1. Health and Safety Policy

Our Health and Safety Policy is to provide and maintain safe and healthy working conditions, equipment and work systems for our employees, sub-contractors and to provide the information, training and supervision needed for this purpose. We also accept responsibility for the health and safety of other people who may be affected by our activities.

Under the Health and Safety Work Act 1974, the company has a responsibility:

• To provide and maintain safe and healthy working conditions, taking in to account any statutory requirements.

• To provide sufficient information, training and supervision to enable employees to perform their work safely and efficiently.

• To make available all necessary safety devices and protective equipment and to supervise their use.

• To maintain a constant and continuing interest in all matters of health and safety, in particular, by consulting and involving employees wherever possible.

Employees have a duty to co-operate:

• By working safely and efficiently, showing due regard to both personal safety and safety of others.

• By using the protective equipment provided, and by meeting statutory obligations.

• By reporting incidents that have led or may lead to injury or damage. • By adhering to the company procedures in place to ensure safe working

conditions. • By assisting in the investigations of accidents and incidents with the objective

of introducing necessary measures to help prevent re-occurrence.

2. Management of Health and Safety

3. Health and Safety Organisation Chart The Health and Safety Management meet quarterly in order to consult with staff to review the company’s Health and Safety performance and consider information from safety audits, risk assessments and any accident and incident reports.

4. Emergency Procedures and Rules All staff are to adhere to the Company Health and Safety policy as condition of their employment.

a) Accidents CALL AN AMBULANCE IMMEDIATELY IF A PERSON IS SERIOUSLY INJURED

AND NEEDS MEDICAL HELP

When an incident happens employees should seek advice from the appointed First-Aid Representative who will provide First-Aid and call fro medical assistance if needed. All accidents will be reported to the Company Director or Appointed Health and Safety Representative and recorded in the Company Accident Book, which is kept at reception, by way of completing the Accident and Incident Report Form (see appendices) and must be completed giving date, method of RIDDOR (Reporting of Injuries, Diseases and Dangerous Occurrences Regulations 1995), time and place of accident, personal details of those involved and a brief description of the nature of the accident or disease. The Company Director or Appointed Health and Safety Representative will review the recordings and will ensure that an investigation takes place and that any action that could prevent future accidents is undertaken. Incidents – Property Damage and Near Misses Any incidents that result in property damage and ‘near misses’ will be recorded on the Company Accident and Incident Reporting Form (see appendices) and will follow the same reporting procedures in place for accidents. RIDDOR The Company Director will complete the formal notification required under RIDDOR (Reporting of Injuries, Diseases and Dangerous Occurrences Regulations 1995)

clearly could have done, then it may be a dangerous occurrence, which must be reported immediately (by telephoning the Enforcing Authority). Send a completed accident report form (F2508) within 10 days

A list of Major Injuries and Dangerous Occurrences are listed in Appendix 2. Forms F2508 and F2508A are available on reception. The Company Director or the Appointed Health and Safety Representative should be contacted and told immediately if an accident needs to be reported under these regulations. Contact details from RIDDOR reporting are shown in Appendix 1.

b) First-Aid Requirements The company produces media for broadcast transmission from 1 office location with under 20 employees. The production department follow first aid requirements as specified for each location. Following an assessment of medical requirements of any location, premises or venues where the company are in control of their business activities, the following controls as appropriate will be in place:-

• A first aid box is provided in the kitchen in the first aid cupboard, please ask Simone for the key. The contents of this are specified in the Health and Safety (First Aid) Regulations 1981 (see appendix 3).

• Designated company personnel are suitably trained to an occupational first aid standard.

• No company first aid person will dispense medicines or pills other than laid down in the appropriate legislation guidelines.

• The company first aid person(s) will be responsible for keeping all equipment and medical supplies stored in the appropriate manner and kept up to date in respect of quantity and quality.

• On location a unit nurse might be required. This will be a suitably qualified person who will be in total charge of dispensing medicines etc. All medical equipment will be in their sole charge.

• Notices telling all personnel of the availability of first aid etc. and the names and location of the qualified persons will be prominently displayed at all locations etc.

• Appropriate recordings to include an official accident book (B1 510) will be kept and its content made available for inspection to any authorised person or external inspecting authority.

• Adequate assistance will be provided to cover for any holidays or sickness to first aid personnel.

Members of staff responsible for first aid are Sheryl Trinh and Simone Charles. Their identities have been notified to company employees.

c) General Fire Safety The following control measures will be in place to help prevent a fire or to mitigate the effects if fire were to break out in any of the premises and locations occupied by the company. Where there is a shared responsibility for fire prevention and precautions, e.g. with venues, multiple occupancy buildings, locations etc., the fire officer who is responsible for organising fire and evacuation procedures will ensure all parties are aware of their responsibilities and the arrangements are in force at all times. The fire officer keeps a fire log, which is regularly updated.

