ISPO Calls for more access to assistive technology Speaking at a side-event to the World Health Assembly (WHA) on 24 May in Geneva, ISPO representative Dr. Nils-Odd Tønnevold called on governments, the WHO and all stakeholders to step up action to ensure increased global access to Assistive Health Technology (AHT) for all people who need it. The WHA is the supreme decision-making body for WHO. It meets in Geneva in May each year, and is attended by delegations from all 194 UN Member States. Its main function is to determine the policies of the WHO and focus for the year to come. The fact that AHT has a place in the 2016 Assembly via a side-event presents a major opportunity to bring the issue of user empowerment and increased access to AHT solutions to the global health stage. Only about 10 % of the world’s population who require AHT actually gets it. This need is rising along with an ageing population, the huge

increase in the incidence of chronic diseases such as diabetes and a rise in trauma resulting from traffic and work accidents, war, terrorism and natural disasters. ISPO welcomes the GATE initiative launched by the WHO to foster cooperation for increased access to Assistive Health Technology and has been contributing to GATE since its inception including recently in the development of a WHO Priority Assistive Products List (APL). In parallel, ISPO has launched its own campaign, #ISPOWER aimed to raise awareness on the need to increase access to assistive health technology for people with disabilities. However, “much more needs to be done for the vision to become reality (…). We must continue to work hard and stay focused to achieve our vision of Global Access to Assistive Technology for all” said Nils-Odd in his statement. “It’s not an expense, it’s an investment” he concluded. ISPO World Congress ISPO 2017 World Congress first announcement - call for submissions! The First Announcement for the 16th World Congress 2017 in Cape Town is available - download your copy from the congress web site. The scientific committee welcomes your submissions for selection as poster or paper presentation at the congress, so start preparing it now! Bidding process to host the ISPO World Congress 2021! ISPO International is now launching the bid process to host

the 18th ISPO World Congress in 2021. ISPO Member Societies interested in bringing the global P&O community to their country or region are invited to complete and submit the Expression of Interest form by 14 October 2016. Following the revision of the received Expressions of Interest the ISPO International Executive Board will invite a selected number of Member Societies to submit full bid proposals. Before considering to engage in the process ISPO Member Societies should thoroughly read the Bid Guidelines, which describe the bid procedure, guidelines and requirements in detail. ISPO meets @ OT World 2016 ISPO leadership recently held a number of important meetings in Leipzig in the fringes of OT World (03-06 May 2016) where ISPO International was present with a booth in the international exhibition throughout the trade show.

Ahead of the OT World Congress and Exhibition, the ISPO Executive Board (EB) held a strategic session on 30 April, to take stock on ISPO’s key activities and set priorities for the next years. The results of a survey conducted by the ISPO International Head Office towards ISPO leadership, Member Societies, industry and NGO partners prior to the Strategy session, were presented. Based on these outcomes, EB members set the following 4 pillars of activities and identified key successes and upcoming priorities under each of these:

Pillar 1: Advocacy and awareness Pillar 2: Education, Training, Knowledge Exchange Pillar 3: Standards & Accreditation Pillar 4: Innovation & Evidence

