Crisis en gezondheid
De economische crisis gaat niet ongemerkt aan ons voorbij; sommige landen worden harder getroffen dan anderen; dit geldt eveneens voor bedrijven binnen ons land.
Crisis wordt doorgaans gekoppeld aan financiële middelen (euro’s in Nederland), maar uit onderzoek blijkt dat de crisis ook gevolgen heeft voor de gezondheid. In The Lancet (oktober 2011) beschrijven Kentikelenis et al., dat in Griekenland de crisis en de daarmee samengaande toename van werkeloosheid, een grote toename in als slecht ervaren gezondheid van de (beroeps)bevolking tot gevolg heeft gehad en tot 25% meer zelfmoorden heeft geleid dan in 2009. Vooral gevoelige groepen, o.a. mensen met chronische aandoeningen en/of ouderen, worden het hardst getroffen. In tijden van crisis bezuinigen bedrijven op arbozorg en gaan ze over tot reorganisatie en inkrimping. Dit is vanuit financieel oogpunt wellicht een goede zet, maar vanuit gezondheidkundig oogpunt helemaal niet. Reorganiseren brengt stress teweeg en een verhoging van de werkdruk voor degenen die in het arbeidsproces blijven, met alle gevolgen van dien. Werknemers in zware omstandigheden verdienen steun om risico op uitval te verkleinen en arbeidsparticipatie te bevorderen.
De monitoring en bewustwording van gezondheidsrisico’s op de werplek, waarvoor ondersteuning en begeleiding door arboprofessionals cruciaal is, zouden in tijden van economische crisis juist niet ‘het kind van de rekening’ moeten zijn, maar zoals gezegd, is helaas het tegendeel waar.
‘Arbeidsparticipatie voor iedereen’, is het motto van de overheid, maar dat vereist maatwerk en evaluatie van de maatregelen en van de begeleiding die wordt ingezet. Preventief Medisch Onderzoek (PMO) bijvoorbeeld, is een goede aanpak maar wordt nog niet door alle bedrijven toegepast en bovendien is onderzoek naar de effectiviteit van PMO voor arbeidsparticipatie nog schaars.
De implementatie van verworven kennis en advies over de veiligheid en gezondheid op het werk is voor werknemers, werkgevers en overheid noodzakelijk om preventie van arbeidsgerelateerde aandoeningen te realiseren. Daarvoor is samenwerking en afstemming tussen onderzoek en praktijk noodzakelijk. Helaas is dit niet altijd aan de orde en nog minder in tijden van crisis. Praktijkgericht onderzoek is het afgelopen jaar een kernactiviteit van de afdeling geweest waarmee goede resultaten zijn behaald. Dit zal ook het komend jaar een uitdaging blijven. Wij gaan ervoor.

Crisis and health
The economic crisis is not passing us by unnoticed; some countries are being hit harder than others, and this also applies to companies in our country.
Crisis is generally linked to financial resources (euros in the Netherlands), but research shows that the crisis is also having an impact on health. In The Lancet (October 2011), Kentikelenis et al., describe how in Greece the crisis and the accompanying increase in unemployment has resulted in a large increase in poor health as perceived by the (working) population and 25% more suicides than in 2009. Sensitive groups, such as people with chronic diseases and/or the elderly, are hit hardest. In times of crisis, companies economize on occupational health care as they turn to reorganization and cutbacks. While this may be a good move from a financial perspective, it is anything but good from a health point of view. Reorganization brings about stress and an increase in workload for those who remain employed, with all the associated consequences. Employees in tough conditions deserve support to reduce the risk of dropout and to promote participation in the labor force.
The monitoring and awareness of health risks in the workplace, for which support and supervision by occupational health professionals is crucial, should not be ‘the victim’ in times of economic crisis, but, as stated above, unfortunately the converse is true.
‘Work participation for everyone’ is the motto of the government, but that requires customization and evaluation of the measures and of the support provided. Workers’ Health Surveillance (WHS), for example, is a good approach but is not yet applied by all companies, and, furthermore, research on the effectiveness of WHS for work participation is still sparse.
The implementation of acquired knowledge and recommendations on safety and health at work is essential for employees, employers and the government in order to achieve prevention of work-related disorders. This requires cooperation and coordination between research and practice. Unfortunately this is not always on the agenda, and even less so at times of crisis. Practice-oriented research has been a core activity of the Institute in the past year, with good results achieved. This will continue to be a challenge in the year ahead. Let’s go for it.

Het Coronel Instituut voor Arbeid en Gezondheid is een
afdeling binnen het Academisch Medisch Centrum (AMC)
te Amsterdam en ressorteert onder de Divisie Klinische
Methoden en Public Health. Het instituut richt zich op
vraagstukken op het gebied van arbeid en gezondheid.
Het hoofd van de afdeling is prof. dr. Monique HW Frings-
Dresen. De gezondheid van de werknemer of patiënt en de
gezondheidszorg voor hen staan centraal. Er wordt gestreefd
naar optimale preventie van beroepsziekten en bevordering
van arbeidsparticipatie. Het Nederlands Centrum voor
Beroepsziekten (NCvB), het Kenniscentrum Verzekerings-
geneeskunde AMC-UMCG-UWV-VUmc (KCVG), de
Polikliniek voor Mens en Arbeid (PMA) en het Kenniscentrum
Medische Keuringen in Arbeid (KMKA) zijn onderdelen van
de afdeling. Het instituut neemt deel in de Netherlands School
of Public and Occupational Health (NSPOH) en de AOWP
(Academische OpleidingsWerkPlaats).
De Wereld Gezondheids Organisatie (WHO) heeft het
Coronel Instituut voor Arbeid en Gezondheid aangewezen als

The Coronel Institute of Occupational Health is a department
of the Academic Medical Center (AMC) in Amsterdam and is
part of the Division of Clinical Methods and Public Health.
The Institute’s research focuses on the field of occupational
health. The department is headed by prof. dr. Monique HW
Frings-Dresen. Its mission relates primarily to the health and
healthcare of employees and patients. The aim is to optimize
the prevention of occupational diseases and to promote
participation in the labor force. The department includes the
Netherlands Center for Occupational Diseases (NCvB), the
Research Center for Insurance Medicine AMC-UMCG-UWV-
VUmc (KCVG), the People and Work Outpatient Clinic
(PMA) and the Expertise Center of Pre-employment Medical
Examinations and Workers’ Health Surveillance (KMKA).
The Institute participates in the Netherlands School of Public
and Occupational Health (NSPOH) and the Academic
Collaborative Training Center (AOWP).
The World Health Organization (WHO) has designated
the Coronel Institute of Occupational Health as a WHO
Collaborating Center in Occupational Health. The Coronel
Institute is a particularly active member of the International
Commission on Occupational Health (ICOH).
The department has four professors, prof. dr. Frank JH van
Dijk, prof. dr. Monique HW Frings-Dresen, prof. dr. Haije
Wind, professor by special appointment of Stichting Instituut
Gak, and prof. dr. Carel TJ Hulshof, professor by special
appointment of the Netherlands Society of Occupational
Medicine (NVAB). The department’s research is directed by
four Principal Investigators (PIs): prof. dr. Frank JH van Dijk,
prof. dr. Monique HW Frings-Dresen, dr. Judith K Sluiter and
dr. Sanja Kezic. The education provided in the Curius
curriculum, to Medical Informatics students, and in post-
graduate and master’s courses, is coordinated by the educational
staff, chaired by dr. P Paul FM Kuijer. Prof. Monique HW
Frings-Dresen and dr. Henk F van der Molen manage the
NCvB and prof. dr. Monique HW Frings-Dresen and dr. Bas
Sorgdrager manage the PMA; dr. Judith K Sluiter and
prof. dr. Carel TJ Hulshof manage the KMKA; the KCVG has
its own management structure.

