Appointment of MHOs

** Substantially in place and improving; won’t be fully up to projected numbers but sufficient for core statutory duties. East Dunbarton Council previously reporting significant risk but improvements reflected in ED para below. East Renfrewshire Currently have 11 operational MHOs with a further 2 on the training course. This should be sufficient to meet our new responsibilities South Lanarkshire Service model reviewed. Discussions underway regarding deployment of existing MHO’s across care groups and geographical locations which will be followed by detailed operational systems GCC For GCC: Review of previous MHO workload analysis in progress to reflect Practice team/CHSCP configurations. Nominations (16 places) secured for further MHO award training May & September 2005 programme. East Dunbartonshire The Council has 4 MHO’s employed as well as 1 agency MHO. The Authority also has 3 social workers currently on MHO qualifying courses. One will qualify in October, the other 2 in November. There are plans to recruit 1 more MHO In relation to MHO’s, the situation in East Dun has improved. The Mental

Health Team Manager also an MHO has now returned from sick leave. As of now we have 5 MHO’s available. By October when the new act comes into effect we will have 8 MHO’s available to us. There are also another 2 who will qualify in January / February 2006 By November depending on retaining current staff the authority will have at least 7 MHO’s (possibly 8) including the Mental Health Team Manager. While still red the increase will mean that we are in a considerably improved situation West Dunbartonshire WDC –AMBER / GREEN– By August 2005 WDC will have 7 WTE social workers who are MHOs of which 3 are in specialist MH teams including 2 SSWs (down from 5.5 in June). One social worker is on the Scotland West MHO Training Programme (April – October) and one other will commence MHO training in April 2006. Recruiting to 3 social worker posts in specialist MH Teams during August. Managing Risk – • a new Mental Health Services Team Leader (MHO not counted

above) now in post will assist operational management & deployment.

Significant capacity problem in number of day 2 s22 training events organised by NES. Large shortfall of s22 trained AMPs anticipated by Glasgow Board meeting 1.9.5. This has been communicated to NES and RCPsych - National need for more day 2 training in advance of 5.10.05 and / or transitional arrangements that allow s20 current approval to be transferable to s22 approval before full day 2 training completed. - joint training materials created as resource for all agencies - over 75% general nursing community and hospital mental health staff now trained ( as of 5.7.5) - acute hospital staff being trained through liaison psychiatry services - team based, joint social work and mental health vignette based training being rolled out. - joint medical records, MWC, Tribunal, MHO, nursing and psychiatry one day event to run through use of forms 17.6.5 - GP training events in September = 2 evening meetings. MHO’s also accessing further 2 day training with the consortium September 2005 2 staff currently undertaking MHO training programme. GCC Transitional training of MHO’s complete. General awareness training (1000 + staff) in progress. Joint training programme in progress. East Dunbartonshire Approximately 60 people in East Dun have been through 1 days training on the Mental Health Act. There are further dates scheduled for September we have around 100 more people to get through the training Transitional training has to be undertaken for 3 MHO’s. This is scheduled to take place in October West Dunbartonshire WDC – GREEN - Transitional MHO Training completed. Between January and March 2005, 421 staff in WDC, voluntary, private sector and community organisations received Level 2 and Level 3 education and training in the 2003 Act. WDC has commissioned the Lomond & Argyll Advocacy Service to deliver MH Act briefing sessions to services users and carers. One social worker is on the Scotland West MHO training

Child and Adolescent Mental Health Assessment 24/7 in place; expansion of treatment options on 24/7 based on progressive implementation from Jan 05 to Jan 06; funding identified for health and social care elements of this expansion based on agreed protocols and interface between CAMH’s and adult managed crisis services providing out of hrs and emergency cover support to CAMH’s. South Lanarkshire SAMH Crisis Service 2 year pilot in East Kilbride/Strathaven area being developed. This increased capacity will ensure that in the area we can offer a comprehensive health and social work rapid response as an alternative to hospital admission. It will also mean that the service is available between 9am- 9pm and at weekends. This increase in intermediate services will mean that capacity is freed up within the CMHT to enable them to provide an assertive outreach service to the proportion of their clients who are difficult to engage.

For the NHS GG part of WDC there are agreed arrangements whereby childrens advocacy and forensic advocacy is provided on GGHB wide basis. No such specific advocacy services in A&C. Boundary consultation has had knock on effect on planning.