• All fire instruction notices are prominently displayed in the designated positions. • All escape routes are clearly signed and appropriate emergency lighting is in

place. • All escape routes, fire lanes etc. are kept clear of obstructions at all times. • All designated fire exit doors are kept closed when not in use and able to open

easily from the inside. • The installed fire alarm system is working correctly and all persons under the

control of the company are familiar with its operation. • All equipment provided for fire detecting and fire fighting are in place, not

obstructed and evidence is provided that there has been a regular inspection of these appliances etc. The fire officer has the numbers, types and positions of all office based fire extinguishers

• Designated assembly points following an evacuation have been clearly established and all personnel are aware of their individual responsibilities regarding the evacuation procedure.

• All ‘No Smoking’ areas are clearly signed and a regular monitoring system is in place to ensure compliance.

• An office based fire evacuation drill is carried out at least twice per year. • All heating and electrical appliances are fit for the purpose intended, and

positioned in a safe manner. • Rubbish and waste are removed from the premises at the end of every working

day. Recycling boxes are emptied on a daily basis. • The supply and storage of flammable liquids, materials etc. are in compliance

with all fire regulations, home office and local authority guidelines.

d) Emergency Procedures – Other Emergencies

• In the cases of other emergencies such as bomb scared the H&S Management Representative or in her absence a nominated senior manager will take charge of the situation, reacting according to the nature of the emergency in accordance with any applicable written procedures, summoning the emergency services.

• If the emergency takes place at an external location or at a studio facility then the instructions of the host venue will take priority.

e) Training and Training Records

• Impossible Pictures Ltd arrange regular one-day recognised Health & Safety introduction courses. It is mandatory for all new employees who are offered the course during their period of employment to attend.

• Training Records will be kept in our personnel file under the individual’s records. Training requirements will be reviewed quarterly at the Health and Safety Management meetings.

• A record of training will also be recorded on the Training Record Form (see section 7) and filed by the Company Manager/Appointed Safety Representative.

f) Office Premises – Rules and Guidelines

• A generic risk assessment is in place to cover the offices at 58 Davies St, London W1K 5JF. Copies of this assessment can be obtained from the Office Manager.

• Display Equipment, employees will be entitled to eye tests if they are regular users of VDUs.

• Impossible Pictures Ltd will ensure that employees are given information as to the proper lay out of their work station in relation to the use of the VDU and in particular the positions of the individual when operating the VDU

• Impossible Pictures Ltd will understand that adequate breaks are given from VDU work. Employees will be advised of this on commencement of the employment, (and regular updates and reviews shall be made). It is the responsibility of the individual that these breaks are taken.

• Manuel handling activities must have a separate risk assessment completed by a trained risk assessor.

• Electrical safety in the office is covered in the risk assessment. All portable electrical equipment will have periodic visual inspections by a suitably competent person. All extension leads will be kept to minimum usage, and included in any periodic inspection. Where possible a fixed 13-Amp wall socket will replace an extension lead.

• Trailing cables must not lie across fire exits or across stairs of ladders. Cables will remain tidy under desks and behind cabinets etc. Regular visual inspection by a competent person must take place to ensure no damage to leads, plugs or sockets has taken place.

• All toilets, internal waste bins and food preparation areas must be cleaned daily. • Stairwells and emergency corridors must be kept rubbish free at all times. • Use and storage of materials and substances will be included in the Generic

Office Risk Assessment. • An ‘Appointed Person’ in terms of First Aid must be available to office employees

at all times. • All accidents must be reported as instructed in section 4.a

g) Production Unit Rules and Guidelines-Locations:

• A specific risk assessment identifying hazards associated with location assignments must be used and in place before departure. Refer to Production Manager for example forms.

• All medical requirements, inoculations, emergency bags etc. must be in place before departure to location.

• All ‘recce’ work must have taken place either directly or through the local ‘fixer’ before the final departure and a written report to include casualty evacuation must be in place before departure.

• A system of communication between production office and the team on the ground must be in place and responsibilities for contact on a regular basis have been assigned before departure.

• First Aid procedures must be in place to include any arrangements for lone worker emergencies.

• All codes of practice for control measures on all major hazards identified must have been circulated to those persons exposed before departure. Training, information and instructions to all crew must have taken place.

• Regular monitoring of the geographic areas of the location in respect of weather reports, long term climatic changes, political situation, civil unrest t include neighbouring countries for travel purposes, permits etc must be monitored by the Production Manager as well as the Production team on location.

• All equipment used under load conditions, e.g. ropes, hoists, shackles, carbines, etc. must have been inspected within the last 12 months and test certificated must be with the production Manager before departure. Locally purchased items must follow the same procedure.

