IV THERAPY (Hanging primary IV line) 1. Wash hands 2. Knock door 3. Introduce myself (hello, my name is Yary, I’m a student nurse and I'm going to be changing the IV bag. 4. May I see your ID please? (Full name and DOB). Do you have any allergies that I should be aware of? 5. Give me just a minute I’m going to wash my hands and gather the supplies 6. Equipment needed: 1 IV bag 0.9% NS 1 continuous tubing pack (primary tubing) 1 Label of primary IV tubing (bright orange) 1 Pair of gloves 1 Long colored label strip 1 5ml flush syringe 2 alcohol swabs 7. Prep everything on table. Calculate the drop factor (gtt/ml) 8. Put on gloves 9. Raise bed at waist level 10. Check IV site for phlebitis or infiltration. 11. Open syringe package, take cap off, take 1 alcohol swab clean the heplock port on patient’ hand. Draw back the syringe a little bit and push forward to get rid of the air bubble. Connect it to the patient heplock and flush. Let them connected. 12. Verify expiration date of the NS bag and that is clear. Write on the colored strip label. Open the 0.9% NS bag, paste the strip matching the ceros, and hang it. Open the continuous tubing bag, CLAMP IT AND LABEL IT with 1 of the 2 Iv tubing label. Remove the NS IV bag port cover(don’t use the yellow tan). Remove the cap from the spike and insert it inside the bag. Press the chamber to allow the solution to flow in 1/3 of the chamber (until the mark). 13. Time to prime the IV to get rid of the air. Invert (put down) the first port and close to the clamp to get rid of the

MAKE SURE ONLY 1 DROP CAN LEAK. 14. Open another alcohol swab, remove syringe on patients hand, clean the port again and insert the IV TUBE. Once connected make sure clamp on pts IV tubing is open. 15. Proceed to set the rate of the gtt/min. LOWER PTS BED BEFORE SETTING THE RATE. The rate is found dividing TOTAL # of gtt (calculated at the beginning) by 4. That is the amount of drops in 15 secs. Once I manage to set the correct rate in 15 secs I proceed to count for 1 min, to make sure is the correct amount of drops in 1 min. Then call the professor to verify. 16. Well this is all for now, is there anything else I can do for you?

IV MED (Hanging secondary IV line aka Piggy back)

1. Wash hands 2. Knock door 3. Introduce myself (hello, my name is Yary, Im a student nurse and I'm going to be changing the IV bag.1. May I see your ID please? (Full name and DOB)2. Give me just a minute Im going to wash my hands and gather the supplies3. Equipment needed: 1 Piggy back 1 secondary IV tubing. 1 Label of Piggy back tubing (bright orange) 1 Med added label (deep orange) 1 Pair of gloves 2 alcohol swabs 4. Prep everything on table. Calculate the infusion rate (gtt/min). Look in the drug book, the medication, under intermittent infusion and use the longest time. Ex. 30 min. On the back of the piggyback is the drop factor (15gtt/ml). Calculate piggy back amount in ml/time of med (ex 30 min) x drop factor in back of the piggy back package. 5. Put on gloves6. Raise bed at waist level 7. Check IV site for phlebitis or infiltration. 8. Verify expiration date of the Piggyback. Write the data on both labels. Open the Piggyback, paste the deep orange label on the back (not written part of the bag) and hang it. Use the blue hanger and hang the primary IV bag (that way stays lower than the secondary) 9. Open the intermittent tubing bag and CLAMP IT AND LABEL IT with the IV tubing label already written. Remove the piggyback port cover (not the tan one). Remove the cap from the spike and insert it inside the bag. Press the chamber to allow the solution to flow in 1/3 of the chamber (until the mark). 10. Time to prime the tube to get rid of the air. Open the clamp slowly, let water run through. MAKE SURE ONLY 1 DROP CAN LEAK. 11. Open another alcohol swab, clean the port (closest to the primary IV bag already hanging and insert the Piggyback tube. Once connected make sure clamp on pts IV tubing is open. 12. Open the piggyback clamp and regulate the drop with the primary IV clamp (the blue one)17. Proceed to set the rate of the gtt/min. LOWER PTS BED BEFORE SETTING THE RATE (in the case of the piggy back is already low) The rate is found dividing TOTAL # of gtt (calculated at the beginning) by 4.