GCC Equal Access Employment Strategy in place – Work Development Team Model expanded. Joint Agreement on 5 Community Development posts (pr CHP) for Mental Health to progress social inclusion agenda on locality basis. East Dunbartonshire Reconfiguring of a clubhouse service is now well progress. Recruitment is under way of support workers who will provide a network of social and therapeutic support groups to the various communities of East Dunbartonshire. These workers will also provide individual social support to clients who have enduring mental ill health and who are extremely socially isolated. The aim of this will

be to reduce the social exclusion of people with mental health issues. We have an in principal agreement to increase investment in vocational services. We are considering expanding the OT service within CMHT to vocational assessment as well as increasing the capacity of our voluntary sector provision. There are other broad areas of agreement for investment and this is in relation to dual diagnosis, mental health and homelessness and mental health and self harm West Dunbartonshire WDC –AMBER / GREEN - Significant investment has been made in community support services in WD. This includes needs led supporting people services in Older People (including functional and organic MH services), Addictions, Learning Disability, Adult Mental Health, Brain Injury, Alcohol Related Brain Damage, Forensic and Childrens Services. There is thus an extensive and flexible community support structure in place. Extra capacity in Council and commissioned well being services has been created through additional investment. However increasing demand for intensive care and support packages (S25), especially for people with associated risk to self and public (challenging behaviour, MDOs) will inevitably compete with resources earmarked for S 26 services. A S 26 mapping exercise has been commissioned – reporting by end of August. Resource in place for Oct – improvements to continue subject to resource funding.

Assistance with travel

Links locally have been established with local transport strategy group West Dunbartonshire WDC – GREEN – WDC provides transport where necessary (for example transporting people with learning disability to Resource Centres). Benefits are maximised and assistance provided with claiming DLA Mobilty Allowance to enable and promote independence. Information is available on concessionary travel. £1000 transport fund set aside against S27 needs. Delivered by Oct.

Care options

Learning Disability Advocacy GREEN GCC The Partnership has recently commissioned crisis citizen advocacy from Equal Say. The service will be provided to people with a learning disability affected by the Mental Health (Care & Treatment) Act in Glasgow and East Renfrewshire. Priority for this service is given to those individuals who are at risk of losing their liberty or compulsion under the Mental Health Act. Access routes into this service are open to individuals, families, services and other relevant individuals/agencies who promote the rights of people with learning disabilities. Priority for a crisis advocacy service will be given to:

• People subject to the Mental Health Act; • People directly affected by Mental Health

legislation. As with any model of citizen advocacy, the Project Co-ordinator will try to match advocate and partner and this will ideally take several meetings. However, due to the pressures of responding to urgent mental health crisis, there may not be sufficient time for matching. In order to promote the service the Crisis Co-ordinator will ensure that literature on the service is available throughout Glasgow and will have responsibility for raising awareness of the service to individuals and relevant agency staff, eg Mental Health Officers and Locality Operations Managers across Glasgow. East Dunbarton Persons with a learning disability will be able to access the enhanced professional advocacy service which will work with people subject to the Act West Dunbartonshire Council - In place and delivered by Oct.

Named Person/Advanced Statements

GREEN GCC Likely to be implemented by October 2005. Not likely to be high numbers of individuals requiring detention ‘prerequisites’ – will require to be an analysis of past detention volume (say over the last 3 years) to inform future service responses and progression of Named Person entitlement. Advanced statement numbers likely to be greater although hopefully manageable. Main issues appear to be around responsive storage of statements – proposed merger with Mental Health Medical Records will support this. Guidance/formatting of written materials in a way which is sensitive/specific to individuals with a learning disability represents a significant challenge. Again, should be implemented by October 2005. Work has been ongoing to produce guidance for staff around advanced statements and named persons under the new mental health legislation. This is now at the stage of a second draft and has been circulated to the people involved in the various working groups around the Act. East Dunbarton It is our intention that the advocacy service we commission take on significant levels of responsibility for assisting clients with named persons/ advance statements. Learning Disability clients would have access to this resource South Lanarkshire Council Procedures in place. Little impact to date. Staff briefed. Anticipated that all services/organisations will participate in assisting individuals progress same. WestDunbartonshire Council - Examining proposal for Advance Statement assistance to be provided by LAAS in association with Community Law Centre. Expect delivery by Oct. Age appropriate community and inpatient services

AMBER GCC Likely to be implemented by October 2005. In - patient services responsive to age 16 years plus (adult services) – therefore safety of younger individuals is a potential challenge. Good community team (again adult service) interface with C.A.M.H.S. services and children’s services, and responsive transition planning is high on the Partnership agenda. Interface between Learning Disability and Mental Health In patient services, including L.D. Forensic services is developing well, as is that between community teams/specialist city–wide services and In patient services. Services for women with children being developed on a support for inclusion basis. In-patient services accommodation being reviewed in terms of level of security and required responsiveness. WDC – In the community, specialist SW Disability Team cover young people with LD. Integrated LD service under single manager serve adults and older people with LD. Delivered by Oct. East Dunbarton Clients with Learning disability who are subject to the 1984 Act to the 1995 Criminal proceedings Act are in receipt of community and inpatient services however we have to fully scope out the age appropriateness of these services .