• All local environmental, game park conservation and wild life rulings regarding endangered species must be strictly observed and the senior person on location made responsible for monitoring upkeep etc.

• Reports on all accidents, illness, damage to plant life through fires, structural collapse of hides etc. must be reported immediately to the Production Manager. All road traffic accidents involving crew must also be reported to the Production Manager.

• Under water filming locations within the UK, involving SCUBA must observe all UK diving regulations. A risk assessment of the activity must be completed by the senior person on location.

• The importance of fatigue, stress and where applicable lone working must be taken into regard when completing contracts, schedules, risk assessments, budgets and local facilities management.

In addition to the above arrangements the Directors of ‘the company’ will also use as guidelines, the PACT Health and Safety Policy in relation to productions that work away from the head office. All location work will be assessed with the PACT policy in mind.

h) Contracted Companies/Sub Contractors (also known as Loan Out Companies) :- H&S Responsibilities: • All companies where a contract for services (written or oral) exist, are employers

in their own right. They must have their own health and safety policy. When however, these companies employ fewer than 5 employees then the policy does not have to be written or recorded. Contractors however must, as employers, keep to the ‘spirit and intention’ of our Health and Safety policy and still have a legal duty to comply with all Health and Safety.

• Contractors, as employers, must assess and control any risks they create, and inform all other companies with whom they share the work place of control measures they are adopting to minimise the risk of injury to other personnel.

These assessments must be recorded if the contractor employs more than 5 employees.

• Contractors have the legal duty to supply employees who are aware of their health and safety responsibilities and have had sufficient training to carry out their work in a safe, healthy and competent manner.

• Impossible Pictures Ltd and all other contracted companies have a legal duty to inform the employees of the contractor about risks to which they might be exposed when sharing the work place with them. This sharing of information before activities commence is a combined responsibility of all employers whether or not they have less than 5 employees or are a sole trader.

• All contractors must use their own tools and equipment whenever practical. This equipment must be fit for the purpose intended and used in a safe and proper manner. All equipment given to the contractor to use, must also be safe and fit for the purpose intended.

• Impossible Pictures Ltd must afford the same welfare facilities to employees of sub-contractors as their own staff. They must also inform contractor’s employees of all emergency procedures. There must be no discrimination or lack of health and safety information because they are not the host company’s own employees.

• A copy of this Policy is available for the reference of all contractors. i) COSHH – Control of Substances Hazardous to Health

• Within our normal operations we purchase few hazardous substances. However, employees should be aware that all substances could be hazardous if used incorrectly. The general advice is to follow the guidance given on the packaging in the event of drinking, eating or splashes on the skin or in the eyes. For the substances commonly found on the premises (cleaning materials etc), the Health and Safety Management Representative will complete a COSHH Assessment using the standard form (see section 6). Freelancers using the company premises will be expected to notify the Health and Safety Representative of any chemicals they wish to use on the premises and provide a COSHH assessment if required.

j) Housekeeping

• Housekeeping is important to the safety of our operations. If we keep our working environment tidy and store things in an orderly manner there will be less chance of accidents happening. The Company Manager will ensure that:

• Good housekeeping standards are maintained on a day to day basis. • Waste materials are disposed of correctly (covered in the relevant COSHH

assessment). • Stock items are stacked safely and stored in the right location. • Gang-ways and exits are kept clear.

Good House keeping is essential, it helps us to use our working environment in a cost effective and efficient manner, tidy storage of files and stock will help us maintain an orderly office and keeps things in good condition.

k) Maintenance of Equipment • Electrical Equipment

It is a requirement under the Electricity at Work Regulations 1989 and the requirements for portable appliance testing, that all electrical equipment is tested periodically by visual inspection and electrical testing to ensure its safety. Staff will check all plugs, extension leads and cables for loose connections and faults before use. If there are any problems then the equipment should not be used and the problem reported to the Company Manager.

l) Hazard and Risk Assessments

Safe Systems of work have been developed covering the routine tasks performed by the company employees. These will be issued to all employees upon commencement of employment. Employees will receive training and advice in these Safe Systems of Work, as appropriate. Safe Systems of are developed from a detailed consideration of the hazards associated with the task and the location of the work, the health effect of these hazards, the chance of these hazards happening and the protective measures required to reduce the overall risk.

General Principles for Risk Assessment and Safe Systems of Work

Multiplying the Health Factor by the Chance Factor can assess the over all hazard risk. This gives a hazard risk between 1 and 25. When risk values are higher than 5, consideration should be given to reducing the risk of hazardous operations through the following actions:

• Avoid the hazard • Combat the hazard at source (i.e. wipe up spillage rather than applying a sign to

raise awareness of risk of slipping) • Implement procedures (e.g. reduce exposure time) • Use warning system (e.g. signs and labels)

• Use personal protective equipment only as a last resort (e.g. hearing protectors, dusk masks etc.)

