COURSE DESCRIPTION CMT 101

This course provides an introduction of two complementary and alternative medicine (CAM) health practices; chiropractic care and massage therapy. The history, techniques and health benefits of these practices will be discussed. Presentations and demonstrations by community practitioners in chiropractic and massage therapy, are part of the course.

GRADING: Certificate of Completion provider when all three days are attended.

Provide a background of my education and years I have been a practicing licensed massage therapist. Review the syllabus and course outline, give out handouts to refer to during today’s lecture.

Explain what CAM is.

Complementary and alternative medicine, or CAM, is a category of medicine that includes a variety of treatment approaches that fall outside the realm of conventional medicine.

Complementary medicine refers to healing practices and products that work in conjunction with traditional medicine.

Alternative medicine differs in that it is not used as a complement to, but rather as a substitute for traditional therapy.

Many people with musculoskeletal problems use complementary and alternative medicine (CAM). Two types of CAM providers used for these problems are Chiropractors and Massage Therapists.

Chiropractic

Chiropractic is based on the theory that vertebral misalignment causes most neuromuscular and related functional diseases and disorders (AAOS, 2010). Doctors of Chiropractic or Chiropractors practice a drug free, hands on approach to health care. These health care professionals focus on ailments associated with the nervous system and musculoskeletal system and how these ailments can affect an individual’s over all general health. Chiropractic care is sought more commonly for complaints associated but not limited to back, neck, and joint pain as well as headaches. Almost 90% of chiropractic patients come to the chiropractor with neuromusculoskeletal complaints.

Massage Therapy

Massage is one of the most-used complementary therapies in the United States (AAOS, 2010). Massage therapy is manual manipulation of soft body tissues (muscle, connective tissue, tendons and ligaments) to enhance a person’s health and well-being. There are dozens of types of massage therapy methods (also called modalities). People seek massage therapy for a variety of reasons – to reduce stress and anxiety, relax muscles, rehabilitate injuries, reduce pain, and promote overall health and wellness (NWH, 2014).

History

The word 'Chiropractic' is derived from the Greek words cheir meaning 'hand' and praktos meaning 'done', Done by Hand (ACA, 2012). The practice of chiropractic care can be traced back to the writings of the Chinese and Greek in 2700 B.C. and 1500 B.C. These writings mention spinal manipulation and maneuvering of the lower extremities to help alleviate low back pain. Hippocrates, the Greek physician, wrote "Get knowledge of the spine, for this is the requisite for many diseases”. In centuries to follow manipulative techniques were passed down from generation to generation within families (Micozzi, 2011). These techniques referred to as ‘bonesetting” methods were passed from father to son and mother to daughter playing a role in history’s nonmedical healing. In the late nineteenth century the United States was recognized for practicing spinal manipulation.

In 1895, Daniel David Palmer, a self-educated healer, founded the Chiropractic profession in Davenport, Iowa. The two fundamental premises Palmer based his practice on are vertebral subluxation or spinal misalignment causing abnormal nerve transmission and chiropractic adjustment or manual manipulation of the subluxated vertebrae. A vertebral subluxation was believed to be the primary cause of all diseases and the adjustment is the cure. Daniel David Palmer went on to begin the Palmer School of Chiropractic in 1897.

Throughout history chiropractic has been at odds with the organized medical community. The most famous confrontation in recent years began in 1976 when four doctors of chiropractic filed suit against the American Medical Association for restricting cooperation between individual MDs and doctors of chiropractic. This case proved to be one of the biggest victories in the history of the chiropractic profession. Evidence heard in U.S. District Court ruled that over the previous 25 years the actions of the A.M.A. and its coconspirators, the American College of Radiology and the American College of Surgeons, had resulted in serious damage to the cooperative process in health care, to the profession of chiropractic as a whole, to individual doctors of chiropractic, and to the patients they served. Studies introduced at the trial showed that doctors of chiropractic were twice as effective as medical doctor, showing orthopedic patients treated in hospital settings were discharged from the hospital in five to seven days, compared with an average of 14 days without chiropractic care. The court declared that the A.M.A., as far back as 1963, was working aggressively to "overtly and covertly" eliminate the profession of chiropractic. Judge Getzendanner issued a permanent court injunction against the A.M.A. to prevent such future behavior (Farr, 2002). This ruling established the first criteria to accredit chiropractic colleges and schools. Twelve schools were accredited that year. To date there are 17 accredited chiropractic colleges in the United States. In a little over 100 years chiropractic grew from a small, at times unacceptable and unrecognized profession, to the third largest independent healthcare profession in the Western world (Micozzi, 2011).

