Toward the end of each Afterschool Academy and Summer Academy Scholars AND Parents are eachasked to complete a 360 Evaluation Survey. Parents provide information about their Scholar’s academicbehaviors, skills, and attitudes when completing the 360 survey; while Scholars provide information about 

their own academic behaviors, skills, and attitudes when completing the 360 survey.

Both Parents and Scholars have 2 options for completing their 360 surveys:

1.  If a parent or Scholar has an active e-mail address; they will receive a link that directs them totheir 360 survey. From here they can complete and submit the survey; and are then finished.

2.  Parents and Scholars can complete their 360 surveys on-line from the Scholar’s profile in ISIS.However, we realize there may be cases when Parents and Scholars may need to complete 360surveys in paper form. If this is the case contact the Center Director for your Scholar’s Achievement Center.

Parents and Scholars: Logging int o I SI S to Complet e your 36 0 Survey 

1.  Go to http://www.higherachievement.org/ 

2.  Click on Login in the upper right corner of the screen:

3.  Enter your Scholar/Parent Username and Password:Username: FirstnameLastname@ha (* Note: there could be a 1, 2, or 3 afterFirstnameLastname…depending on how many other scholars have the same name; inthese cases, for example, Username: FirstnameLastname1@ha).Password: Welcome1 (*Note: may be different if you have changed the password.)

* Note: if this is your first time logging into your account, one of the Center Staff canprovide you with the correct Username and Password information if this information doesnot work.

4.  If this is your first time logging into the account, you may be prompted to change your Password.In this case enter Welcome1 for “Old Password”, “New Password” and “Confirm Password”. Click Submit button when finished.

5. Once logged into the system; click on 360 Survey tab located on the left side of the screen.

5.  Now you are ready to complete your survey. In the table titled Complet e Current 360

Surveys:a.  PARENTS: Click on the Complete Survey link that corresponds to the row with the

Responder indicated as “Parent”.

b.  SCHOLARS: Click on the Complete Survey link that corresponds to the row with theResponder indicated as “Scholar”.

6.   You will then be directed to the first of nine screens where you are asked questions about yourScholar’s academic behaviors, skills, and attitudes. At t he end of t he survey please

rememb er to click on Subm it t o complet e the survey.
