What is the trap?
• We often re-train without proper incident investigation and direct root cause analysis – So we re-train– Assumption that employee lacked necessary
knowledge– Blame culture

• The mission/goal– Specific job type procedures and task– Identifying hazards/risk – To maintain the bottom line- production
• Avoiding work stoppage by way of injuries
– To keep employees safe• Creates good culture