Doel onderzoekslijnDe onderzoekslijn schenkt aandacht aan het ontstaan en de preventie van arbeidsgerelateerde aandoeningen i.e. beroeps-ziekten. Het onderzoek richt zich enerzijds op factoren in arbeid die (beroeps)ziekten veroorzaken en anderzijds op vraag stukken rond arbeidsre-integratie bij mensen met een (chronische) aan doening. Een aantal (chronische) ziekten/aandoeningen in relatie tot werk zijn aandachtspunt van studie, zoals aan-houdende vermoeidheid, klachten van het bewegingsapparaat, reuma, tumoren, en niet-aangeboren hersenletsel.Samen met de curatieve sector wordt onderzoek uitgevoerd op het gebied van arbeidsmogelijkheden tijdens het behandel proces van de patiënt, zoals bijvoorbeeld met het Gastro-Intestinaal Oncologisch Centrum Amsterdam (GIOCA), en reumatologie. Een aantal onderzoeksprojecten is gericht op de kwaliteit van verzekeringsgeneeskundig handelen, zoals bijvoorbeeld naar factoren die langdurige arbeidsongeschikt heid in stand houden, naar interventies die effectief zijn bij (zieke) werklozen met depressieve stoornissen en naar factoren die relevant zijn bij de beoordeling van jonggehandicapten (Wajongers) en voor bege- leiding naar werk bij patiënten met niet-aangeboren hersenletsel.
Wat is in 2011 bereikt?In 2011 is de tweede fase van het onderzoekprogramma van het Kenniscentrum Verzekeringsgeneeskunde AMC-UMCG-UWV-VUmc van start gegaan waarin als eerste stap met stakeholders is besproken wat relevante en op de praktijk toe- gespitste onderwerpen zijn. TargetThis line of research addresses the etiology and prevention of work-related disorders, i.e. occupational diseases. The research focuses on work-related factors that cause occupational diseases as well as issues surrounding the occupational reintegration of people with chronic diseases. A number of chronic work-related diseases/disorders are the subject of study. These include chronic fatigue, musculoskeletal disorders, rheumatism, tumors and acquired brain injuries.In cooperation with the curative sector research is being conducted on the possibilities of patients working during the treatment process. Examples include the Gastrointestinal Oncology Center Amsterdam (GIOCA) and rheumatology. A number of projects concern the quality of insurance medicine, such as the factors that perpetuate long-term occupational disability; finding effective interventions for people who are unemployed (due to illness) and suffer from depressive disorders; and factors that are relevant to the assessment of young people with disabilities (Wajong) and to helping people with acquired brain injuries find work.
What was achieved in 2011?The second phase of the research program of the Research Center for Insurance Medicine AMC-UMCG-UWV-VUmc was launched in 2011, with the first step being to discuss relevant and practice-oriented issues with stakeholders. The research program Reintegration Improvement Study (RVO) funded by the Stichting Instituut Gak held its concluding symposium in 2011, with a small number of projects carrying on into 2012. Two doctorates were conferred, to Margot Joosen for her research on effective rehabilitation strategies for patients with chronic fatigue symptoms, and to Marie-Christine Plat for her research on the implementation of job-specific worker’s health surveillance (WHS) for fire suppression personnel. These two doctoral programs were carried out under the responsibility of two PIs, Monique Frings-Dresen and Judith Sluiter, and attracted a lot of attention from the field. The feasibility of E-health intervention to help rheumatism patients retain their jobs and/or return to work was investigated in cooperation with the AMC’s rheumatology department. A major implementation study to prevent musculoskeletal injuries in

Door middel van literatuuronderzoek en

afstemming ten aanzien van arbeids participatie
geëffectueerd. Zorgverleners, die betrokken
zijn bij NAH en arbeidsparticipatie, dienen
over voldoende expertise te beschikken. Een
kern van bedrijfsarts of verzekeringsarts in de
arborol en revalidatiearts, bijgestaan door
neuropsycholoog, arbeidsdeskundige (en
eventueel) zorgcoördinator, is wenselijk.
Samen dragen zij zorg voor eenduidigheid ten
aanzien van informatieverstrekking en voor
continuïteit in hulpverlening waarbij al vroeg
aandacht besteed wordt aan arbeid en de
visie van patiënt en mantelzorger worden
meegenomen.
De multidisciplinaire NAHrichtlijn voor
beoordelen, behandelen en begeleiden (3B),
biedt betrokken professionals, werkgevers en
patiënten handvatten hoe arbeidsparticipatie
bij NAH, vanaf het begin tot het moment van
arbeidsparticipatie en daarna, kan worden
vormgegeven. Wetenschappelijk onderzoek is
noodzakelijk om dit proces verder te ont
wikkelen en samenwerking tussen betrokken
disciplines te intensiveren en te optimaliseren.
Multidisciplinary healthcare coordination with
regard to participation in the labor force was
implemented through a review of the literature
and consensus. Healthcare providers involved
in acquired brain injury and participation in
the labor force must have sufficient expertise.
A core of an occupational physician or insurance
physician of an occupational health service,
assisted by a neuropsychologist, employment
expert and healthcare coordinator (if necessary),
is advisable. Together they are responsible for
clarity with regard to the provision of information
and for continuity of care, with attention for
work and the views of the patient and informal
carers taken into consideration at an early stage.
The multidisciplinary acquired brain injury
guideline for assessment, treatment and
support serves as a guide for professionals,
employers and patients in the interpretation
of participation in the labor force for patients
with acquired brain injuries, from the beginning
up to the time of labor force participation and
afterwards. Scientific research is needed in
order to further develop this process and
intensify and optimize cooperation between
the disciplines concerned.

gefinancierde onderzoekprogramma Re-integratie Verbeter Onderzoek (RVO) heeft in 2011 het eindsymposium gehouden, een gering aantal projecten lopen nog door in het jaar 2012. Twee promoties hebben plaats gevonden, namelijk van Margot Joosen, gericht op effectieve re-integratie strategieën bij patiënten met aanhoudende vermoeidheidsklachten en van Marie-Christine Plat naar de implementatie van een beroeps-specifiek PMO voor repressief brandweerpersoneel. Deze twee promotietrajecten zijn onder de verantwoordelijkheid van twee PI’s uitgevoerd, namelijk Monique Frings-Dresen en Judith Sluiter, en hadden veel belangstelling vanuit de praktijk. Samen met de afdeling reumatologie (AMC) is de haalbaarheid van een E-health interventie voor behoud en terugkeer naar werk voor reumapatiënten nagegaan. Ter preventie van klachten