Awareness training on the Act for staff in LD services

GREEN Glasgow City Council The Partnership has two Professional Nurse Advisers who are trained in the process of rolling out awareness sessions to Health staff (Nursing). Awareness sessions have also been provided to social care providers, service users and carers. Between December 2004 and March 2005 48 qualified nursing staff from Inpatient Assessment and Treatment Services and Out of Hours Services attended a full day’s training on the Act. During this time 43 unqualified nursing staff from these services attended training. The training contained an introduction and overview of the new Act, Civil Detention, Criminal Procedures, Safeguards and other relevant issues, supported by three case scenarios. At the end of each session staff were provided with information on where to learn more on the Act and we are at present beginning to identify the individual learning nursing staff have undertaken since attended the training. Each area of service has also included the Act as part of clinical support sessions, particularly for staff unable to attend the sessions. Assessment and Treatment Services staff will also be revising the awareness information at their Service Network in September, a few days prior to the implementation of the Act. Raising awareness to the Act has also been provided in a learning session with social care providers and, more recently, a one day seminar with service users, carers and other Partnership staff. West Dun Council – Level 2 & 3 Workshops & briefings completed March ’05. More planned for later in the year. Amps for LD will join in MHO / AMP workshops in Sept ’05. East Renfrewshire Training programme for all groups of staff has been underway for some time East Dunbartom The Joint Health and Social Work LD Team have been included in the training that is being provided on the Act. SLC – Training agenda as noted earlier

Crisis response GREEN GCC Well developed (Nursing) Health managed Partnership Out of Hours service with 24/7 support arrangements (adults) and well developed interface with other crises response services, eg Social Work Standby Services, NHS 24, Glasgow Emergency Medical Services (GEMS), Mental Health Out of Hours Services, Police, Criminal Justice Service. Workforce planning GREEN GCC Specialist MHO (learning disability) remains a problem area. Consultants are progressing well through their learning. Training on Mental Health (Care and Treatment) (Scotland) Act is ongoing but requires a specific Learning Disability day two to be developed West Dun Council - NHS & Council staff operate in one team under single manager. East Dunbarton The joint LD team has 1 Social worker currently undertaking MHO training. This will ensure that the LD team has someone who has an expertise on the new Act. They will qualify as an MHO early next year. SLC 2 staff currently undertaking MHO training programme. MHO’s dispersed across care group locations.

Other issues

WDC – Information sharing and information management systems have yet to be put in place with NHS A&C. The Minister announced recently that A&C will be absorbed into NHS GG and NHS Highland following consultation on boundaries (Aug – Oct). There is a degree of uncertainty until the consultation is completed. MHO service at the State Hospital is in deficit and recruitment problematic. There are implications for all Councils and issue has been raised at national level (via ADSW MH Group). WD in discussion with TSH re implications & also use MDO Transfer & Discharge Protocol drawn up in 1999. MH Act Forms can not be saved – national issue that has been raised.

Appendix 1 Additional comments in response to previous JLiP assessment points ( primarily issues raised related to CAMH’s ) 1) Further information was requested on the options and proposals for transitional arrangements for children and young people (1) Transitional arrangements for children and young people will be greatly assisted by the structural changes in integrated CHPs i.e. joint child and adolescent teams and integration with social work, and the closer relationship between CAMHS and adult services within CHP structures. Specific care groups will benefit from proposals that will ease transitions i.e. Eating Disorders (effectively a 12 - 25 service if plans run to fruition) and esteem ('reach down'). Autism services are another strong candidate for these benefits that are also being developed in LD partnership. (2) Assurances that work had been undertaken on single shared assessments for child and adolescent mental health (2) Single shared assessment is being piloted in the MGF2 consortium in Glasgow, initially for child protection cases and then for disability and LAAC. This uses an integrated assessment framework which is compatible with the IAF being launched by the SE this summer. Electronic solutions are also being piloted in MGF2 that result in electronic sharing of information between health, social work and education, making it easier to populate and share info in the IAF. The pilot is in May 2005 for 6 weeks. NHS child and adolescent staff are involved in this pilot. (3) Assurances also that children and young people were included in patient/public involvement (3) Plans to involve children and young people, parents and carers are being developed as a result of an extensive organisational development programme with stakeholders in CAMHS carried out over the last year. One of the 3 recommendations accepted by the CAMHS planning group proposed the development of a user carer involvement plan for CAMHS. By November 2005, user/carer involvement plans will be agreed by stakeholders and implemented over the next 12 months across CAMH services (within the new integrated CHP structure). This will be the vehicle used to develop a plan of involvement of children, young people and carers in the development of the new adolescent inpatient unit. Plans have been agreed with the 5 local authority partners to have 8 stakeholder events over the next 18 months, one in each CHP, focusing on the implementation of the National Framework for Promotion, Prevention and Care and this will involve children, young people and their parents and carers. Services for adolescents with Eating Disorders already has a steering group with parent and carer involvement and this will be developed as part of the consultation on the proposed 12 -25 service.

In addition to this extensive funding programme, additional resources have been committed: £6m cap and £3.5m revenue (additional £1.7m rev on current levels) for adolescent inpatient unit (shared by WoS boards) £250K for adolescent eating disorders £300K for adult eating disorders £150K for MH Act resource £70K for young people's advocacy £40K for adolescent liaison £548K for implementation of ASL Act (impacts on autism and LD CAMHS developments) The earlier feedback also sought assurances/further clarification on the position in relation to learning disabilities and the MH Act and further information in relation to EDC risks around MHO’s. This information is currently being progresses and supplementary information will be forwarded in due course to respond to these points.