Following this meeting, a strategic plan will be produced identifying priorities, actions, responsibilities and timelines for the coming 2-3 years. On 1 May, the ISPO Executive Board held its first face-to-face meeting of the year to progress on standard society operations (elections, constitutional changes, membership, etc.) and Committee activities (Education, Finances, Marketing, Publications, Short Courses, USAID, World Congress, etc.). Committee meetings were further held in the Leipzig Convention Centre in the following days thanks to the courtesy of ISPO-partner the Leipziger Messe (Finance, World Congress, Scientific and Education Committees). Representatives of about 20 ISPO Member Societies participated in the International Committee interim meeting held on 02 May. Being the governing body of ISPO International, the International Committee aims to provide a platform for information and updates, discussion and exchange. ISPO President Rajiv Hanspal and ISPO Committee Chairs updated participants on the various society programmes and offered national representatives opportunities to feedback, report on their local initiatives and challenges, ask questions to the ISPO international leadership and make suggestions and recommendations. A meeting of the ISPO Council of Industry Partners took place on 03 May where the ISPO leadership updated company representatives on ISPO’s participation in the WHO GATE initiative, ISPO’s work on the P&O Service Provision Guidelines and ISPO’s awareness raising campaign #ISPOWER. discussed including companies’ participation in ISPO World Congress 2017 in Cape Town (www.ispo2017.org). Finally, the #ISPOWER Steering Committee held its first face-to-face meeting and it was an opportunity for the ISPO Head Office and ISPO Member Societies of #ISPOWER pilot countries present in the room to update each other on the roll out of the campaign at central and local levels. A fruitful discussion was held with Members of the Steering Committee on identifying synergies to further cooperate under the #ISPOWER banner.

ISPO is an international organization with over 60 Member Societies working in close collaboration and with common goals. Historically, nominations for the Executive Board have come from Member Societies nominating individuals from a different Member Society. This supports the ‘international spirit’ of the organization and is encouraged. However, ISPO Member Societies are free to suggest one of its own members. The Executive Board should represent the multi-disciplinary membership of the society and the wide international distribution of the membership. Further to the criteria of proven leadership experience in national and/or international organizations and the evidence of commitment and potential contributions to the EB, it is therefore useful to consider the different professional disciplines and national background of the Board members to ensure an equitable distribution. The following nomination papers are to be submitted to ISPO Head Office:

1. The Nomination Template (downloadable below) to be completed by the candidate, which includes the candidate’s personal information, a motivation/vision statement not exceeding 250 words, and curriculum vitae (CV) of maximum one page of an A4 paper;

2. A supporting citation by the nominating Member Society or member not exceeding 500 words to present reasons for the nomination and highlighting why the candidate is considered appropriate for election.

ISPO welcomes new Member Society in Tanzania! The ISPO Board formally approved the formation of a Member Society in Tanzania at its latest meeting in June 2016.

ISPO Tanzania was established in 2016, with the following main goals: Facilitate accessibility to information and feedback from ISPO International to ISPO Members in Tanzania concerning issues pertaining the society. Bridge the gap on the limited annual membership payment options available to ISPO members residing in Tanzania. Create awareness of ISPO International by incentivizing students of ISPO-recognized programmes in Tanzania to register as members.

ISPO Turkey setting P&O education for Syrians ISPO-partner Human Study e.V., together with ISPO’s Member Society in Turkey and other initiatives such as Sequa, the National Syrian Project for Prosthetic Limbs (NSPPL) and Ankara University, have completed a Cat II-level P&O training programme in Ankara, with exams having taken place between 11-19 December 2015. The training focused on lower and upper limb prostheses casting, manufacturing and fitting, and were given at the level of Category II as established by ISPO International (note: the programme is not currently recognized by ISPO, but the programme coordinators expressed their wish to undergo ISPO recognition soon). Fifteen Syrian students and four Turkish students were educated and are now able to facilitate high patient demand. ISPO Turkey and Human Study e.V. are already working on offering a similar programme focused on orthoses. For more information, contact ISPO Turkey.

Partners updates

WHO adopts Priority Assistive Products List

On 24 May 2016, at the 69th World Health Assembly in Geneva, the WHO Priority Assistive Products List (APL) was adopted by the world’s health leaders. The launch event was hosted by the Health Minister of Pakistan Saira Afzal Tarar and the Health Minister of Ecuador Margarita Guevara along with other distinguished representatives. The launch took place during the side event ‘Assistive Technology for All’, at which ISPO made an official statement (read more here). The objective of the side event was to raise awareness about the need for and benefit of assistive technology and its relevance to the health sector prior to the WHO 139th Executive Board discussion on assistive