Dit project wordt in 2012 gecontinueerd met een implementatie onderzoek van de richtlijn in de praktijk.
the Netherlands Organization for Health Research and Development (ZonMw), a guideline on the assessment, treatment and support of patients with acquired brain injuries and their participation in the labor force was developed in cooperation with professionals from various disciplines and patients. Binnen het thema ‘Kwaliteit van de zorg op het gebied van Arbeid en Gezondheid’ staan vier onderwerpen centraal:
de mentale gezondheid, beroepsziekten, werkenden met een
chronische aandoening en de verspreiding van wetenschap-
pelijke kennis.
Psychische problemen en aandoeningen hebben ongunstige
gevolgen voor de werkende, maar mogelijk ook voor anderen
zoals klanten, leerlingen en patiënten. Dit jaar verschenen drie
publicaties van het project Mental Vitality at Work over een
groot onderzoek bij verpleegkundigen en paramedisch
personeel. Publicaties gaan over de eigenschappen van een
nieuwe vragenlijst om het functioneren op het werk goed te
kunnen meten.
The four main topics within the ‘Quality of Care in Occupational Health’ theme are: mental health, occupational
diseases, employees with chronic diseases and the
dissemination of scientific knowledge.
Psychological problems and disorders have adverse effects on
the affected employees themselves, and possibly also on others
such as customers, schoolchildren and patients. This year,
there were three publications by the Mental Vitality at Work
project, a large study conducted among nurses and allied health
professionals. The publications are about the qualities of a new
questionnaire to measure performance at work.
More and more employees have chronic diseases. Qualitative
research shows that the existing support does not help in the
transition from partial to full resumption of work among
employees with psychological problems. Fatigue and
depression, accompanied by few modifications at work, are

Kwalitatief onderzoek laat zien dat de bestaande begeleiding
niet helpt bij de overgang van gedeeltelijke naar volledige werk-
hervatting bij werknemers met psychische problemen. Bij
kankerpatiënten die weer aan het werk zijn komen vermoeid-
heid en depressie veel voor, samenhangend met weinig aan-
passingen op het werk. Beide onderzoeken bieden aanknopings-
punten voor de praktijk. Het blijkt dat de opvattingen van
human resource managers en lijn managers over hoe werknemers
te ondersteunen bij het behoud van hun werk, deels gelijk maar
ook deels verschillend zijn. Dit is belangrijke informatie voor
arbodeskundigen die nauw met hen samen werken. Een door
ons zelf ontwikkelde ‘empowerment’ training voor deze groep
werknemers blijkt gelukkig succesvol: het zelfvertrouwen in
het eigen kunnen neemt toe en de vermoeidheid wordt minder
in vergelijking met een controlegroep.

common among cancer patients who return to work. Both
studies are useful to improve health care practice. It turns out
that the views of human resource managers and line managers
on how to support employees in keeping their jobs are the
same in some respects and different in others. This is
important information for occupational experts who work
closely with them. Fortunately, the ‘empowerment’ training
course we developed for this group of employees is proving
successful, with an increase in self-confidence in their own
ability and a decrease in fatigue compared to a control group.
Occupational diseases are still common. Many patients with
brain disorders caused by exposure to solvents (chronic
solvent-induced encephalopathy) have been found to have
problems with depression and anxiety disorders, a finding that
can lead to better support. A study by the Netherlands Center
for Occupational Diseases produced a favorable finding among
employees with arm, neck and/or shoulder complaints: their
symptoms improved significantly within one year. Reports of
occupational diseases by employees themselves are a new
theme of research. The first article has been published.
The question is what happens to the thousands of scientific
articles on work and health. The importance of scientific
knowledge for the work of insurance company physicians was
explained in an article through examples. Along with the
experts, employees and managers have many questions about
work and health, but are often unable to find good answers.
Therefore an online network of experts (ArboAntwoord) has
been set up to answer questions on the Internet. This network
is rated highly and the information provided is clearly of a
higher quality than that found when seeking answers oneself
on the Internet. The Coronel Institute operates on an
international scale, for example for the WHO. Our Spanish
publication on how to find valuable information on occupational
health on the Internet is used in Latin America and Spain.
Beroepsziekten komen nog steeds veel voor. Patiënten met een
hersenaandoening door oplosmiddelen (chronic solvent
encephalopathy) blijken veel problemen te hebben met
depressie en angststoornissen, een constatering die kan leiden
tot een betere begeleiding. Gunstig is een bevinding bij
werkenden met klachten aan de arm, nek en/of schouder in
een onderzoek door het Nederlands Centrum voor Beroeps-
ziekten: zij blijken in één jaar tijd sterk te verbeteren in hun
klachten. Meldingen van beroepsziekten door werkenden zelf
is een nieuw onderzoeksthema. Een eerste artikel is verschenen.
De vraag is wat er gebeurt met de duizenden wetenschappelijke
artikelen over werken en gezondheid. Het belang van weten-
schappelijke kennis voor het werk van verzekeringsartsen is
met voorbeelden uitgewerkt in een artikel. Werknemers en
leidinggevenden hebben, net als deskundigen, veel vragen over
werken en gezondheid, maar kunnen vaak geen goede ant-
woorden vinden. Daarom is een online netwerk van deskundigen
opgezet (ArboAntwoord) om vragen via het internet te
beantwoorden. Dit netwerk wordt bijzonder gewaardeerd en
biedt informatie van een duidelijk hogere kwaliteit vergeleken
met antwoorden door zelf te zoeken op het internet. Het
Coronel Instituut is internationaal actief, zoals voor de WHO.
In Latijns Amerika en Spanje wordt gewerkt met onze
publicatie in het Spaans over hoe te zoeken naar waardevolle
informatie over werken en gezondheid op het internet.

Wat is in 2011 bereikt?De gezondheid en werkvermogen van beroepen in de
gezondheidszorg, en meer specifiek van ziekenhuispersoneel
stonden centraal in 2011. De ontwikkeling van een functie-
specifiek PMO voor medisch specialisten (in opleiding) is
afgerond en de haalbaarheid van dit PMO is getest. Drie medisch
specialismen van één UMC deden mee bij het testen. De arbo-
dienst kon het opgestelde protocol goed uitvoeren en meer dan
tweederde van de deelnemende artsen gaven aan te verwachten
dat deelname aan het PMO en de verkregen adviezen hun
gezondheid en werkfunctioneren positief kan beïnvloeden.
Voor verpleegkundigen en paramedisch ziekenhuispersoneel is
een psychische module voor PMO ontwikkeld en getest in een
gerandomiseerd gecontroleerd onderzoek met drie onderzoeks-
armen (een controlegroep en twee PMO strategieën). PMO met
een bezoek aan de bedrijfsarts werd hierbij vergeleken met een
volledig elektronisch PMO waarbij E-health interventies
digitaal werden aangeboden. De menselijke, medische maat

and crew, elite athletes and construction workers).
• Specific patient populations (e.g. chronic fatigue, rheumatoid
arthritis/RSI, acquired brain injuries, stress/psychological
disorders, (congenital) cardiovascular diseases).
What was achieved in 2011?The health and work ability with regard to healthcare
occupations, and specifically of hospital staff, were the focus in
2011. The development of a job-specific WHS for medical
specialists and residents has been completed and the feasibility
of this WHS has been tested. Three medical specialties of one

This line of research defines the development, implementation
and evaluation of medical examinations (preemployment and
mandatory medical examinations and workers’ health
surveillance) in collaboration with the sectors concerned. The
Expertise Center of Preemployment Medical Examinations
and Worker’s Health Surveillance (KMKA) is run together with
the Netherlands Society of Occupational Medicine (NVAB)

University Medical Center took part in the testing. The
occupational health service was able to implement the protocol
effectively and more than two-thirds of the participating doctors
stated that they expected that participation in the WHS and
the recommendations received could have a positive influence
on their health and performance at work. A psychological WHS
module for nurses and allied health professionals in hospitals
was developed and tested in a three-armed randomized
controlled trial. Two strategies, a visit to the occupational
physician were compared with a fully electronic WHS >

