Student Registration Form

HOME LANGUAGE SURVEY (on Student Registration Form)

Consists of three questions:

1- What was the first language learned?2- What is the language spoken most often?3- What is the language spoken at home?

IDENTIFYING PHLOTE STUDENTS (Primary Home Language Other Than English)

Federal regulations state that any students whose answer is a language other than English, to ANY of the three Home Language Survey questions on the Student Registration Form is considered a PHLOTE student. PHLOTE students must be referred for language proficiency assessment.

Informal Student Assessments

As soon as the student is identified as PHLOTE by the Home Language Survey, and is a student arriving from a foreign country, student should be given the Informal Math and Writing assessments while parents complete registration forms. A quite place should be designated for these student assessments.

Students transferring within the U. S., or Kindergarteners do not need to take this assessment.)

International Student Profile Form

This form should be completed only for students entering the U.S. from a foreign country. Students transferring within the U. S., or Kindergarteners do not need to complete this form.

The International Student Profile Form: Provides educational history, interrupted schooling or gaps. Languages of instruction. Identifies special interests and talents. Student behavioral or learning problems in previous

schools.

Explanation to Parents

If student is identified as PHLOTE, registrar explains to parents that a language assessment will be administered, and that the teacher will send notification informing parents if the student is eligible for ESOL services or not. (Show the parent a copy of the notification form so that they know what to expect.) Upon receipt of the form, parents will need to sign and return the form to the teacher.

Important Points to Remember

Front office must refer PHLOTE students to ESOL teacher for testing immediately!

Language Assessment must be administered within 10 days of enrollment.

TRANSFER STUDENTS WITHIN GEORGIA

If a student is transferring from another Georgia district, school should contact the former school district to obtain ACCESS or recent W-APT scores so that the W-APT will not be administered unnecessarily.

Note: A list of Georgia school system contacts is provided

to help schools obtain scores for transferring ELLS.

Determining Eligibility for Language Assistance

ESOL teacher or designated staff administers W-APT to determine eligibility.

List all students assessed on the PHLOTE Eligibility Assessment Report.

Not Eligible Students If test score determine that a PHLOTE student is

NOT ELIGIBLE:

1. Teacher completes the Notification of English Language Assistance Program Placement Form and sends the form to parents. Keep a copy in permanent record.

2. ESOL Teacher completes the Data Entry-ESOL Assessment/Status Roster and forwards the roster to the Student Information System (SIS) administrator on a weekly basis. SIS administrator forwards the information to the Data Entry Clerk for entry in the Student Information System.

3. ESOL Teacher completes blue ESOL Not Eligible Form and places it in the permanent record folder.

4. W-APT tests of non-eligible students should be maintained for one year.

Eligible StudentsIf test score determines that a PHLOTE student

IS ELIGIBLE:

1. Teacher completes the Notification of English Language Assistance Program Placement Form and sends the form to parents for their signature stating acceptance or waiver of language assistance services.

2. ESOL Teacher completes the Data Entry-ESOL Assessment/Status Roster and forwards the roster to the Student Information System (SIS) administrator on a weekly basis. SIS administrator forwards the information to the Data Entry Clerk for entry in the Student Information System.

3. ESOL Teacher starts an ESOL Student Portfolio (Blue Folder) for each student.

3. On permanent record “Blue” must be coded to indicate student is ESOL eligible.

Migrant Parent Survey Form

Also known as “the green form”.

This quick survey determines if the enrolling student may qualify for additional educational, and/or supportive services provided through the Migrant program.

This form is for all students (not just PHLOTES).

Withdrawal Forms

Front office must assure that Withdrawal Forms are sent to ESOL teachers for completion of ESOL section:

So that students can be placed appropriately at new school. ACCESS/W-APT information is crucial.

ESOL teacher immediately places ESOL Student Portfolio in permanent record to send by courier to new school within our district.

In case the students are moving out of district, the record will be housed at the school, and copies will be made to forward accordingly.