technology. The APL is the first step of WHO’s GATE initiative towards improving global access to assistive products for everyone, everywhere. ISPO contributed to the development of the list and congratulates WHO on this important achievement. The Priority Assistive Products List follows the principle of the WHO List of Essential Medicines; like the List of Essential Medicines, the APL aims to act as a catalyst in promoting access to assistive technology by creating awareness amongst governments, the public, directing public health resources and stimulating development. The APL is not a restrictive list but aims to provide each Member State with a model from which to develop a National priority assistive products list. Like the WHO Model List of Essential Medicines, the Priority Assistive Products List will also provide guidance for procurement and reimbursement policies, including insurance coverage. The APL includes wheelchairs, artificial limbs, hearing aids, communication aids, eye glasses, memory aids and other essential items to help older people and people with disabilities to be able to live an active, productive and dignified life. The APL is the first of four tools to be developed by the GATE initiative, towards increasing access to high-quality affordable assistive products as an integral component of universal health coverage. In addition to the APL, the WHO aims to :

Establish Single-window service provision model (ATSP)

ISPO USAID - Togo/Benin impact assessment report now available

The last report in our USAID funded impact assessment series is now available. As part of ISPO’s United States Agency for International Development (USAID) funded programme ‘Rehabilitation of physically disabled people in developing countries’ and building on past work, ISPO conducted a study evaluating the impact of training personnel to the minimum standards of ISPO Category II in Benin and Togo. ISPO also conducted a partial audit of the clinical skills and competencies of the graduates of the Ecole Nationale des Auxiliaires Médicaux of Lomé (ENAM) in Togo. Finally, the study determined the professional development needs of graduates in both Benin and Togo. In Africa, as for other less-resourced countries, access to appropriate prosthetic and orthotic services is hampered by different factors, including a lack of trained professionals. Established in 1977, the Ecole Nationale des Auxiliaires Médicaux of Lomé (ENAM) is the only French-speaking P&O training institution in Africa (and one of the few around the world). ISPO found the impact of ENAM graduates on clients, their families and communities was positive. They had positively impacted on the availability of P&O services and more globally on the development of rehabilitation services, despite the challenging economic and social context of Togo and Benin. The most significant impact is that most people with disabilities using their services have regained their mobility and are able to achieve their own goals. ISPO USAID developing country programme report now published Our programme report “Rehabilitation of people with physical disabilities in developing countries” is the latest publication from our positive collaborative agreement with ISPO-partner the United States Agency for International Development (USAID). This programme has accomplished much to support and inform us and the P&O community about the field of P&O in developing countries. It also offers us new hope and opportunities to develop our sector. We delivered three distinct activities:

1. Scholarship support 2. Measuring the impact of training in prosthetics

and orthotics 3. Enhancement of prosthetics and orthotics service

provision Firstly, our successful scholarship programme saw 112 candidates complete their scholarships to become prosthetists/orthotists and orthopaedic technologists, reflecting a 95% student progression rate due to rigorous selection criteria and students with a dedication to their studies. These scholarship alumni belonged to 34 different home countries and were all committed to work in prosthetics and orthotics clinical services after graduation. We welcome these new professionals into our field and look to them for fresh ideas and solutions to help meet the need for better services. Secondly our impact assessment series with field visits to 13 countries produced six impact assessment reports. When analysed all together, four themes of recommendations emerged for sector development:

Theme 1 = Leadership and Governance Theme 2 = Workforce Theme 3 = Service Provision Theme 4 = Practice and Technology

