services, CILA, and Home Based Services. Their findings of 93%-100% compliance with standards confirmed programming excellence.
