INTRODUCTION
Diabetes is a major public health problem world-wide. It currently
affects 246 million people and this is expected to increase to 380
million by the year 2025. The WHO estimated a rise of 170% in the
incidence of diabetes in the developing countries. This figure accounts
for 75% of worlds diabetic population. It is obvious that the developing
countries will bear the brunt of diabetes epidemic in current century.
In the year 2005, an estimated 101 million people died from diabetes
and about 80% of these deaths occurred in low and middle income
countries. WHO projects that deaths due to diabetes will increase
more than 50% in the next ten years if urgent measures are not taken.
Individuals with diabetes are 25 times more likely to become blind
than individuals without this disease. In many countries diabetes
retinopathy is the leading cause of new cases of visual impairment
and blindness among adults aged 20 74 years. Individuals with
diabetes have a higher risk of other complications such as nephropathy,
neuropathy, cardiovascular and cerebrovascular disease. Thus diabetes
and its complications impose significant economic consequences on
individuals, families, health system and countries.
On the other hand, UKPDS epidemiological data in type 2 diabetes
mellitus proved that every 1% reduction in mean HbA1c translates
to significant reduction in diabetic complications. 37% microvascular,
21% any endpoint related to diabetes, 21% in deaths related to diabetes,14% in fatal and non-fatal myocardial infarction and 43% amputation
or deaths from peripheral vascular disease.