Verder is een evaluatieonderzoek afgerond naar een interventie

tatie van hulpmiddelen bij de medische beoordeling van
werkvermogen staat centraal in twee projecten die door en
voor verzekeringsgeneeskundigen worden uitgevoerd.
Twee promotietrajecten zijn in 2011 succesvol afgerond:
Margot Joosen (re-integratie van chronisch vermoeide werk-
nemers met participatieproblemen) en Marie-Christine Plat
(implementatie van beroepsspecifiek PMO voor repressief
brandweerpersoneel) promoveerden. In 2011 zijn 11 nationale
en 21 internationale wetenschappelijke publicaties verschenen.
Op internationale congressen werd kennis verspreid via
presentaties of posters. Maatschappelijk succes is dat zowel in
de ambulance- als in de brandweersector via CAO of andere
regelgeving vanaf 2011 is opgenomen dat de in deze onderzoeks-
lijn ontwikkelde medische keuringen en preventief medisch
onderzoek landelijk worden geïmplementeerd. Expertise is
ingezet voor NWO en ZonMw. Met het Kenniscentrum
Medische Keuringen in Arbeid zijn diverse onderwijs- en
adviesactiviteiten ondernomen. Twee nieuwe ZonMw
projecten starten in 2012: een nieuwe maat voor werk-
functioneren wordt ontwikkeld in de komende jaren en samen
met de NVAB is een project gestart waarin het preventie-
consult in de bedrijfsgezondheidszorg wordt uitgetest.

this context. The first doctorate will be conferred for this
latter study in 2012.
Further, an evaluation study was completed on an
intervention focused on reducing neck strain among
helicopter crew. The cohort study among medical
students from multiple UMCs, which enters its third year
in 2012, has led to its first publication (Van der Veer,
Frings-Dresen and Sluiter, 2011, PLoS One) and
received a lot of attention from the national media. The
development and implementation of tools used in the
medical assessment of work ability are the focus of two
projects being carried out by and on behalf of insurance
company physicians.
Two doctoral programs were completed successfully
in 2011: Margot Joosen (vocational rehabilitation of
workers with chronic fatigue and participation problems)
and Marie-Christine Plat (implementation of job-specific
WHS for fire fighters) obtained their doctorates. Eleven
Dutch and 21 international scientific publications were
released in 2011. Knowledge was disseminated via
presentations and posters at international conferences.
Societal impact was shown in the ambulance and
firefighting sectors collective labor agreements or other
regulations: the medical examinations and workers’
health surveillance developed in this line of research
will be implemented nationally from 2011. Expertise
was provided to the Netherlands Organization for
Scientific Research (NWO) and the Netherlands
Organization for Health Research and Development
(ZonMw). Various educational and advisory activities
were undertaken with the Expertise Center of Pre-
employment Medical Examinations and Worker’s Health
Surveillance (KMKA). Two new ZonMw projects will
commence in 2012: a new measure of performance at
work will be developed in the coming years, and a project
has been launched with the NVAB to test preventive
consultation in occupational healthcare.
Individuele gevoeligheid voor (arbeidsgerelateerde) huidaandoeningen PI: dr. Sanja Kezic
Het onderzoek richt zich op de betekenis van genetische
variaties voor de ontwikkeling en prognose van
beroepsgerelateerd contacteczeem (CE). Als mogelijke
“gevoeligheidsgenen” worden een aantal polymorphismen
onderzocht in genen die betrokken zijn bij de vorming van de
huidbarrière en bij het reguleren van ontstekingsprocessen.
De studies richten zich met name op de functionele gevolgen
van de genetische variaties in het filaggrine gen (FLG). FLG
mutaties leiden tot een minder goed functionerende
huidbarrière, waardoor de huid meer doorlaatbaar wordt voor
stoffen die sensitisatie en ontstekingen in de huid kunnen
veroorzaken. Deze mutaties zijn een belangrijke factor in de
predispositie voor atopische dermatitis (AD). Individuen met
een (geschiedenis van) AD hebben een grotere kans op het
ontwikkelen van CE. Echter, de afzonderlijke bijdragen van de
FLG mutaties en AD voor het verkrijgen van CE zijn niet
bekend.
Om te bestuderen in hoeverre FLG mutaties en AD bijdragen
tot het ontwikkelen van beroepsmatig CE werden twee
onderzoeken uigevoerd: een prospectief onderzoek uitgevoerd
bij leerling-verpleegkundigen en een case-control studie bij
patiënten met chronisch contacteczeem.
De functionele gevolgen van FLG mutaties en AD werden
onderzocht in een experimentele irritatiestudie. Gezonde
vrijwilligers en AD patiënten met en zonder FLG mutaties
werden blootgesteld aan water en aan een model irritatiestof
(sodium laurylsulfaat). De irritatierespons werd waargenomen
door het meten van de huidbarrière, huiderytheem, en het
gehalte van verschillende ontstekingsmediatoren en filaggrine
afbraakproducten. Daarnaast werden de morfologische
veranderingen in de epidermis bestudeerd door middel van
confocale microscopie (Vivascope, MAVIG).
De studies zijn onderdeel van het promotieonderzoek van
Maaike Visser en zullen afgerond zijn in 2012.

Individual susceptibility to (occupational) skin disorders PI: dr. Sanja Kezic
This line of research focuses on the significance of genetic
variations in the development and prognosis of occupational
contact eczema (CE). A number of polymorphisms in genes
which are involved in the formation of the skin barrier and in
the regulation of inflammatory processes are being
investigated as possible ‘sensitivity genes’.
The studies focus primarily on the functional consequences
of genetic variations in the filaggrin gene (FLG). FLG
mutations lead to an improperly functioning skin barrier, as a
result of which the skin becomes more permeable to
substances that can cause sensitization and inflammation in
the skin. These mutations are an important factor in the
predisposition for atopic dermatitis (AD). Individuals with a
history of AD have a greater chance of developing CE.
However, the individual contributions of the FLG mutations
and AD in the development of CE are not known.
To investigate the extent to which FLG mutations and AD
contribute to the development of occupational CD, two
studies are being conducted: a prospective study among
student nurses and a case-control study among patients with
chronic contact eczema.
The functional consequences of FLG mutations and AD were
investigated in an experimental irritation study. Healthy
volunteers and AD patients with and without FLG mutations
were exposed to water and to a model irritant (sodium lauryl
sulfate). The irritation response was observed by measuring
the skin barrier, skin erythema and the level of various
inflammatory mediators and breakdown products. In
addition, the morphological changes in the epidermis were
studied by means of confocal microscopy (Vivascope,
MAVIG).
The studies are part of the doctoral research of Maaike Visser
and will be completed in 2012.

