1.0 INTRODUCTION

This summary As-built report has been written at the request by Galore Creek Mining Corporation (GCMC) by the Independent Environmental Monitors responsible for the bridge construction – WestCan Projects (WestCan).

This document is required by the Department of Fisheries and Oceans (DFO) to comply with Section 5.1 of the DFO Authorization for Works or Undertakings Affecting Fish habitat; Authorization No.1 04-HPAC-PA1-000-000021 (herein referred to as the Authorization). The purpose of this document is to describe in detail the construction and environmental monitoring activities that occurred between May 20 to September 12, 2007 during the installation and construction of the temporary work platform and permanent bridge over the Iskut River. This report will review construction activities (in order of occurrence) with particular attention and detail to instream works associated with the project. In addition to the aforementioned information, this report will also address any changes that were made to the original Site Construction Plan and Practices as a result of unforeseen construction difficulties or prolonged periods of adverse weather conditions.

2.0 BACKGROUND The Iskut River is a tributary of the Stikine River and is located at kilometer 15+600 on the Devil’s Creek Forest Service Road (FSR)

The crossing structure at this location is a steel girder bridge with pier, approximately 109.7m in length. The installation of this bridge is to facilitate the transportation of mining goods and services to and from the mine during the life of the mine. A temporary work platform was also installed to aid

In the construction of instream works (pile driving of pier grouping) as the original temporary access was not suitable due to existing site conditions. 3.0 MONITORING PROGRAM

The following is a description of the Independent Environmental Monitors (EM) Roles and Responsibilities as per GCMC contract standards and WestCans own set of criteria to meet or exceed these roles and responsibilities. In GCMC’s “Construction Environmental Standards and Guidelines” March 4 edition and latest August 13 edition. The EM’s roles and responsibilities were as follows:

• Ensure that the Environmental Coordinators, Galore Creek Construction are notified immediately of all potential non-compliance issues.

• Ensure that day to day filed related activities that require environmental monitoring are being undertaken in accordance with direction from the Environmental Coordinators.

• Have the authority to shut down a project if there is non-compliance and/or infraction until that non-compliance issue and/or infraction is rectified.

• Conduct environmental assessments ahead of the construction team in the field as required within his or her area of expertise and upon the advice of the Environmental Coordinator(s)

• Provide advice to the construction team, contractors etc. in the field within his or her area of expertise and seek advice from senior environmental team members as needed.

WestCan has ensured that all of the aforementioned roles and responsibilities were met on this project by providing qualified EM(s) that have experience working with road and bridge building projects. In addition we also conduct our own “in house” training ensuring that the EM (s) are aware of the project(s) status, on site contractors and GCMC staff to be contacted in case of an emergency. These site-specific details are either shared verbally, in writing (cross over notes) or in person. The criteria that our staff are continuously monitoring for are as follows:

• Is the contractor completing the installation as per the Environmental Management Plan (EMP) and General Arrangement Drawings

• Do they have a copy of all necessary permits on site • Is the contractor applying all Best Management Practices as per the EMP • Is the contractor using heavy machinery that has been cleaned of deleterious substances • Are all sediment and erosion control measures installed as per the schedule and correctly • Are all sediment and erosion control measures checked periodically, and maintained on a

weekly basis and more readily during intense precipitation events

All of the aforementioned criteria are tracked and documented from the project’s start to completion. WestCan has a series of documents that include, daily journal(s), turbidity monitoring table(s), fish absence and presence table(s) that are used to record the affects of the various construction activities on-site. Additionally, WestCan also provides aid in site isolation, and fish salvage as needed throughout the project. These items would have been recorded in the daily journal notes The following section details the daily events as observed by the Environmental Monitor (EM) while on-site. Appendix A shows a detailed photo log of these events and Appendix B includes the monitor’s daily journals. Appendix C summarizes the turbidity monitoring results for the entire project. Appendix D contains the Final Environmental Audit.

Construction Activity • installation of Bubble Curtain and lock block foundations for temporary work platform • Held pre work meeting with groups as to preparing works within the Iskut channel on the dry gravel bar, all Sigfusson Northern, McElhanney were in attendance during this meeting. • Established level working footprint for lock blocks, bubble curtain placement also occurred lat morning. • Lock blocks flown ion shortly thereafter for temporary work bridge foundation.