In all cases consider what training is required to ensure that the operation is carried out safely. In designing safe systems of work, the fundamental principles are: Avoidance Avoid risk altogether Prevention Prevent danger arising Reduction Where risks cannot be avoided or prevented they should be reduced. REMEMBER

1) Identify the hazards 2) Assess the seriousness of the health effects of the hazard 3) Assess the chance of it happening 4) Ask who will be affected 5) What controls already exist and are they effective? 6) Decide how the overall risk could be reduced. 7) Record the safe system of work 8) Develop a training scheme 9) Ensure that the person doing the job has been trained in the safe system of work m) Annual health and Safety Review Each year the Company Directors will sit down with the Health and Safety Representatives and review the Health and Safety performance and ensure that our management procedures continue to support our policies. Where the management procedures are changed or safe systems of work are issued then the employees will be informed. The Quarterly Health and Safety Management Meeting reports will be reviewed at the Annual Health and Safety Review and a report will be drawn up to reflect the outcome of these meetings and analyse the effectiveness of any policy changes implemented. m) Display of signs It is a legal requirement that the following signs are displayed in the main office: i. Copy of the employers’ liability insurance certificates. ii. Copy of the company’s Health and Safety Policy (dated and signed by the

Managing Director). iii. Health and Safety Law poster giving health and safety information to employees

(poster ISBN 0117 01 4249) with details of the enforcing authority and the employment medical advisor for the area. A leaflet containing the same information will be given to all employees.

iv. A fire instructions notice covering: fire alarm sound and what to do when the alarm sounds as well as what to do if a fire is discovered.

APPENDIX 2 – DEFINITION OF MAJOR INJURIES, DISEASES AND DANGEROUS OCCURRENCES Reportable major injuries are:

• Fracture other than to fingers, thumbs or toes • Amputation • Dislocation of shoulder, hip, knee or spine • Loss of sight (temporary or permanent) • Chemical or hot metal burn to the eye or a penetration injury to the eye • Injury from electrical shock or electrical burn leading to unconsciousness or

requiring resuscitation or admittance to hospital for more than 24 hours • Any other injury: leading to hyperthermia, heat induced illness or

unconsciousness requiring resuscitation or admittance to hospital for more than 24 hours

• Unconsciousness caused by asphyxia or exposure to harmful substance or biological agent

• Acute illness requiring medical attention, or loss of consciousness arising from absorption of any substance by inhalation, ingestion or through the skin.

Reportable Dangerous Occurrences:

• Collapse, overturning or failure of load bearing parts of lifts or lifting equipment.

• Explosion, collapse or bursting of any closed vessel or pipe-work • Failure of load bearing parts of freight container • Plant or equipment coming in to contact with overhead power lines • Electrical short circuit or overload causing fire or explosion • Any unintentional explosion, projection of material beyond site boundary or

injury caused by explosion • Collapse of partial collapse of scaffold over 5 metres high, or erected near

water where there could be a risk of drowning after a fall • Dangerous occurrence at a well or reservoir • Unintended collapse of any building or wall or floor structure under

construction or alteration • Explosion or fire causing suspension of normal work for over 24 hours • Accidental release of any substance, which may damage health

Reportable Diseases include:

• Certain poisonings • Some skin diseases • Some infections such as hepatitis and tetanus

APPENDIX 3 – CONTENTS OF FIRST-AID BOXES AND KITS

Guidance under the Health and Safety (First-Aid) Regulations 1981 states that the content of first aid boxes and kits should contain sufficient quantities of:

• Card giving general first aid guidance • 20 individually wrapped sterile adhesive dressings (assorted sizes) appropriate

to the working environment • 2 sterile eye pads with attachments • 6 individually wrapped triangular bandages • 6 safety pins • 6 medium sized individually wrapped sterile un-medicated wound dressings

(approx 10cm x 8cm) • 2 large sterile individually wrapped un-medicated wound dressings (approx

13cm x 9cm) • 3 extra large sterile individually wrapped unmediated wound dressings (approx

28cm x 17.5cm) • Where mains tap water is not readily available for eye irrigation, sterile water

or sterile normal saline (0.9%) in sealed disposable containers should be provided. At least 900ml should be provided in 300ml bottles.

Travelling first aid kits should contain at least:

• Card giving general first aid guidance • 6 individually wrapped sterile adhesive dressings • 1 large sterile adhesive dressing • 1 large sterile un-medicated dressing • 2 triangular bandages • 2 safety pins • Individually wrapped moist cleansing wipes.