Daarnaast verzorgt het Coronel Instituut onderwijs bij ‘in
company’ opleidingen, onder andere met het Nederlands
Centrum voor Beroepsziekten. Met KLM Health Services is een
workshopcyclus over het signaleren van beroepsziekten
gegeven.
De staf onderwijs zorgt voor ontwikkeling, coördinatie en
kwaliteitsborging. Dr. Paul Smits toonde aan dat 2e jaar-
studenten Geneeskunde bedrijfsgeneeskunde door e-learning
niet positiever waarderen dan het gebruik van boeken, artikelen
of richtlijnen (www.ncbi.nlm.nih.gov/pubmed/22034545). In
2011 hebben drie collega’s hun Basis Kwalificatie Onderwijs
(BKO) gehaald, een bewijs van didactische bekwaamheid voor
docenten in het wetenschappelijk onderwijs welke wordt
erkend door de Nederlandse universiteiten.
In 2011 is het Spaanstalige boek van prof. dr. Frank van Dijk,
‘¿Cómo buscar la evidencia en las fuentes de internet?’
verschenen als stimulans voor het gebruik van Evidence Based
Medicine in Zuid-Amerika. Er is ook een website voor
internationaal onderwijsmateriaal over arbeid en gezondheid
gestart: www.workershealtheducation.org
In 2011 heeft het Coronel Instituut voor Arbeid en
Gezondheid meer dan 200 studenten Geneeskunde in de
juniorco- en in de coschappen getraind om scherper te letten
op de rol van werk bij het ontstaan van ziekten èn de zorg voor
zieken ook te richten op deelnemen aan werk. Dit is gelukt
dankzij de goede samenwerking met de Arbodienst van het
AMC, ArboUnie, Achmea Vitale en HumanCapitalCare
Arbozorg, en anderen zoals sportartsen en zelfstandig
bedrijfsartsen. Ook het 2e jaars keuzeonderwijs ‘Topsport,
dans en muziek als werk’ is weer goed beoordeeld.
Het Coronel Instituut voor Arbeid en Gezondheid is ook actief
in de AMC opleiding Medische Informatiekunde en in de
opleiding, bij- en nascholing van bedrijfs- en verzekerings-
artsen bij de Netherlands School of Public & Occupational
Health (NSPOH). Dit laatste gebeurt in de Academische
OpleidingsWerkPlaats (AOWP) voor Arbeid, Maatschappij
en Gezondheid. In 2011 is een driejaarlijkse Topclass
Occupational Health onder verantwoordelijkheid van prof. dr.
Carel Hulshof gestart. Binnen de opleiding voor verzekerings-
artsen is de samenwerking met het Kennis centrum Verzekerings-
geneeskunde AMC-UMCG-UWV-VUmc versterkt.
In 2011, the Coronel Institute of Occupational Health trained
more than 200 medical students in (junior) internships to pay
closer attention to the role of work in the development of
diseases and to also focus patient care on participation in work.
This was possible thanks to the close cooperation with the
AMC Occupational Health and Safety Service, ArboUnie,
Achmea Vitale and HumanCapitalCare, and others such as
sports medicine specialists and independent occupational
physicians. The second-year elective program ‘Top-class
sports, dance and music as a profession’ was once again rated
highly.
The Coronel Institute of Occupational Health also plays an
active role in training students of Medicine and Medical
Informatics at the AMC, and provides training and additional
schooling to occupational and insurance physicians at the
Netherlands School of Public & Occupational Health
(NSPOH). This is done in cooperation with the Academic
Collaborative Training Center (AOWP) for Labor, Society and
Health. A triennial Occupational Health Topclass was launched
in 2011, under the responsibility of prof. Carel Hulshof.
Cooperation with the Research Center for Insurance Medicine
AMC-UMCG-UWV-VUmc within the study program for
insurance company physicians was intensified.

The Coronel Institute also provides in-company training in
collaboration with organizations such as the Netherlands
Centre for Occupational Diseases. A workshop cycle on the
identification of occupational diseases was given in cooperation
with KLM Health Services.
The teaching staff are responsible for development, coordination
and quality assurance. Dr. Paul Smits showed that second-year
medical students did not rate occupational medicine through
e-learning more positively than the use of books, articles or
guidelines (www.ncbi.nlm.nih.gov/pubmed/22034545). In
2011, three colleagues earned their Basic Qualification in
Education (BKO), a teaching competence certificate for
lecturers in academic higher education, which is recognized
by the Dutch universities.
Prof. Frank van Dijk’s Spanish book ‘¿Cómo buscar la evidencia en las fuentes de internet?’ was published in 2011 to encourage
the use of Evidence-based Medicine in South America. In

Monitoring en signalering van beroepsziekten
Het Nederlands Centrum voor Beroepsziekten (NCvB)
registreert in opdracht van het ministerie van Sociale Zaken
en Werkgelegenheid het vóórkomen van beroepsziekten in
Nederland. Bedrijfsartsen en verzekeringsartsen in de arborol
zijn wettelijk verplicht deze te melden aan het NCvB. Het
doel van deze beroepsziekteregistratie is het stimuleren van
preventie van werkgerelateerde aandoeningen, individueel én
collectief. Immers elke beroepsziekte is te vóórkomen. In het
verslagjaar 2011 is met een vragenlijstonderzoek extra aandacht
besteed aan het verkrijgen van inzicht in de factoren die het
melden van beroepsziekten beïnvloeden. Er lijken nu meer
barrières te bestaan dan tien jaar geleden voor een werkende
om een mogelijk werkgerelateerd gezondheidsprobleem voor
te leggen aan een bedrijfsarts. De meldende bedrijfsartsen
blijken vooral intrinsiek gemotiveerd door hun professionele
taakopvatting.
In 2011 zijn 6.989 meldingen van beroepsziekten geregistreerd.
Het merendeel van alle meldingen over 2011 komt uit drie
categorieën beroepsziekten, namelijk: gehooraandoeningen
monitoring en signalering
(40%), aandoeningen van het houding- en bewegingsapparaat
(34%) en psychische aandoeningen (19%). In tabel 1 staan de
tien meest gemelde beroepsziekten van 2011.
In totaal melden 174 bedrijfsartsen via het “Peilstation
Intensief Melden” de door hen gediagnosticeerde beroeps-
ziekten. Daarnaast wordt extra informatie gevraagd over de
aard en omvang van hun werknemerspopulatie. In 2011
zijn via dit peilstation 1472 meldingen van beroepsziekten
geregistreerd, hetgeen een incidentie betekent van 302 (95%
betrouwbaarheidsinterval (BI): 287-318) beroepsziekten per
100.000 werknemersjaren. Tabel 2 geeft een overzicht van
de incidentiecijfers van de beroepsziektemeldingen per
100.000 werknemersjaren per economische hoofdsectie met
het bijbehorende 95% BI over 2011.
Daarnaast wordt veel aandacht besteed aan de ontwikkeling en
verspreiding van registratierichtlijnen en wetenschappelijke
achtergronddocumenten als hulpmiddel voor het diagnosticeren en melden van beroepsziekten. >

Monitoring and notification of occupational diseases
The Netherlands Center for Occupational Diseases (NCOD)
registers occupational diseases in the Netherlands for the
Ministry of Social Affairs and Employment. Occupational
physicians of occupational health services and insurance
physicians are required by law to report these diseases to the
NCOD. The purpose of registering these occupational diseases
is to stimulate the prevention of work-related disorders,

disease can be prevented. During 2011, a questionnaire was
used to devote extra attention to obtaining insight into the
factors that influence the notification of occupational diseases.
There appear to be more barriers now than there were ten
years ago for employees to put possible occupational health
problems to an occupational physician. The reporting
occupational physicians are intrinsically motivated primarily
by their professional attitude. >

> Monitoring and notification of occupational diseases
In 2011, 6,989 reports of occupational diseases were
registered. The majority of all reports for 2011 were in three
occupational disease categories: hearing disorders (40%),
musculoskeletal disorders (34%) and mental disorders (19%).
Table 1 shows the ten most frequently reported occupational
diseases in 2011.
A total of 174 occupational physicians report occupational
diseases diagnosed by them via an Intensive Notification
Surveillance Project. They are also asked to provide additional
information regarding the nature and size of their employee
populations. In 2011, 1472 reports of occupational diseases
were registered via this surveillance project, corresponding to
an incidence of 302 (95% confidence interval (CI): 287-318)
occupational diseases per 100,000 employee years. Table 2
provides an overview of the incidence figures of occupational
diseases per 100,000 employee years per economic category
and the corresponding 95% CI for 2011.