Environmental Issues/Concerns

• No environmental issues or concerns on-site this day. All works occurred on the dry gravel bar, with the exception of the Bubble Curtain, which was placed within the wetted channel using a helicopter. No sediment release was noted during this process.

• All instream works completed May 23. Cutting down road right of way (RoW) up to the Iskut River on either side or approach.

Environmental Issues/Concerns

Approached GCMC Environmental Department, Scott Davidson and Area Road Managers for an additional Environmental monitor as Doug Stokes Occupational Health and Safety Coordinator/Environment is occupied in meetings throughout the day and cannot fly out to site early with crews prior to commencement of works. As contractor will be starting instream works soon WestCan would like to have additional environmental representation on-site.

Please refer to site-specific details as per the mitigation plan submitted on this day.

June 02, 2007

Construction Activity • Limited construction activities commenced on-site today. Crew has completed

assembly of all heavy machinery on-site including crane on high chain abutment. • Crane placed concrete lock block abutment for the launch of first 50-foot span of

temporary bridge as per the “Emergency Site Isolation and Flood Mitigation Plan for the Iskut River Crossing Galore Creek Mine AccessRoad” and (see Appendix A photos).

Environmental Issues/Concerns

• No environmental issues or concerns on-site this day. The portable generator and air compressor were placed near top of riverbank in preparation of bubble curtain usage. River water levels have risen throughout the day. Based on current weather conditions during this week, June 2 was the warmest day on record, which would account for the rise in water levels.

Construction Activity • Construction started on this day. Crew has begun installing temporary work

platform. Water levels have risen several centimeters over the course of the day. • Crew began removing concrete lock block abutment as the river was beginning to

scour beneath the abutments. The current design was unsafe due to the stability of the temporary work platform. The temporary abutment design will have to be redesigned due to the high waters (see Appendix A photos).

Environmental Issues/Concerns

• Environmental issues and/or concerns on-site this day. The concrete lock blocks had to be removed due to rising water levels within the Iskut River. Background turbidity levels during the removal of the lock blocks were already high, little changes in turbidity were observed while removing the lock blocks.

• The bubble curtain that was initially installed at the center pier location and pinned with concrete lock blocks has moved downstream as a result of the fast moving river current and high waters. The bubble curtain will have to be retrieved at a later date when the waters receded. Please refer to “Site Construction Plan for Remediation Works” discussed in Section 3.2.4, 3.2.5 respectively.

• A new bubble curtain will have to be designed as a result of losing the other bubble curtain in order to proceed with instream works and construction timeline.

• Silt fencing was installed at the base of the high chain approach as a proactive measure due to the heavy rains incurred over night. No signs of erosion or sediment entering the Iskut River

June 04, 2007

Construction Activity • Stop work order issued on low chain approach due to the lack of design for jump

span approach and all instream works occurring on the high chain approach must also cease as per personal communication with Scott Davidson and Dianna Stoopnikoff (GCMC).

Environmental Issues/Concerns

• Stop Work Order issued until a new construction plan and practices are submitted to GCMC by Formula showing a new design and mitigation measures to install temporary work platform. Formula removed all remaining temporary work platform materials from the Iskut River prior to the issuance of the Stop Work Order by GCMC.

June 05-09, 2007

Construction Activity • Stop work order still in effect. The contractor has not been on-site for the past 4

days. EM ensured that all sediment and erosion control measures were functioning as intended throughout the shutdown period. Site was stable throughout the shutdown.

Environmental Issues/Concerns

• No environmental issues noted throughout this week.

• Stop Work Order still in effect. Crew working outside wetted on top of bank mobilizing building materials for both approaches. No heavy machinery has been working at or near the Iskut River.

• Site has been monitored throughout the shutdown period. All sediment and erosion control measures were functioning as intended

June 15-16 Construction Activity

• Stop Work Order has been lifted, contractor now allowed to continue working on both approaches to the bridge. Crew commenced pile driving of temporary work platform piles following the guidelines set forth in the “Emergency Site Isolation And Flood Mitigation Plan for the Iskut River Crossing Galore Creek Mine Access Road” (May 2007) approved by GCMC and DFO.

