Participants will receive:• Professional instruction by the Magic Dancers in two (2) exciting dance classes!• All participants will PERFORM on the Orlando Magic court during HALFTIME on Dec. 14. in front of thousands of Orlando Magic fans.• Two (2) Tickets to Orlando Magic game vs. Golden State Warriors on Dec. 14.• A video link of their footage from on-court performance.• An official camp T-Shirt.• Photo autographed by the Magic Dancers!

I hereby authorize the staff of the Jr. Magic Mini Dance Camp to act for me according to their best judgment in any emergency requiring medical attention, and I hereby waive and release RDV Sports, the Orlando Magic, the National Basketball Association and its Member Teams, NBA Properties, Inc., and each of their respective parents, subsidiaries, affiliates, directors, owners, employees and agents from any and all liability for any injuries or accidents incurred while at camp. I have no knowledge of any physical impairment that would be affected by the named participant in the camp program outlined. I also understand that the camp retains the right to use for publicity and advertising purposes photographs of participants taken during the clinic. The Jr. Magic
Mini Dance Camp does not qualify for the child and dependent care expense credit under the IRS guidelines.