In addition, a great deal of attention is paid to developing and
distributing registration guidelines and scientific background
documents as tools for the diagnosis and reporting of
occupational diseases. The guidelines can also be used with
regard to interventions and preventive measures. In 2011, the
website was completely redesigned and European information
notices were added. These information notices have been
compiled by experts from the European Union. They provide
additional information for the reporting of occupational
diseases related to exposure to chemical and physical factors.
This primarily applies to occupational diseases for which
NCOD registration guidelines are not yet available.

People and Work Outpatient Clinic
Binnen de Polikliniek Mens en Arbeid (PMA) worden complexe
vragen op gebied van arbeidsgerelateerde gezondheids-
problemen behandeld. De PMA is een topreferent centrum
binnen het AMC/Coronel Instituut voor Arbeid en Gezondheid
en vormt de patientenzorg binnen de afdeling; het is een
van de poliklinieken van het AMC. Door bedrijfsartsen,
verzekeringsartsen, huisartsen, medisch specialisten kunnen
vragen gesteld worden en patienten worden aangemeld;
daarnaast verricht de PMA expertises op verzoek van
verzekeraars of rechtbank. Onderwerpen die binnen de PMA
worden behandeld betreffen verschillende onderwerpen zoals
bijvoorbeeld OrganoPsycho Syndroom (OPS), gehoor-,
cardiologie-, oogheelkunde-, neurologie-, endocrinologie- en
bewegingsapparaatklachten en werk. Expertises gaan vooral
over het houdings- en bewegingsapparaat en arbeidsgebonden
huid aandoeningen. Incidenteel zijn er complexe vraagstukken
over kanker en werk, long aandoeningen en psychische
beroepsziekten.
Kenmerk van de vragen waarmee wordt verwezen is dat zij
een complex karakter hebben en dat de beantwoording
multidisciplinaire samenwerking vereist met inbreng van
arbeidsgeneeskundige en klinische expertise.
De PMA is door de Inspectie Leefomgeving en Transport, Rail
en Wegvervoer, aangewezen als expertisecentrum voor
advisering (aan keurend (bedrijfs)arts)) op gebied van medische
geschikt heidseisen en functionele mogelijkheden voor veilig-
heidstaken van spoorwegpersoneel. De advisering ligt daarbij
met name op gebied van medicijngebruik, problemen met
horen en zien, hartaandoeningen, diabetesvragen en neuro-
logische aandoeningen.
De verwerking van het toenemende aanbod is onderwerp van
evaluatie en vraagt herinrichting van het administratieve en
logistieke proces.
Complex questions concerning work-related health problems
are answered in the People and Work Outpatient Clinic (PMA).
The PMA is a specialist referral care center within the AMC/
Coronel Institute of Occupational Health and forms the patient
care branch of the department; it is one of the AMC’s outpatient
clinics. Occupational physicians, insurance company physicians,
general practitioners and medical specialists can ask questions
and refer patients; the PMA also conducts expertise studies at
the request of insurance companies or courts. Issues addressed
within the PMA concern a variety of subjects such as Organo
Psycho Syndrome (OPS), hearing, cardiology, ophthalmology,
neurology, endocrinology and disorders of the musculoskeletal
system in relation to work. Expertise studies mainly concern
the musculoskeletal system and occupational skin disorders.
Complex questions concerning occupational cancer, pulmonary
disorders and occupational mental disorders occur incidentally.
The questions referred are characterized by the fact that they
are complex in nature and that answering them requires
multidisciplinary cooperation with contributions from
occupational physicians and clinical expertise.
The Netherlands Shipping Inspectorate, Rail and Road
Transport has designated the PMA as a center of expertise
for consultancy (to examining physicians and occupational
physicians) with regard to medical suitability requirements
and functional possibilities for safety tasks of railway staff.
The consultancy focuses primarily on the use of medication,
hearing and eye problems, heart disorders, diabetes questions
and neurological disorders.
Processing of the increasing supply is a subject of evaluation and
requires a reorganization of the administrative and logistics process.

De Helpdesk van het NCvB (Nederlands Centrum voor
Beroepsziekten) richt zich met name op het beantwoorden van
vragen van professionals op het gebied van arbeid en gezond-
heid. De Helpdesk van het NCvB is een onderdeel van de
website www.beroepsziekten.nl en de website www.kiza.nl. Naast arbo professionals en medici kunnen bijvoorbeeld ook
beleidsmedewerkers en journalisten met vragen terecht bij
deze helpdesk. Er wordt naar gestreefd om een binnen komende
vraag binnen 2 werkdagen te beantwoorden. Soms is overleg of
literatuuronderzoek nodig. Voor de beroepsziektespecialisten
van het NCvB kan het probleem van de vragensteller een signaal
zijn van een groter achterliggend probleem of een nieuw risico.
In 2011 werden er 675 vragen gesteld aan de Helpdesk van
het NCvB. Ruim de helft van de vragen was afkomstig van
bedrijfsartsen; de andere vragen werden gesteld door arbo-
adviseurs, beleidsmedewerkers, journalisten, werkgevers,
juristen, studenten, huisartsen en medisch specialisten.

NCvB Helpdesk and ArboAntwoord
The Helpdesk of the NCvB (Netherlands Center for
Occupational Diseases) is aimed primarily at answering
the questions of professionals in the field of occupational
health. The NCvB Helpdesk is part of the websites
www.beroepsziekten.nl and www.kiza.nl. As well as occupational
health and safety professionals and doctors, this helpdesk is
also available to policy officials, journalists and others. The aim
is to answer questions within 2 working days of when they are
received. Sometimes consultation or a review of the literature
is required. For the NCvB’s occupational disease specialists, the
questions submitted may be an indication of a larger under-
lying problem or a new risk.
In 2011, the NCvB Helpdesk received 675 questions. More
than half of the questions came from occupational physicians;
the other questions were asked by Occupational Health Service
consultants, policy workers, journalists, employers, lawyers,
students, general practitioners and medical specialists.