Environmental Issues/Concerns

• A qualified construction monitor continuously monitored the site. Turbidity readings were taken throughout the day in coordination with the pile driving activities. First set of piles to be driven were located within Iskut back channel.

Environmental Issues/Concerns • Contractor advised to have spill containment for welders when placed within the

Iskut River channel regardless of location (i.e. dry gravel bar). Contractor quickly outfitted welders with a secondary means of containment and also had spill kits on site as a proactive measure.

June 19-23, 2007 Construction Activity

• Contractor continued driving temporary work platform piles on these days on the high chain approach. Once this procedure was complete contractor started building the diaphragm/jig as a template for the main pier grouping.

• Contractor has excavated the low chain in order to proceed with installation of jump span.

• Excavated footing location to pour mudsill and completed concrete formwork for spread footing.

• Contractor approached by GCMC to provide a concrete wastewater management plan. In turn the contractor asked that WestCan write up a plan covering all of the associated environmental issues concerning the cast in place concrete procedures.

• Concrete Wastewater Management plan submitted to GCMC during this period and accepted.

Environmental Issues/Concerns Low chain approach

• Contractor approached WestCan to write and develop a concrete wastewater management plan.

• Plans submitted to GCMC and approved by Scott Davidson and Dianna Stoopnikoff as per conversation with DFO. Contractor began concrete pour on June 23 at 09:00 hours.

• A pre-work was held amongst the contractor, concrete subcontractor and Rescan that morning. Site isolation was completed prior to the concrete pour,

• Contractor had CO2 canister and necessary deployment equipment on site (see Appendix A Photos)

• Continuous pH monitoring was completed during the concrete pour. No noticeable differences in pH were detected. Please refer to Appendix C for pH values recorded. No concrete or concrete wastewater was introduced to the Iskut River.

High chain approach

• Contractor experiencing difficulties due to buried concrete lock blocks. A pair of lock blocks was removed from the main pier grouping location. These lock blocks had been buried during the earlier high water event that occurred on June 3rd and 4th of this month.

June 24-28 Construction Activity

• Contractor continued driving piles at the main pier grouping location. • Bubble curtain was used during all instream pile driving • Concrete formwork was installed to pour second lift of concrete over mudsill at

low chain approach Environmental Issues/Concerns

• No environmental concerns noted on site during these days, all works that took place at both approaches were completed in isolation of the Iskut River.

• The second concrete pour was not monitored for pH, as the mudsill was located above the natural ground water table at the time of the pour and naturally isolated from the Iskut River (large rock formation and surrounding earth walls

• All pile driving occurred in isolation as contractor used bubble curtain, turbidity monitoring was conducted during all subsequent pile driving activities.

• All instream pile driving was completed during this recording period

• No environmental issues or concerns were noted during the entire procedure (refer to Appendix B for Turbidity Data)

July 01 Construction Activity

• Contractor continued with installation of jump span. Piers were bolted and cross-braced; the entire spread footing was then backfilled and compacted as per the engineered drawing. Once backfilled the ballast wall was then fastened to the jump span pier grouping.

• Main pier grouping was aligned and the bent cap was welded onto existing piles

Environmental Issues/Concerns • No environmental issues or concerns noted today

July 02-03 Construction Activity

• Contractor continued working on main pier grouping stiffening main pier grouping and welding cross bracing as per engineered drawings. Preparation for bridge launch from low chain approach.

• High chain abutment piles were also driven during this period. No isolation was required as the piles were located well out of the Iskut River.

• Installation of stiffeners to cross bracing. • Girders were bolted together. Engineer on-site to inspect that the bolts were

torqued to adequate pressure • Construction of a trail within existing construction footprint to the abutment

face for crane access to launch the swift water rescue boat.

Environmental Issues/Concerns • Silt fence was installed along the upstream side of the low chain approach to

prevent any runoff from reaching riparian area and the Iskut River.

July 16-18, 2007 Construction Activity

• Continuation of bridge launching. One section was pushed out today but bridge and the rollers need to be lifted to proceed any further.

• A set of bridge rollers was transported from low chain to high chain to be installed on the high chain abutment to aid in landing the bridge.

• Girders were fastened to abutments to create a stable safe platform on which to place bridge decking.