The impact assessment also generated new information about modes of learning: there were no significant differences in the professional practice of orthopaedic technologist graduates who attend face-to face training compared to blended distance learning training. We also discovered that ISPO Category I trained personnel took on more senior positions and higher level responsibilities in clinical, teaching and management roles than their ISPO Category II trained colleagues in developing countries. There are not enough ISPO Category I personnel working in less resourced settings. Thirdly, our work in partnership with the World Health Organization has resulted in progress towards the first WHO Standards for Prosthetics and Orthotics Service Provision. We believe that this is an extremely important piece of work and that the launch of these standards at our World Congress in 2017 will be a catalyst for the development of the prosthetics and orthotics sector. The Standards will be a significant milestone in providing UN member states and other stakeholders with guidance and

encouragement to enhance prosthetics and orthotics services in all countries, but will be of special significance in developing countries where poverty and disability are closely associated. We look forward to building on the body of work derived from the programme and improving the quality of life of the people we serve in prosthetics and orthotics services. Download the full report

What the Editor is Reading

According to the 2016 WHO Global Report on Diabetes, the global prevalence of diabetes has reached 8.5% of the adult population, nearly double the 1980 figure, and is rising faster in middle and low income countries than in high income countries. Reduced quality of life and high morbidity and mortality associated with the disease have made it a global health priority. Orthotists direct efforts to reducing the incidence of foot ulceration and subsequent amputation that result from foot deformities associated with the disease. In our themed issue on Lower Limb Orthotics (April), a paper by Allan, Munroe and Figgins (2016)reviews our current understanding of the causes of deformity. The common belief that foot deformity is a result of motor neuropathy and atrophy is not supported by the review. Much remains to be done to enable an understanding of the aetiology of deformity and to design interventions to reduce this problem. In systematic reviews of the medical literature, we often encounter a statement that only English language papers were included in the review. Readers are often left wondering whether there is valuable information in non-English publications that has been overlooked by the reviewers. It is reassuring to see a systematic review of Japanese language RCTs on the effectiveness of orthoses for management of knee osteoarthritis. The review by Mine, Nakayama, Milanese, and Grimmer (2016)recently published in OnlineFirst, makes a body of previously inaccessible evidence available to English language researchers and clinicians. The review supports the use of short-lever elastic coil knee braces for pain relief in walking and squatting but does not support the use of wedged foot orthoses. Phantom limb pain affects upward of 80% of amputees to some extent. It is a distressing problem for amputees and a frustrating and intractable problem for prosthetists. Recent trials of prosthetic liners with electromagnetic shielding have produced promising results for the management of this problem. A study by Fisher, Oliver, Sedki, and Hanspal (2016) published in the current (June) issue of POI presents a placebo-controlled, double-blind crossover trial which demonstrates the potential of this approach to provide some relief for amputees. Asian Prosthetic and Orthotic Scientific Meeting 2016 The Asian Prosthetic and Orthotic Scientific Meeting 2016 (APOSM 2016) will take place in Seoul (Korea) between 4-6 November 2016, organized and hosted by the Korean Society for Prosthetics and Orthotics together with the Korea Orthotics and Prosthetics Association and ISPO Korea. The

ISPO International Executive Board has been invited to attend the event and participate in the scientific programme. Furthermore the Executive Board will hold a face-to-face meeting just before APOSM. Organizers expect 500 participants from around the world to attend the event for discussions about new technologies and research results in the P&O fields. II ISPO Latin American Congress & VI ISPO Argentina Updating Conferences

The II ISPO Latin American Congress and the VI ISPO Argentina Updating Conferences will be held between 10-11 August 2016 in Buenos Aires (Argentina). The event will be organized by the alliance with the trade exhibition Expo-Ortopédica. Expo-Ortopédica and the ISPO Member Society in Argentina have organized this event on an annual basis since 2011. It has become a reference for all representatives from assistive technology in the region. The organizers welcomed 1010 attendants in 2015 and even more are expected this year. ISPO Argentina has been establishing alliances with different sectors and organizations with the aim of making the event a remarkable one. In 2016, the organizers will be working with the Sociedad Argentina de Medicina Física y Rehabilitación (SAMFYR), the Argentinian Society for Physical and Rehabilitation Medicine. The scientific symposium core themes will be adaptive sports, rehabilitation technologies, education in rehabilitation science, orthotic-prosthetic and diabetic foot.