Goed antwoord op gezondheidsvragen: ArboAntwoord
Ongezonde en onveilige werksituaties roepen vaak bezorgd-
heid en vragen op bij werkenden; niet alleen bij werknemers,
maar ook bij werkgevers, zelfstandigen, managers en
preventiemedewerkers. Een goed antwoord op deze vragen kan
bijdragen aan het verbeteren van werkomstandigheden; een
slecht antwoord brengt daarentegen mogelijk risico’s met zich
mee. Uit het promotieonderzoek van Martijn Rhebergen blijkt
onder meer dat werkenden regelmatig vragen hebben over
werk en gezondheid, maar dat ze zelf vaak incomplete of
onjuiste antwoorden geven. Onder meer omdat ze informatie
moeilijk kunnen vinden, selecteren en beoordelen.
Om werkenden te ondersteunen bij het verkrijgen van
kwalitatief goede antwoorden op vragen over werk en gezond-
heid, is daarom een online netwerk van experts gecreëerd:
ArboAntwoord. Via de website www.arboantwoord.com
konden werkenden (werknemers zowel als werkgevers en
preventiemedewerkers) vragen over gezondheid, veiligheid en
werk stellen aan circa 80 geselecteerde en hooggekwalificeerde
experts op het gebied van veiligheid, gezondheid en welzijn. In
de onderzoekperiode van twee jaar werden 851 vragen gesteld
door 460 vragenstellers. Verreweg de meeste vraagstellers
vonden ArboAntwoord gebruiksvriendelijk (81%), goed
toegankelijk (75%) en makkelijk te gebruiken (67%). De
vraagstellers beoordeelden de ontvangen informatie en
antwoorden als volledig (69%) en bruikbaar (70%) en bijna
alle vraagstellers gaven aan dat zij de informatie snel genoeg
ontvingen (84%). Veel vraagstellers gaven ook aan dat de
ontvangen informatie en antwoorden impact hadden. Voor
74% van hen vergrootte de informatie hun kennis en inzicht.
Tweederde van hen gaf aan de informatie ook daadwerkelijk
toe te willen passen in de eigen werksituatie. Voor een kwart
van de vraagstellers veranderde de ontvangen informatie
daadwerkelijk iets aan de werksituatie, het functioneren op
het werk of de ervaren werkgerelateerde gezondheid.
Daarnaast is ook gekeken naar de kwaliteit van de antwoorden
ten opzichte van het in de literatuur gevonden bewijs. Hierbij
werden ook de antwoorden in de helpdesk van het NCvB
betrokken. Over het geheel genomen bleek de kwaliteit van de
antwoorden goed: 86-93% van de antwoorden gegeven via
ArboAntwoord en de Helpdesk bleek gedeeltelijke of volledig
in overeenstemming met de beste mogelijke evidence uit de
wetenschap en praktijk. In een onderzoek waarbij werknemers
standaard vragen beantwoordden met informatie verkregen
via ArboAntwoord of bronnen naar eigen keus, bleek dat
de informatie via de experts van ArboAntwoord veel vaker
kwalitatief goed en juist was dan die van andere bronnen.
Samenvattend blijkt uit het proefschrift dat er reden is om
aan te nemen dat werkenden met vragen over gezondheid
en werk meer hulp nodig hebben in het proces van vraag
naar antwoord. Momenteel kiezen veel zorgaanbieders,
kennisinstituten en overheden ervoor om informatie over
veiligheid, gezondheid en welzijn aan te bieden via websites.
Dit onderzoek laat echter zien dat het simpelweg aanbieden
van online informatie, zonder rekening te houden met de

Unhealthy and unsafe situations at work often raise concerns
among workers; not just employees, but also employers, the
self-employed, managers and prevention workers. A good
answer to these questions can help improve working
conditions; on the other hand, a poor answer can lead to risks.
The doctoral research carried out by Martijn Rhebergen shows
that workers regularly have questions about work and health,
but that they often give incomplete or incorrect answers
themselves. This is partly because they have difficulty finding,
selecting and assessing information.
Therefore, in order to help workers obtain good quality
answers to questions about work and health, an online
network of experts was created: ArboAntwoord. At www.
arboantwoord.com workers (employees, employers and
prevention workers) were able to ask questions about health,
safety and work to some 80 selected and highly qualified
experts in the field of health, safety and welfare. During the
two-year research period, 460 people asked 851 questions. The
vast majority of those who used the site to ask questions found
ArboAntwoord to be user-friendly (81%), easily accessible
(75%) and easy to use (67%). The users rated the information
and answers received as complete (69%) and useful (70%) and
almost all users indicated that they received the information
quickly (84%). Many users also indicated that the information
and answers they received had an impact. For 74% the
information increased their knowledge and understanding.
Two-thirds indicated that they actually wanted to apply the
information to their own situation at work. For one-quarter of
the users the information received actually changed something
about their work situation, their performance at work or their
perceived work-related health.

The research also examined the quality of the answers
compared to evidence found in the literature. This included the
answers in the NCvB’s helpdesk. Overall, the quality of the
answers was good: 86-93% of the answers provided through
ArboAntwoord and the Helpdesk corresponded fully or
partially with the best possible evidence from science and
practice. In a study in which employees answered standard
questions with information obtained through ArboAntwoord
or from sources of their choice, the information obtained from
the ArboAntwoord’s experts was found to be of a good quality
and correct far more often than that obtained from other
sources.
To summarize, the doctoral thesis shows that there is reason to
assume that workers with questions on health and work need
more help in the process leading them from questions to
answers. Many care providers, knowledge institutes and
government authorities currently choose to provide
information on health, safety and welfare via websites.
However, this research shows that simply providing online
information, without taking into account the barriers
encountered by workers while searching, is insufficient. When
workers can consult highly qualified experts for advice, for
example through an online expert network like
ArboAntwoord, the answers they receive are of a much better
quality. Such advice services have the potential to further
promote safe and healthy working, because the information
and advice provided often has an impact on both the person
asking the question and on his environment.

Wat houdt een promotietraject in?
What does a PhD study involve?
Een promotietraject houdt in dat een promovendus zich gedurende
vier jaar verdiept in een onderzoeksthema, maar ook wordt geschoold
in veel verschillende methoden van onderzoek en hiermee van
doctorandus (letterlijk: ‘hij/zij die nog doctor worden moet’) tot doctor
promoveert.
In mijn geval betekende het dat ik in april 2007 startte met mijn
werkzaamheden binnen het Coronel Instituut voor Arbeid en
Gezondheid. Vaak start een promotietraject met het uitvoeren van een
systematisch literatuuroverzicht over het onderzoeksthema waar het
promotietraject over gaat. Vervolgens kunnen de inzichten van het
betreffende artikel, de basis zijn van verder onderzoek. Alle onderzoeks
resultaten worden in artikelen beschreven, die worden gepubliceerd
in internationale tijdschriften zodat dit wereldwijd bekend wordt en
men hier gebruik van kan maken. Binnen ons instituut worden veelal
vragen uit de praktijk op een wetenschappelijk wijze onderzocht,
waardoor ook terugkoppeling naar de praktijk van groot belang is.
Naast de publicaties, dient er van het onderzoek een proefschrift te
verschijnen. Dit ontstaat door aanvullend aan de artikelen een
algemene introductie en discussie te schrijven, waarin het thema
wordt geïntroduceerd en achteraf bevindingen worden bediscussieerd.
Tijdens het promotietraject lever je als promovendus ook bijdragen
A PhD study involves a PhD student spending four years focusing on a
research theme, while being trained in different methods of research,
after which he/she is ‘promoted’ from being a doctorandus (literally
‘he/she who has yet to become a PhD’) to being a PhD.
In my case it meant that I started working at the Coronel Institute of
Occupational Health in April 2007. A PhD study often starts with a
systematic literature review on the research theme of the project. The
insights of the article in question can then form the basis of further
research. All research results are described in articles, which are
published in international journals so that the results are made
available for use worldwide. Within our institute many questions from
the field are investigated in a scientific manner, which means that
feedback to the field is of great importance.
In addition to the publications, a PhD thesis must be produced on the
research. This is done by supplementing the articles with a general
introduction and discussion, in which the theme is introduced and the
findings discussed afterwards. During the PhD study, as a PhD student
you also contribute to conferences in the Netherlands and abroad and
establish contacts with fellow researchers and people in the field.
After the PhD thesis is assessed by an independent reading committee,
the PhD study concludes with the actual defense of the thesis. This is a
day on which thoughts are exchanged with opponents from the
reading committee ‘in an organized manner’, followed by all kinds of
festivities. The greatest reward is, of course, that you can replace your
MSc. title with that of PhD!

