Annual Open Enrollment
The Glendale open enrollment period for the 2010/2011 employee benefit plan year will begin May 1, 2010 and conclude on May 21, 2010. Open enrollment meetings will start on April 19, 2010.
Open enrollment changes will be effective July 1, 2010.
This is your once a year opportunity to add, drop or change existing coverage.
Next opportunity to make a change without a qualifying event will be May 2011 for a July 1, 2011 effective date.

Examples of preventive care
• Mammogram – annually for women starting at age 40 for normal risk populations
• Colonoscopy – every 10 years starting at age 50 for normal risk populations
• Immunizations appropriate to age
• Pap smear – once annually for women who are 18 years of age or older

• Breast biopsy to identify abnormality on a mammogram
• Colonoscopy and biopsy in a person with unexplained bleeding

Medical ID Cards All employees who move to a new medical plan will
receive new ID cards. The cards will be shipped to your home mailing address.
Continue to use your current ID cards until 6/30/10. Use the new ID cards effective 7/1/10.
Employees who enroll in the HSA $2,500 medical plan option will receive a debit card by 7/1/10.

Optional Life Employee: An amount between $10,000 and $500,000, in increments of $10,000, not to exceed
5x basic annual earnings. Guaranteed Issue Amount is $100,000. Benefits cease at retirement. Any amount in force prior to age 70 reduces to $30,000 from age 70-79, and then
$1,000 at age 80+.
Spouse: An amount between $5,000 and $250,000, in increments of $5,000. Guaranteed Issue amount is $50,000. Spouse Optional Life coverage may not exceed 50% of the employee’s coverage. Any amount in force prior to age 70 reduces to $15,000 at age 70.

Flexible Spending Account – Wage Works
Contribute to up to $3,000 per year to the health savings account and/or up to $5,000 per year to the dependent care savings account.
Money is deducted from your paycheck on a “before tax” basis to cover the cost of eligible items such as: copays, glasses, dental work, prescriptions, daycare for your children, etc.
Don’t forget the use-it-or-lose-it rule
Should you enroll in the new HSA $2,500 plan, you can enroll in a Limited Purpose FSA to receive reimbursement for Dental, Vision and/or Dependent Care expenses. The HSA account can be used for Medical, Dental and Vision expenses as well.