• An approved riprap source at 6.5km was used to supply the necessary armoring for the low chain abutment.

July 20-21, 2007 Construction Activity

• Temporary work bridge was being removed and readied for transportation to the construction site at the More Creek Bridge location

• Spread footings on the low chain side are backfilled and returned to a natural state.

• High chain approach to bridge is being developed to join bridge with high chain roadway.

• Works still outstanding within the Iskut River channel include removal of temporary work piles, before this can happen the sheeting must be completed on the pier grouping.

Environmental Issues/Concerns

• Removal of work pilings will only go ahead once water levels have receded due to safety and environmental concerns.

• Both high and low chain approaches need a birding and archaeological survey prior to removing material for road construction.

• All work occurring outside of wetted perimeter, no concerns noted.

• Low chain approach was being filled with material and compacted. Each layer of fill is approximately 300mm as per engineeredspecifications.

• High chain approach is awaiting fill material to build the required ramp to the bridge deck.

• Riprap is placed along low chain approach to protect fill material in event of heavy rains and or high water. The riprap is locked in place with the excavator and is placed on top of filter cloth/geotextile fabric as per requirements.

• Bridge running decks are resurfaced in anticipation of future traffic.

Environmental Issues/Concerns • No instream works were taking place on this day within the wetted perimeter of

the river so no concerns were noted.

• Crew was reminded to pack all garbage offsite to designated disposal facilities on a daily basis not to attract wildlife.

• Also noted that Koehring Carne located at low chain approach staging area was seeping motor oil. Contractor was instructed by EM to remove off site as the machine was not suitable to work on-site. Contractor had it removed shortly thereafter (refer to Appendix A Photos).

• Safety meeting on-site to discuss usage of manlift. • Manlift was utilized to lift worker to bottom of bridge decks to secure deck

panels to steel girders using a 6 inch threaded lag bolt. Four bolts per deck were inserted.

• Imported fill material was located on the high chain side and was deposited and compacted to build up the approach elevations to the existing bridge height and tie into engineered drawings of the road flare.

anything into river or to drive manlift in any wetted areas. July 30-August 04, 2007 Construction Activity

• High chain fill and compaction continued. McElhanney started testing compaction to ensure adequate density was being achieved.

September 02, 2007 Construction Activity

• Completion of all remaining remediation works as stated in the Formula Site Construction -Remediation Plan that included the removal of the remaining concrete lock blocks and bubble curtain from the Iskut River.

• Westcan’s Doug Stokes on-site to monitor all instream works and to sample turbidity.

• The excavator was visually inspected for leaky hoses and fittings. The excavator tracks were cleaned and walking lumber pads were utilized on the dry gravel bar minimizing its footprint.

• Once at the rivers edge the excavator lifted the two lock blocks and bubble curtain out with slow controlled lifts. The resulting sediment discharge was minimal and recorded later in this report under turbidity monitoring (see Appendices A and B). No fish were observed in the area during the removal of the construction materials

Environmental Issues/Concerns

• Walking the excavator along the gravel bar had limited impacts, these impacts were kept to a minimum by the cleaning of the tracks and walking the machine on the lumber pads.

• Turbidity was kept low by slow and controlled lifts and the use of silt fencing around the concrete blocks as described in the Remediation Works Plan.

September 09-12, 2007 Construction Activity

• Riprap work completed on low chain approach, the fill was layered and compacted and riprap was placed as a protective measure

• Filter cloth was placed between riprap and all fill and natural areas. • Archaeologist Mike Wills was on site to re-establish fencing around the

chance finds archaeological sites found near the Iskut Bridge location. • An extensive site clean up was conducted removing all wooden waste, scrap

steel and geotextile cloth and disposed off site

Environmental Issues/Concerns • The archaeological sites fence was repaired by Mike Wills to protect any

historical areas or artifacts located within its boundaries.

• A site clean up was conducted to remove all waste materials to improve safety and clear the site of all debris.

4.0 FINAL SITE AUDIT A final environmental site audit was carried out on September 20, 2007. The audit consisted of examining general site conditions and site-specifics: stream channel and banks, sediment and erosion control measures, and further actions required. No significant environmental concerns were noted during the final audit. The site still required seeding, as the earthworks contractor