Van kleine deeltjes, de dingen die voorbij gaan
Small particles, a never ending story?The asbestos problem is far from over. Professor Zielhuis told me
in 1976 about his interviews with widows of mesothelioma patients
in Walcheren. Almost all of the men had been working at a large
shipyard, where a lot of blue asbestos was used as insulation
material. A ban on asbestos came only into effect in 1993 in the
Netherlands and at a EU level in 2005. In 2010, approximately
500 people in the Netherlands died from this fatal asbestosrelated
disease.
The subject of Workrelated Cancer is something I have followed
throughout my working life. In 1980, Zielhuis and I wrote >
De asbestproblematiek is nog lang niet voorbij. Professor Zielhuis
vertelde me in 1976 over de interviews met weduwes van mesothelioom
patiënten in Walcheren. De mannen hadden bijna allen op de
scheepswerf De Schelde gewerkt, waarbij veel blauwe asbest als
isolatiemateriaal werd verwerkt. Maar pas in 1993 kwam er een
asbestverbod in Nederland, en in Europa in 2005. In 2010 overleden
in Nederland circa 500 mensen door deze fataal aflopende asbest
gerelateerde aandoening.
Het onderwerp Kanker door Werk heb ik mijn hele arbeidsleven
bijgehouden. In 1980 heb ik hierover samen met Zielhuis >
aan congressen in binnen en buitenland en doe je contacten op
met collega onderzoekers en mensen uit de praktijk.
Na de beoordeling van het proefschrift door een onafhankelijke
leescommissie, wordt het promotietraject afgerond met de
daad werkelijke promotie. Dit is een dag waarbij ‘op een geregelde
manier’ van gedachte wordt gewisseld met opponenten uit de
leescommissie, waarna allerlei feestelijkheden volgen. De mooiste
beloning is natuurlijk dat je vervolgens je titel Drs. mag vervangen
voor Dr.!
Voor mij betekende het promotietraject een interessante vier jaar
werken als onderzoeker, onder begeleiding van dr. Judith Sluiter
en prof.dr. Monique FringsDresen, met als onderzoeksthema de
bedrijfsgezondheidszorg in zware beroepen. De implementatie van
het preventief medisch onderzoek voor brandweerlieden werd
hierin bestudeerd. Het was een leerzaam traject, waarbij ik in
oktober 2011 promoveerde tot doctor!

For me, the PhD study was an interesting four years spent
working as a researcher under the supervision of Judith Sluiter,
PhD and Prof. Monique FringsDresen, with the research
theme of occupational healthcare in highdemand jobs.
The implementation of the workers’ health surveillance for
firefighters was studied. It was a very informative project,
after which I was conferred a PhD in October 2011.

geschreven en daarna op allerlei plekken voorlichting over de risico’s
van blootstelling aan asbest mogen geven. Soms was dat emotioneel
beladen, zoals bij de Rijkswerf in Den Helder, waar ik voorlichting gaf
aan mannen van wie de vader of broer aan asbest overleden was en
die zelf ook een forse blootstelling hadden gehad.
In maart 2012 was er paniek onder de bemanning van een bagger
schip in de haven van Dubai dat onder de asbestdeeltjes zat, omdat
loodsen van asbestcement op de kade grofstoffelijk werden gesloopt
met ongunstige windrichting. Ik werd gevraagd om ter plekke
voor lichting en counseling aan de ongeruste mannen te geven. Echt
geruststellen kon ik ze niet, maar wel risicocijfers laten zien waaruit
blijkt dat de incidentele blootstelling beduidend minder risico met zich
meebrengt dan bij de echte asbestwerkers.
Ook heb ik een uitnodiging gekregen om voor de Wereldgezondheids
organisatie (WHO) in Tirana een cursus over asbestgerelateerde
ziektes te geven aan de Arbeidsinspecteurs en gezondheids
autoriteiten in Albanië. Daarna mag ik in Dresden een discussie leiden
over zin en onzin van screening van asbestwerkers met CTscans;
heel eervol!
Je vastbijten in een onderwerp en dat niet meer loslaten heeft mijn
leven erg verrijkt. De laatste jaren heb ik me ook in de risico’s van het
werken met nanodeeltjes verdiept. De 65jaarsgrens heb ik over
schreden, maar ik werk nog volop met veel plezier en moet strak
plannen om in de zomermaanden de zeiltocht naar Scandinavië te
kunnen maken.

Tijdschrift voor Geneeskunde). Afterwards I was asked to provide
information on the risks of asbestos exposure in all kinds of
workplaces. Sometimes this was emotionally charged, for example
at the Naval Shipyard (Rijkswerf) in Den Helder, where I provided
information to men whose fathers or brothers had died as a result
of asbestosrelated disease and who had been exposed to significant
amounts of asbestos themselves.
In March 2012, there was panic among the crew of a dredger in the
port of Dubai, which was covered in asbestos particles after ware
houses, containing asbestos cement on the quay, were demolished
without proper precautions during unfavorable wind conditions. I was
asked to provide the worried men with information and counseling on
site. Although I was not able to truly reassure them, I showed them
risk statistics which indicated that incidental exposure is clearly
associated with lower risks than those faced by real asbestos workers.
I have also been invited to give a course on asbestosrelated diseases
to Labour inspectors and health authorities in Tirana, Albania, on
behalf of the World Health Organization (WHO). Afterwards, I will go
to Dresden to chair a discussion on the pros and cons of screening
asbestos workers by means of CT scans; a real honor!
Grabbing hold of a subject and never letting go has truly enriched my
life. In the last few years, I have also studied the risks of working with
nanoparticles. My 65th birthday has been and gone, but I still very
much enjoy working and will have to manage my schedule carefully in
order to be able to make the sailing trip to Scandinavia in the summer
months.

weilanden was het dit jaar tijd voor een educatiever programma in
de Domstad. De feestcommissie had het Spoorwegmuseum in petto,
waar we een kijkje kregen in andere werelden en tijden. Na ontvangst
in het Maliebaanstation zijn onder andere de Oriënt express, een
mijndorp uit 1800 en het koninklijk rijtuig van Bernhard en Juliana de
revue gepasseerd. In de 19e eeuw was het duidelijk anders gesteld
op het spoor met de arbeidsomstandigheden voor de stokers,
machinisten en remmers. Zo liep de conducteur aan de buitenkant
van de trein over een plank om zo de kaartjes te knippen. Alarm slaan
bij een sein ging via een touw met een grote bel. Waar machines nu
het werk doen, ging het destijds om noeste arbeid.
Omdat samen met het natte weer ook de speurtocht in het water viel,
trokken we verder naar de gemoedelijke sferen van de Utrechtse
werfkelders. Hier zorgde de steengrill voor hoge temperaturen en
hebben we gezellig gedineerd met deze bijzondere groep collega’s!
Following an active sailing trip on the IJmeer and a Solex tour among
the farmland, this year it was time for a more educational program in
Utrecht. The organizing committee had the Railway Museum in store
for us, where we caught a glimpse of different worlds and different
eras. After being welcomed at Maliebaan Station, we viewed the
exhibits, including the Orient Express, a mining village from 1800 and
the royal carriage of Bernhard and Juliana. Things were clearly very
different on the railways in the 19th century in terms of the working
conditions for the firemen, engineers and brakemen. The conductor
walked on a board along the outside of the train to punch the tickets.
The alarm was raised by ringing a large bell with a rope. What
machines do for us today used to require hard manual labor.
Because the treasure hunt was a washout along with the wet weather,
we moved on to the cozy atmosphere of Utrecht’s wharf cellars where
the stone grill provided higher temperatures and we enjoyed dining
with this special group of colleagues!