As a Profession

To help achieve a level of optimal health a chiropractor uses hands on techniques to manipulate, massage, mobilize, adjust, stimulate, apply traction to, provide myofacial release, cranial adjusting, and trigger point therapy. Prior to these techniques being used a chiropractor will conduct a series of diagnostic tests to insure the patient doesn’t present with signs of fractures, muscle tear verse a strain or sprain or any underlying systemic disease. A chiropractic analysis can consist of any or all of the following practices. A patient’s history intake, palpation or hands on touching of the area of pain, range of motion testing to determine any limitations of the joints or muscles, X-ray analysis, thermal scan or SEMG to measure the abnormal nerve function of the body, and applied kinesiology. Kinesiology focuses on the muscle symmetry and strength and the relation to the nervous system, visceral function, and the pathways. "The human body is a machine run by the unseen force called life, and that it may be run harmoniously it is necessary that there be liberty of blood, nerves and arteries from their generating point to their destination" (Palmer, 1902) .

Overview of Massage Therapy History Techniques Benefits in combination with Chiropractic Demonstration

HistoryMassage therapy dates back thousands of years. References to massage appear in writings from ancient China, Japan, India, Arabic nations, Egypt, Greece (Hippocrates defined medicine as "the art of rubbing"), and Rome. Massage became widely used in Europe during the Renaissance. In the 1850s, two American physicians who had studied in Sweden

introduced massage therapy in the United States, where it became popular and was promoted for a variety of health purposes. With scientific and technological advances in medical treatment during the 1930s and 1940s, massage fell out of favor in the United States. Interest in massage revived in the 1970s, especially among athletes (MedicineNet, 2014).

Techniques

Two fundamental categories of massage are:

Relaxation massage - also known as Swedish massage; practiced in settings like spas, wellness centers and resorts

Rehabilitative massage – also known as deep tissue, medical, therapeutic or clinical massage; practiced in many settings like clinics, hospitals and chiropractic offices

½ hour DEMONSTRATION OF TECHNIQUES

Swedish massage/five basic strokes, effleurage (stroking) petrissage (muscles are lightly grabbed and lifted) friction (thumbs and fingertips work in deep circles into the

thickest part of muscles) tapotement (chopping, beating, and tapping strokes) vibration (fingers are pressed or flattened firmly on a muscle, then

the area is shaken rapidly for a few seconds).

Benefits in combination with Chiropractic

When chiropractic care and massage are combined the benefits are:

1. Quicker Recovery – Patients who receive both therapies will often experience a faster and more complete recovery. The spine, soft tissue, and all supporting structures rely on each other to function properly, and the use of both chiropractic care and massage allows them all to be addressed and treated appropriately.

2. Easier Adjustments – Massage allows for relaxation of muscle tension allowing chiropractic adjustments to be performed more easily and successfully.

3. Increased Range of Motion – The goal of chiropractic care is generally to provide increased comfort and mobility. In addition to pain, many chiropractic patients also suffer from reduced range of motion due to their injury. Massages are able to improve the

flexibility and mobility of impacted areas such as muscles, ligaments, and other soft tissue.

4. Improved Circulation – Massage techniques are often found to enhance blood flow through congested areas and relieve lactic acid, while chiropractic also helps by relieving pressure on nerves due to subluxations.

In addition to these benefits, massage and chiropractic care may also work together to;

Improve posture Manage pain caused by conditions such as arthritis or fibromyalgia
